Complies with all federal, state, local, Carilion Clinic, and departmental policies and standards as applicable. Demonstrates knowledge of growth and development to appropriately care for patients served. Provides nursing care for patients of all ages. Completes patient assessment to include height, weight, vital signs, chief complaint, current medications, etc. Prepares patient for provider evaluation, including obtaining specimens, positioning for exams, listing medications, assisting during exams and diagnostic and therapeutic procedures. Coordinates tele-retinal and tele-dermatology clinic set up. Administers medications and injections as directed by the provider. Maintains work area, exam, treatment and x-ray rooms and lab in a clean, safe manner; restock with supplies and instruments; sterilize reusable equipment as needed. Documents and maintains patient information in the electronic medical record in a timely manner to include update treatment procedures, medication refills, test results, patient concerns, etc. Contributes to Carilion Clinic's performance improvement activities. Provides patient teaching as indicated and directed by the provider, including lab results, diet instructions, medication instructions, appropriate follow-up, and reinforcing provider instructions as needed. Assists with referral authorizations for specialists and pre-authorizations for testing and ancillary services. Assists with lab procedures, including collecting urine specimens, collecting and preparing blood samples for reference lab, labeling all specimens with accurate patient ID. Observes all safety precautions and standards; assures proper disposal of specimens and contaminated waste. LAB (if applicable) Performs lab procedures as directed by the provider including phlebotomy, peripheral sticks, collecting and preparing urine specimens, throat smears; conduct lab testing onsite after appropriate training, including waived and moderate complexity studies. Prepares equipment to begin testing lab specimens. Runs daily quality control tests as directed by lead clinical/lab staff. Adheres to quality control measures with all lab procedures, performing proficiency testing and conducting control testing and calibration on all equipment. Maintains Q/A Plan. Prepares equipment to begin testing lab specimens. Runs daily quality control test as directed by the clinical/lab staff. RADIOLOGY (if applicable) Performs x-ray procedures as directed by the provider. Takes measures to prevent excessive exposure to patient and self. Completes all documentation and labeling of x-rays. Performs any other tasks relating to the operation of the practice that may be assigned or requested for which training has been provided. What We Require: Education: Graduate of accredited school of practical nursing required, or new graduate of approved program eligible to sit for the Virginia licensure exam. Experience: One (1) year experience in a physician office preferred. Licensure/Certification: Current licensure as a Practical Nurse in the State of Virginia (LPN) required. Licensure as a Radiologic Technician Limited if performing radiology. Current AHA BLS-HCP certification required. Other: Customer driven. Demonstrates a positive demeanor, good verbal and written communication skills, and a professional appearance and approach. Must have the ability to problem-solve and communicate with a wide variety of people in an effective manner. Must exhibit skills to utilize automated computer systems, including electronic medical records. Keyboarding skills required for automated computer systems, including electronic medical records. The LPN Inpatient utilizes the nursing process to provide direct patient care to patients whose care requirements are routine and standardized with predictable outcomes. Practices under the direction and supervision of the Registered Nurse while assisting the healthcare team to achieve desired patient outcomes. Job Responsibilities Provides direct patient care utilizing the nursing process under the direction and supervision of the Registered Nurse. Reviews diagnostic test results and reports abnormal results and changes in patient condition to the Registered Nurse, physician and appropriate healthcare team members in a timely manner. Collaborates with the Registered Nurse on the execution of patient plans. Assists with patient education and discharge planning under the direction of the Registered Nurse. Evaluates and maintains the patient care environment to promote safety. Collaborates with the Registered Nurse and healthcare team to advocate for the clinical, emotional, ethical, spiritual and cultural needs of patients. Participates as appropriate in unit based quality improvement initiatives. Maintains patient privacy and confidentiality. May perform additional duties as assigned. Requirements Certification - Basic Life Support upon start Licensure - Licensed in the Commonwealth of Virginia as a Licensed Practical Nurse upon start Experience - None Required Education - Vocational / Technical Nursing Graduate of an accredited Licensed Practical Nurse school. Preferred Qualifications: At least 1 year of wound care experience, home care, skilled nursing facility experience.
