Organize “Into Lunches” and Outings. Schedule entertainment, special programs and performing groups. Coordinate food related activities via dietary order. Plan and schedule activities for monthly calendar: group, 1:1, theme weeks, etc. Assure therapeutic group activities occur per schedule. Manage and keep a running balance in the activity checking account, and prepare all forms for the monthly budget. Sends special invitation / announcements to residents' families for special events and parties. Coordinates voting for residents. Drives bus for resident activities Assists with the coordination of staff retention program All other duties specified by Administrator. QUALIFICATIONS Meets one of the following criteria: Has two years of experience in a social or recreational program within the past five years, one of which was full-time in a patient activities program in a health care setting; or Eligible for certification as a therapeutic recreation specialist by a recognized accrediting body; or Qualifies occupational therapist or occupational therapy assistant; or Has completed activities state approved certification course. Follow through with individual and/or group maintenance programs initiated by specialized therapies ordered by a physician. Maintain individual records of maintenance activities for each resident as required. Record resident daily activities and file as required. Act as a resource for CNAs in maintenance program techniques. Assists and feeds residents in dining room with meals as appropriate. QUALIFICATIONS Certified Nursing Assistant or graduate of a PT, OT, COTA or PTA accredited program who has yet to receive state licensure. Ability to function in a team environment. JOB TITLE: Speech-Language Pathologist/Therapist REPORTS TO: Clinical Manager * BASIC PURPOSE: A clinically competent, licensed speech-language pathologist who delivers care to patients in their place of residence in accordance with the patients individualized Plan of Care through the process of data collection, planning, implementation and evaluation. The Speech-Language Pathologist/Therapist works effectively and interacts with the patients, caregivers, and other members of the health care team while maintaining quality care in accordance with accepted standards of practice. * PRIMARY RESPONSIBILITIES: * Conducts appropriate evaluative procedures, assesses patients home environment as appropriate, and identifies patients equipment needs relative to speech/language function. * Identifies functional speech defects, establishes goals to improve patients speech function, and participates in the development of the team care plan. * Administers speech therapy program utilizing specialized therapeutic techniques and and/or equipment in accordance with the Plan of Care. * Recommends communication devices/aids as indicated. * Confers with the attending physician, home health team members and patient and/or family/caregivers to promote coordinated and comprehensive care. * Participates in meetings, inservices and other related activities as needed. * Identifies patient and family needs for other home health services and refers them to those services. * Supervises Homemakers/Home Health Aides when they are participating in the patients therapy regimen. * Prepares and submits clinical and progress notes in a timely manner and in accordance with Altenheim Home Health Care, state and federal guidelines and regulations. * Maintains clinical competency in speech language pathology practice and theory. * Instructs patient and/or family/caregiver and other team members in the patients treatment regimen as indicated. * Performs other duties as required to facilitate the delivery of speech language pathology services and swallowing assistive services. * Completes a minimum of twelve (12) hours of inservices per year. * Meets or exceeds delivery of Altenheim Home Health Care service standards in a consistent fashion. * Interacts with all staff in a positive manner, supporting the Altenheim Home Health Care Mission. * Complies with local, state, and federal laws, Medicare regulations and established Altenheim Home Health Care policies and procedures (which includes confidentiality and HIPAA). * Conducts all business activities in a professional and ethical manner and adheres to Altenheim Home Health Cares Code of Conduct.
