Facilitates school climate activity. Completes a need assessment as required by grant. Provides family and youth engagement activities. Reinforces other universal supports and practices. Attends school functions at assigned school that teachers, guidance counselors, and/or administration would normally attend that would enhance the relationship and rapport between the liaison and school staff/faculty/administration and with students and parents, regardless of whether it is required for teachers or not. Meet Tier 2 services at assigned school as specified under the grant including: Provides psycho-educational groups for students according to the state grant and requests from teachers and administration at assigned school. Provides supportive brief individual consultation or skills building for students. Offers individualized classroom support. Provides mental health/behavioral health screenings. Participates in school meetings (e.g. IEP meetings, disciplinary meetings). Provides other services as requested by school or school district with approval of supervisor (e.g. covering a class to give a teacher a break, proctoring tests, etc.). Meet Tier 3 services at assigned school as specified under the grant including: Provides mental health/behavioral health assessments (including intakes). Provides clinical services (e.g. individual, group, and/or family therapy). Makes referrals for community-based services and supports (e.g. medical appointments, case management, etc.). Completes the requirements of the grant regarding number of teachers/staff and students serviced and outcome effectiveness. Maintains all necessary documentation to meet grant and to complete monthly and annual reports in REDCap. Completes pre/posttests for required trainings and inputs data into REDCap. Prepares and maintains all clinical records in accordance with Center quality assurance policies and procedures, including developing quality treatment plans and comprehensive safety/crisis plans. Participates, at minimum, in one hour of clinical supervision on a weekly basis, or as otherwise directed by the Center. Participates in required training to maintain credentialing status. Follows all guidelines outlined in the CAREY Policies and Procedures Manual including Corporate Compliance Policies. Consistently shows up for work on time according to the assigned school schedule during the school year, keeping supervisor informed of schedule, and follows Carey policy for SBBHL during summers and for leave requests. Other assigned duties by supervisor or management to meet the needs of the organization. Minimum Qualifications: Graduation from an accredited college or university with a master's degree in psychology, counseling, social work, or other master's degree eligible for licensure for clinical social worker or professional counselor. At least two (2) years of experience working with children with SED, and proficiency in or ability and willingness to become trained in the MTSS framework, a trauma-informed approach and relevant trauma-informed practices (e.g. trauma-specific behavior interventions and trauma- responsive schools). Examples of Duties: Provides case management services and completes related documentation in compliance with established criteria and time frames for such services. Provides home visits to clients, as needed, in order to link, monitor, and assess their overall functioning in their natural environment but in compliance, at a minimum, with the contact requirements of the level of the client's case management. Is responsible for maintaining a designated amount of direct service time with clients and a designated amount of contacts for the assigned caseload. Participates in the initial individual program planning process with all service providers (i.e., therapist, medical staff) and provides ongoing information regarding client progress to the service providers. The PATH Case Manager is to provide effective outreach and referral services to eligible adults: who are homeless or at risk of homelessness and dealing with a suspected or diagnosed serious mental illness or co-occurring disorder. PATH Case Managers follow The Department of Housing and Urban Development's (HUD) definition of homelessness and use screening tools to asses suspected mental health issues. Examples of Duties: Provides identification/assessment of individuals who are homeless or at risk of homelessness and have or are suspected of having a serious mental illness or cooccurring disorder. Makes presentations internally to educate staff on the PATH program. Provide street outreach and community outreach to seek eligible adults to enroll. Maintain open communication and regular contact with local shelters, churches, and other agencies that may be of assistance to the homeless. Attend weekly case management supervision meetings to provide clients with effective transition from PATH to community mental health services. Provides a monthly contact report and a quarterly service summary to the program director for state reporting. Attend local interagency, state and regional meetings including the annual PATH conference and monthly Continuum of Care meetings. Keeps accurate PATH client files and actively inputs PATH client information into Homeless Management Information System (HMIS) as well as attends regular HMIS trainings through COC. Participate in the annual Point in Time (P.I.T.) Count, organized by the Continuum of Care. Link PATH clients to community mental health services and other necessary community resources. The prescriber will order all required and necessary labs and EKGs as outlined by the industry standards of care and scope of practice and will sign off on those labs within 24 business hours. (examples include but may not be limited to: typical and atypical antipsychotics, Lithium, Depakote, Tegretol, Clozaril, and psychostimulant medication). The prescriber will complete an AIMs assessment of patients who are prescribed typical and atypical antipsychotic medications at a minimum of once every 6 months in compliance with industry standards of care and scope of practice. The prescriber will complete all consent form documentation for initiated and discontinued medications in compliance with industry standards of care.
