    Outside In is seeking a compassionate and skilled Medical Provider - either a Physician or an Advanced Practice Provider (NP or PA) - to join our integrated, mission-driven team. This position provides primary care to individuals of all ages and backgrounds within our Federally Qualified Health Center. Our clinics offer comprehensive acute and chronic care, with an emphasis on equitable, trauma-informed, and culturally responsive services. The provider will deliver patient-centered care that includes diagnosis, treatment, and management of acute and chronic conditions, preventive health services, and health education. Working collaboratively within a multidisciplinary care team, the provider will promote continuity of care through joint planning and problem solving with internal and external partners. Advanced Practice Provider Starting Salary Range: $131,421.66 to $145,064.93 per year Physician Starting Salary Range: $225,860.02 to $249,307.20 per year Advanced Practice Provider Essential Duties * Provide patient-centered care in a certified medical home model. * Provide a wide range of health care services including the diagnosis and management of acute, chronic, and complex health problems, health promotion, disease prevention, health education, and counseling to individuals, families, groups and communities. * Perform history taking, physical examinations, diagnosis and therapeutic treatment planning consistent with evidence based practice guidelines, clinical policy and procedures, and the scope of practice. * Perform minor procedures, biopsies, and pelvic exams. * Order and interpret diagnostic lab and radiologic tests assuring that results are communicated to individual patients or a parent/guardian of the patient. * Share after-hours call with other staff providers. * Meet all regulatory obligations of the Health Center's medical practice: FTCA, CLIA, HIPAA, State and Federal regulations and other appropriate regulatory bodies as applicable. * Fully participate in the clinic and medical staff development through involvement in service evaluations, health care plans, chart reviews, and program development. * Act as a resource to the care team and communicate appropriate knowledge, skills, and conduct. * Actively participate in quality improvement initiatives aimed at improving population health outcomes. * Maintain a record of assessments, plans, treatments and referrals. * Support the organization as needed. * For Physician Assistants: Enter into a Collaboration Agreement under the Collaborative Practice Model as outlined in OAR 847-050-0080 Physician Essential Duties * Provide outstanding acute and chronic care to low income children and adults who have limited resources * Work in collaboration with multidisciplinary provider team (allopathic, naturopathic, acupuncturist, dentists) at OI to provide innovative and evidence-based care * Maintain specialized clinical skills as applicable, e.g., minor procedures and biopsies, pelvic exams, blood draws * Provide medications management for low acuity mental health patients in conjunction with behavioral health and psychiatric consultation as appropriate for a primary care setting * Maintain a panel of Suboxone opiate replacement therapy patients in conjunction with the agency's alcohol and drug treatment case management program (may obtain waiver after employed) * Share after-hours call with other staff practitioners * Meet all clinical obligations of the Health Center's medical practice: CLIA medical, HIPAA, State and Federal regulations, etc. * Participate in the Health Center's Quality Improvement and care delivery changes in coordination with regional health reform initiatives * Fully participate in the Clinic and medical staff development through involvement in service evaluations, health care plans, chart reviews, and program development * Act as a resource and communicate appropriate knowledge, skills, and conduct * Maintain a high level of quality, accuracy, and ethical behavior * Prioritize customer service and satisfaction * Master the Health Center's electronic medical record and other technologies supporting quality patient care and disease management.

Essential Duties • Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary. • Use only authorized abbreviations established by this facility when recording information. • Report all changes in the resident's condition to the Nurse as soon as practical. • Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner. • Report all accidents and incidents you observe on the shift that they occur. • Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator. • Report any known or suspected unauthorized attempt to access facility's information system. • Perform only those care procedures that you have been trained to do. • Ensure that the resident's room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.). • Greet residents and escort them to their room. • Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate. • Make resident comfortable. • Inventory and mark the resident's personal possessions as instructed. • Store resident's clothing. • Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged. • Transport residents to new rooms or to the receiving area. • Assist with loading/unloading residents' to/from vehicles as necessary. • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. • Meet with your shift's nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services. • Report all complaints and grievances made by the resident. • Participate in and receive the nursing report as instructed. • Follow established policies concerning exposure to blood/body fluids. • Make beds as instructed. • Put extra covers on beds as requested. • Ensure that residents who are unable to call for help are checked frequently. • Answer resident calls promptly. • Check residents routinely to ensure that their personal care needs are being met. • Assist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.). • Keep residents' water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident. • Perform after meal care (i.e., remove trays, clean resident's hands, face, clothing, etc.). • Receive the nursing report upon reporting for duty. • Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. • Follow work assignments, and/or work schedules in completing and performing your assigned tasks. • Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that services can be adequately maintained to meet the needs of the residents. • Notify the facility when you will be late or absent from work. • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents. • Assist residents with daily functions (dental and mouth care, bath functions, combing of hair, dressing and undressing as necessary). • Keep residents dry (change gown, clothing and linens, when it becomes wet or soiled). • Assist residents in preparing for medical tests (i.e., lab work, x-ray, therapy, dental, etc.). • Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. • Perform restorative and rehabilitative procedures as instructed. • Assist in preparing the resident for a physical examination. • Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes. • Observe and report the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores). • Report injuries of an unknown source, including skin tears. • Observe disoriented and comatose residents. Provides personal care and household tasks for patient comfort and/or rehabilitation to patients in the home, adult foster home, assisted living, residential care, and intermediate care facility settings under a plan of care directed by the patient's physician(s) to provide quality health care outcomes and facilitate the achievement of maximum functional independence of the patient utilizing health care resources within Kaiser Permanente and the community. Essential Responsibilities: Receives, understands, and follows care plan and assignment from public health nurse, physical therapist, or occupational therapist. After instruction by RN for individual patient care procedures, perform the following: Routine bowel care, colostomy, stoma, skin cleansing and bag application; dry (non-sterile) dressing reapplication following removal of dressings for the purpose of bathing, and catheter care. After instruction by the physical therapist follow Rehab Care Plan, promoting activities of daily living independence, increase strength, and range of motion. Assists/provides patient with: Bathing, oral care, and skin care including bed bath, shower, tub, shampoo, hair care, shaving, perineal care, cleaning of teeth/dentures. Elimination activities including toilet, bedpan, urinal and/or commode. Transfer in and out of bed, and when directed, provides ambulation and assistance with activities and performing exercises. Assists with medication ordinarily self-administered by the patient instructed by the PHN with assistance limited to: Reminding patient of when it is time to take prescribed medication. Application or instillation of skin, nose, eye and ear preparations only when directed to do so for a specific patient by PHN who instructs the home health aide in the care requirements of the individual patient. Reports and documents care provided: Notifies and documents changes in the patient's condition or family situation in a timely manner to the public health nurse, licensed physical therapist, or occupational therapist. Records intake, output, weight, temperature, blood pressure, pulse, respiration after demonstrated competency and as instructed by RN or PT. Agency required documentation completed and submitted in appropriate format within agency documentation guidelines. Assists with household duties essential to the patients' health care at home, such as changing linen, personal laundry, dusting, vacuuming, washing dishes, shopping and meal preparation. On an infrequent basis, performs above household duties as assigned, as determined by specific care programs reflecting benefits for the patient's family members during respite care. Attends Conferences, meetings, and in-services: Patient Care Conferences, as appropriate and as required.
