Position Summary: The Nocturnist Advanced Practice Provider (APP) provides care to complex acute, critical and chronically ill patients and manages the plan of care. The APP is experienced and works autonomously, as well as in collaboration with other health care professionals, to diagnose and manage patients' acute and chronic health care needs. APP patient coverage requires regularly scheduled nights, including weekends, holidays and on-call obligations. The nocturnist position is a full-time night commitment with a reduction in total number of hours per month when compared to day team members.
The Newborn and Infant Critical Care Unit (NICCU) is a sophisticated 58-bed unit that provides a complete range of medical and surgical services for neonates and infants with the most complex conditions. Our unit serves as a regional referral center for the western United States as well as an international referral center. Our specialized programs include Fetal & Neonatal Surgery, Extracorporeal Membrane Oxygenation (ECMO), Whole-Body Therapeutic Hypothermia, Congenital Diaphragmatic Hernia, complex surgical specialties, highly specialized ventilatory support & home ventilation program, Newborn Follow-Up, and more.
Job Responsibilities:
Assesses the complex acute, critical and chronically ill patient for urgent and emergent conditions. Uses relevant data to evaluate for physiologic instability. Utilizes the data to formulate a plan of care.
Performs therapeutic interventions to stabilize acute and critical health problems.
Analyzes the indications, contraindications, risk of complications and cost-benefits of therapeutic interventions.
Manages the plan of care through evaluation, modification and documentation according to the patient's response to therapy. Manages changes in condition with a focus on optimizing the patient outcomes.
Facilitates the patient's transitions between and within health care settings.
Serves as an educational resource to the patient/family, nursing staff, and provider team. Acts as a consultant to multidisciplinary health care team members.
Participates in scholarly activities including research and education. Participates in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate educational offerings. Participates in the professional development of others. Acts as a preceptor and mentor to support the development of nurses, advanced practice providers, students, and other health care providers.
Demonstrates a commitment to patient safety by speaking up about safety issues and changing practice to enhance safety. Shares errors or potential errors through system reporting mechanisms. Demonstrates shared accountability for safe practice.
Minimum Qualifications: NICU clinical experience is required.

For NNP: Graduate of a NLN accredited graduate program and NCC certification
For PA: Graduate of an ARC-PA accredited graduate program and NCCPA certification

• Make resident rounds at the beginning of each shift and every two hours thereafter to administer quality nursing care.
• Verify the identification of a resident before administering nursing care or serving food, to assure that appropriate care is being provided to the correct resident. Report missing ID bands to admission coordinator immediately.
• Make beds, clean bedside and overbed tables.
• Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident's reach. Pass water pitcher as per established policy and procedure.
• Dress residents neatly and in their own clothing. Assist residents in dressing as necessary.
• Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.).
• Serve nourishment in accordance with established facility procedures.
• Feed residents who cannot feed themselves.
• Assure that resident's food is accessible and self-help devices are available as needed.
• Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities.
• Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary.
• Do not permit residents to smoke in bed.
• Assure that the nurse's call system is attached to the bed and within easy reach at all times when residents are bedfast.
• Answer residents' call lights promptly.
• Check for food in residents' room. Ensure that food is kept in a sealed container.
• Report all complaints to the Charge Nurse.
• Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.)
• Report any medication found in the resident's room. Take to the Charge Nurse immediately.
• Report all hazardous conditions and faulty equipment to the Charge Nurse immediately.
• Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately.
• Attend in-service educational programs, on the job training programs and meetings as directed.
• Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.
CLINICAL
• Wash hands before and after caring for each resident.
• Place residents in correct and comfortable position. Head of bed elevated at 30° for all residents on enteral feeding.
• Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner.
• Record resident's food and nourishment intake as directed.
• Perform after meal care as required - cleaning resident's hands, face, clothing, dentures, etc.
• Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed).
• Check and document bowel movements and character of stools every shift.
• Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities).
• Comb and brush resident's hair daily. Shampoo hair on shower days.
• Clip and trim fingernails.
• Shave male residents daily or as needed.
• Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed.
• Observe and report the presence of reddened areas on bony prominences.
• Keep drainage bag off the floor when resident is in bed or in wheelchair.
• Apply Vaseline or A & D ointment to dry skin in accordance with established policies.
• Move and turn bedfast residents at least every two hours and record such information on CNA notes.
• Observe disoriented and comatose residents and notify Charge Nurse immediately.
• Apply postural support properly. Check with Charge Nurse for type of restraint to be used.
• Apply hand roll or splint to residents with contractures as outlined within the facility's established policies and procedures.
• Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary.
• Chart required information every shift.
• Complete Intake and Output documentation on all residents with orders before the end of each shift.
• Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator.

The successful candidate will be responsible for generating revenue by developing new territory, establishing strong referral networks, signing up clients for care services, and securing contracts to support our agency's growth.
Key Responsibilities
Develop and Maintain Referral Network (Ongoing, Weekly KPI):
Conduct face-to-face touchpoints with a minimum of 75 referral sources weekly to maintain strong relationships. These referral partners will serve as a pipeline for new client leads, with success measured by referrals translating into signed clients and billable hours.
Grow Client Base through Referral Networks (First 4 months):
Establish relationships with healthcare providers, social workers, and community-based organizations to sign up new clients. By month six, you should be generating 30-50 hours of billable care per week through your own referral networks. Success is measured by sustained client acquisition and weekly billable hours.
Collaborate with Care Team for Client Success (Ongoing):
Work closely with the internal care team to ensure the successful onboarding and satisfaction of new clients. Act as a liaison between clients and the care team to optimize service delivery, ensuring all clients receive high-quality care from day one.
Daily/Weekly Activities
Making outbound calls and visits to potential referral partners and clients- Goal 75 faces a week
Scheduling and conducting meetings with referral partners
Following up on leads and maintaining a detailed CRM
Attending networking events and community functions
Preparing and delivering presentations to potential clients and partners
Sell and sign up prospects to be clients
Collaborating with the caregiving team to address client needs
Tracking and reporting on sales activities and performance metrics

Requirements
Preferred Qualifications
Masters in Social Work- MSW (preferred)
2+ years marketing and sales experience in home care, home health, hospice, assisted living, hospital or related organizations
An established and working network of home care referral sources
Proven track record of generating $600,000 in annual revenue
Experience developing new territories from 0 hours to 400 weekly hours
Consistently achieved 100% of monthly and quarterly quotas
Secured at least one contract (staffing or provider) per quarter
Established a book of 25 referral partners from scratch
Successfully signed up clients who were arranging for care
Demonstrated ability to build revenues through cold calling and referral relationships
Increased billable hours from client referrals by 40 hours quarterly
