The RN is fully responsible for the client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes, is designated as the case manager for hospice patients. The Hospice RN in collaboration with the physician and other Hospice team members anticipates and manages patient's symptoms, assists in identifying pt/family/caregiver grief/loss issues and assists in implementing the Hospice plan of care. Provides functional support to Hospice Aides and LPN/LVNs as needed.
*What You Will Do:*
* Informs the client/caregiver about the agency, care/services available, client obligation for payment, and other responsibilities and rights, before care is initiated. Accurately explains/understands the Hospice Benefit.
* Develops and implements an individualized Plan of Care that incorporates realistic, and attainable goals that is also outcome-focused so that the services provided will improve the patient's quality of life and promote dignity.
* Makes appropriate referrals for evaluation/care to other disciplines and services, and coordinates care with others to ensure effective and efficient care is provided.
* Utilizes interview, observation, and evaluation in assessing clients and applies nursing judgment, practice standards, in formulating nursing interventions and making recommendations to the physician, client/family and IDT/IDG.
* Reports changes in client condition as appropriate and in a timely manner, to the client's physician and/or Case Manager/designee and obtains orders for changes in the plan of treatment to respond to the client's condition.
* Re-evaluates and updates patient's plan of care based on patient goals and progress towards outcomes.
* Assess patient and family learning styles and needs for teaching regarding disease process, self-care, end of life care, and dealing with ethical concerns as well as patient goals as part of plan of care.
* Responds appropriately to changes in patient's physical, psychological, or spiritual conditions.
* Models' teamwork to accomplish to accomplish organizational goals and provide quality patient care.
* Demonstrates respect for cultural diversity in all care delivery and communication with co-workers, patients, and families.
* Maintains and enhances knowledge of pain and symptom management in hospice and palliative care patients, utilizing evidence-based protocols and standing guidelines per policy.
* Adequately prepares patients and families regarding the expected disease process.
* Provides anticipatory education and guidance to patients and families regarding the death and dying process.
* Accurately documents observations, interventions and evaluations pertaining to client care management and services in accordance with agency policy and procedure utilizing the point of care information system. This will include assessments, visit reports, medication information, treatments, coordination of services, client progress or lack of progress toward goals, and contacts with physicians and agency and community providers.
* Takes appropriate steps to assure timely submission of client data to the office for inclusion in the client record (by timely electronic transmission of data, or when appropriate by fax or mail).
* Documentation reflects progress towards patient/family goals.
* Documents all collaborative contact with physicians and other members of IDT/IDG team.
* Assures that documentation and orders are up to date and completed in a timely manner.
*Minimum Qualifications:*
* Graduate of an approved Nursing education program
* Current Registered Nurse licensure in the state of practice.
* Must have a minimum of one (1) year experience as a professional acute care nurse.
* Home healthcare/hospice experience preferred.
* Exhibits strong communication and interpersonal skills, with a special emphasis on grief/loss management.
* Must have current Driver's license and reliable transportation to and from work site.
* Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program.

Job Summary
Under the supervision of the Director of Foster Family Adoption (FFA) and Intensive Services Foster Care (ISFC), the FFA/ISFC Social Worker is responsible for a caseload of children and their families. Primary duties include conducting initial assessments and placements for children. The FFA/ISFC Social Worker also provides ongoing training, evaluation, and monitoring of foster and adoptive parents. They are responsible for completing all paperwork in accordance with County and agency regulations.
Essential Functions
Provide case management and coordination of services as required by contract and Title XXII regulations for safety and health.
Participate in recruitment of foster parents and provide orientation and training to applicants.
Complete and regularly update case related documentation, quarterly treatment plans, discharge summaries, initial quarterly and termination reports and files of foster parents and children in specified time frames as required.
Provide support to foster parents as well as enforcement of safety and health per Title XXII regulations.
Coordinate office duty and after hours duty coverage for emergencies as scheduled.
Assess referrals for appropriateness of placement
Provide initial and on-going training, evaluation and monitoring of foster and adoptive families
Responsible for intake assessments and placements of children
Interface with agency staff to complement the client's therapeutic milieu experience
Provide appropriate discharge and after care planning
Maintain contact with outside agencies and their representatives
Attend staff meetings, trainings, conferences and relevant informational meetings
Provide resources for services of placed children with Aviva
Conduct visitation with the placed child per contractual agreements with DCFS and Title XXII for both FFA and Adoptions
Complete Suspected Child Abuse Reports (SCARs)
Available after hours depending on visitation requirements and/or emotional and behavioral needs of the foster child

The role

Communicates effectively and openly with other Interdisciplinary Team members on participant status and needs.
Maintains current and thorough written case management records, including periodic reassessments of program Participants.

May develop and lead group counseling, support groups, and/or other relevant activities.
Refers Participants and families to appropriate community agencies or facilities, acts as a liaison with such organizations, and as an advocate for participants.

Requirements:
Your Experience and Education:
Masters of Social Work Degree from a graduate school of social work accredited by the Council on Social Work Education (Licensed Clinical Social Worker preferred)
A demonstrated ability in casework and counseling, including one (1) year of clinical geriatric experience required; hospice experience preferred.
Excellent communication and counseling skills
Demonstrated skills in assisting individuals and families in problem solving and utilizing community resources
Demonstrated ability to efficiently operate a computer, prior Electronic Health Records (EHR) preferred
Valid CA drivers license, with a reliable vehicle that is insured in accordance with state requirements
Is open to an occasional flexible schedule to work evenings or weekends
Is comfortable being mobile and can travel throughout service area

Overview:
Supports resource parents in meeting the needs of youth placed in their care by providing guidance, assistance, and ongoing case support. Delivers permanency-focused services to both families and youth, helping ensure stability, well-being, and successful long-term outcomes.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
Support and implement therapeutic interventions and structured activities for youth and families.
Conduct SAFE home studies and complete required monthly in-home visits, as well as monthly out-of-home visits for youth ages six and older. Maintain accurate youth and resource parent records within the electronic database.
Connect youth and resource parents with appropriate community resources based on identified needs.
Assist in evaluating and developing resource family homes, including assessing environmental factors and personal characteristics to determine suitability.
Promote and help maintain each youths stability within the resource family home, ensuring safety and overall well-being.
Collaborate with youth and resource parents to address concerns related to adjustment, care planning, social functioning, behavioral modification, and rehabilitation services when applicable.
Assist in coordinating respite care services, including scheduling respite days and nights for youth and resource parents.
Maintain detailed, dated, and signed case records and reports in accordance with agency policies and procedures.
Education and/or Experience
Masters Degree in Social Work is required.
Conditions of Employment:
Must possess a valid CA Drivers License, current auto insurance, and a clean DMV printout (no more than 3 points).
Must pass a clear background check with CACI, DOJ & FBI.
CPR and First Aid Certified.
TB Exam and Physical at the time of employment.
Driving Requirement:
This position requires the use of a personal vehicle for business purposes and on-call rotation.
Driving is a required function of this position. Employees must use a personal vehicle that meet all agency requirements, including maintaining liability insurance that meets or exceeds state requirements.
Employees are responsible for ensuring the timely renewal of their vehicle insurance policies and providing updated documentation to the agency to remain in good standing and compliant with position requirements.
