Responsibilities include (but are not limited to):
Client Management: Responsible for receiving and processing incoming service inquiry calls, converting calls into in-home assessment visits, and performing client assessments. Develop individualized client care plans, ensure client satisfaction, and implement care plans. Establish initial client records and complete all client onboarding requirements.
Client Interaction: Introduce caregivers to clients, provide support, feedback, and training to caregivers. Conduct reassessment of clients and quality assurance visits.
Scheduling & Staffing: Schedule shifts and manage shift changes based on company standards and benchmarks. Verify hours worked, clock-ins, and clock-outs through our advanced telephony scheduling system. Assist with recruiting, screening, and staff hiring.
Office Duties: Cross-train with office duties including scheduling and basic office duties. Data entry and file system maintenance.
Customer Service: Answer telephone calls, problem-solve, and provide information to caregivers, clients, and industry professionals. Ensure internal and external customer service and client satisfaction.
On-Call Duty: Accept limited on-call duty a minimum of Two (2) weekends per month and 1-2 evenings per week.
Client Care: Willing to train caregiver staff and go to the field to provide Client Care as needed.
The ideal candidate will possess the following:
One year of related home care and/or home health, office administration, or specific scheduling background
One year of scheduling experience required
CNA certified with 2 years minimum experience preferred.
High School Diploma required; Associate's degree or higher preferred
Outgoing, compassionate, and positive personality
Ability to multitask, stay calm under pressure, work quickly and efficiently
Exceptional attention to detail, organization, and ability to prioritize
Strong time management, written, and verbal skills
Problem-solving, problem prevention, and issue resolution skills
Leadership and sound decision-making abilities

POSITION OVERVIEW

POSITION OVERVIEW
The MDS Coordinator (LPN, RN) is responsible for overseeing the resident assessment and care planning process and ensuring compliance with federal and state regulations related to resident assessments, quality of care and Medicare/Medicaid reimbursement.
Key Responsibilities
* Conduct and complete the Minimum Data Set (MDS) assessment to evaluate residents' physical, psychological and functional status, including the implementation of Care Area Assessments (CAA)s and triggers.
* Evaluate each resident's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
* Prepare and electronically transmit timely reports to the national Medicare and Medicaid databases.
* Develop a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
* Provide information to residents/families on Medicare/Medicaid and other financial assistance programs available to the residents.
* Ensure that MDS notes are informative and descriptive of the services provided and of the residents' response to the service.
* Assist with completing the care plan portion of the residents' discharge plan. Evaluate and implement recommendations from established committees as they pertain to the assessment and/or care plan functions of the health campus.
Qualifications
* Must have and maintain a current, valid state LPN or RN license
* Three (3) to five (5) years' experience working in the MDS or assessment role in a senior residential care, healthcare, senior living industry or long-term care environment, preferred

The MDS Coordinator (LPN, RN) is responsible for overseeing the resident assessment and care planning process and ensuring compliance with federal and state regulations related to resident assessments, quality of care and Medicare/Medicaid reimbursement.
Key Responsibilities
* Conduct and complete the Minimum Data Set (MDS) assessment to evaluate residents' physical, psychological and functional status, including the implementation of Care Area Assessments (CAA)s and triggers.
* Evaluate each resident's condition and pertinent medical data to determine any need for special assessment activities or a need to amend the admission assessment.
* Prepare and electronically transmit timely reports to the national Medicare and Medicaid databases.

* Provide information to residents/families on Medicare/Medicaid and other financial assistance programs available to the residents.

Qualifications
* Must have and maintain a current, valid state LPN or RN license

POSITION OVERVIEW

POSITION OVERVIEW
The Registered Nurse (RN) is primarily responsible for providing direct, quality clinical care and serves as a Team Leader to patient care staff.
Key Responsibilities
* Lead a team of direct care providers to ensure appropriate execution of medications and treatments, documentation, family teaching, care planning and patient care conferencing in compliance with the Health Campus Policies and Procedures.
* Utilize the nursing process in delivering patient care and ensure continuity of care from admission through discharge and return to family and community.
* Visit your residents on assigned unit daily to observe and evaluate each resident's physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing (DON) when there is a change in a resident's condition.
* Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
* Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary.
The Weekend Package Program details:
* Works a minimum of 2*(8hr) or (12hr) shifts every weekend
* The Weekend Package incentive is an additional $7.00/hour working between the hours of 6pm on Friday to 7am on Monday.
* Additional shift differentials apply when working evening or night shifts on the weekends
Qualifications
* Must have and maintain a current, valid state RN license
* Current, valid CPR certification required
