The Transfer Center Nurse Specialist provides leadership and assumes accountability for appropriate patient placement and resource utilization through collaboration with Carilion Management, Clinical Staff, Unit Staff, and Transfer Center Staff. Facilitates patient flow into and within the Carilion Clinic. Connects the medical staff of Carilion Clinic with referring physicians. Facilitates effective two-way communication and resolution of operational issues in support of Carilion Clinic goals and objectives. Provides support for all staff members while delivering quality service through a team oriented, collaborative approach.
The Transfer Center Nurse
Facilitates patient and physician access to Carilion Clinic's comprehensive services.
Facilitates effective patient throughput by ensuring proper patient placement through collaboration with Carilion's management and patient care teams.Identifies and provides solutions for throughput issues on an ongoing basis.
Ensures and facilitates communication to Carilion Clinic's internal stakeholders (HeartNet, Emergency Department, PACU, etc.).
Facilitates physician communication. Serves as a liaison between Carilion Clinic physicians and external facility physicians. Serves as the primary contact for physicians or their representatives regarding notifications of patient transfers and admissions.
Connects customers with patient transportation needs with the Communications Center of Carilion Clinic Patient Transportation (CCPT).
Establishes, maintains, and promotes positive and professional communication with all those served - physicians, administration, staff members, patients, families, visitors as well as members of the community.
Receives and screens incoming calls via multi-line phone system. Routes calls to appropriate individuals and takes messages accurately.
Operates specialized software programs and computer systems.
Assists other team members with solving problems with the various computer systems and software programs utilized in the facilitation of patient flow within Carilion Clinic.
Appropriately and effectively place patients.
Updates patient information regarding admission status and level of care. Makes changes and adjustments in “patient type” to ensure proper coding for billing purposes.
Utilizes triage techniques, prioritization and delegation optimizing Carilion Clinic resources for the communities in which it serves.
Maintains awareness of facility census levels and related patient flow issues.
Understands medical treatment of advanced patient types.
Conducts intakes, assessments, arrangements, and placements of all levels of patient types.
Assists with staff training, functions as a patient advocate, and assists other team members with policy interpretation.
Prioritizes and shifts from one function to another with short notice.
Maintains an overall awareness of bed availability, total discharges, total admissions and patient flow in order to appropriately and effectively assign patients.
Evaluates and prioritizes daily surgical and procedure schedules to coordinate patient placement needs based on surgical and acuity needs.
Facilitates interdepartmental communication, customer service, negotiation and decision-making regarding issues with patient transfers and bed placement.
Mentors and professional role models in facilitating problem resolution for clinical and non-clinical issues.
Organizes, prioritizes, directs and provides feedback regarding nursing care delivered by various levels of staff.
Communicates and documents pertinent information (verbal and/or electronic) in a timely manner to hospital administration and department leadership.
Documents pertinent information, actions and decisions and communicates to appropriate levels of leadership.
Utilizes excellence customer service and problem solving skills for both internal and external customers.
Contributes to Carilion Clinic's performance improvement activities.
Obtains adequate facts and evaluates data to identify and intervene in actual and/or potential safety and risk management issues.
What We Require:
Education: Graduate of accredited school of nursing. Bachelor's Degree preferred.
Experience: Minimum of 3-5 years required. Minimum of 2 years adult patient care required. Critical Care / ED experience preferred.
Licensure/Certifications/Registration: Current Virginia RN license required.
Life Support: Must achieve Advanced Cardiac Life Support (ACLS) certification, Pediatric Advanced Life Support within 6 months of accepting position.
Other Minimum Qualifications: Demonstrates leadership qualities with integrity at current level of practice and experience. Must demonstrate effective interpersonal, written and oral communication skills in a positive, courteous manner. Performs as a team player who is focused on the customer and has demonstrated problem solving skills. Must have the ability to resolve complaints, problems and issues with tact and focus of service excellence. Embodies a philosophy of service and patient centered care. Situational Awareness is essential. Demonstrates the ability to work within a technologically advanced environment. Minimum Qualifications:
Education: BSN or AD. If RN has an AD, within six months from date of hire, they must meet with their nurse leader and conduct the following: 1.) Identify which accredited school they plan to attend 2.) Provide a written plan with anticipated BSN completion date 3.) Submit a review of transcripts from the school indicating the required pre-requisites and timeline for taking the courses 4.) Complete BSN within 24 months of start date.
Experience: 1 year of research training or protocol implementation
Certification: Basic Life Support from the American Heart Association Upon Start; RegisteredNurse License Upon Start; Must be eligible to practice in VA
Preferred Qualifications:
Experience: L&D, C-section, and/or similar; research is a plus. Proficient in Spanish.

Serves as liaison between research sponsors, site management organizations, local Institutional Review Board (IRB), physicians/staff, and R&D.
Recruit, screen, and enroll eligible study participants.
Obtain informed consent from study participants.
Manages completion and submission of study related documentation.
Communicates with research sponsor or CRO, local or central IRB, Research and Development, and other ancillary departments to ensure smooth implementation of research protocol and ongoing maintenance.
Assist in monitoring enrollment goals.

What We Require:
Education: Bachelor degree in biology or medical/clinical sciences (i.e. physiology, nursing, etc.), or a related field required. Graduation from an accredited nursing program required. FDA knowledge required. Masters Degree preferred.
Experience: Minimum of fifteen (15) years of Clinical Research (or equivalent research experience) and a Bachelor Degree in Science required.
Licensure/Certifications/Registration: License or certification specific to the discipline according to Virginia requirements. Current license to practice as a RegisteredNurse in the Commonwealth of Virginia required. Nationally recognized clinical research certification from Society of Clinical Research Associates (SoCRA) or Association of Clinical Research Professionals (ACRP) required upon hire. I.A.T.A. (International Air Transport Association) certification is required for projects that will be transporting specimens prior to completion of IRB application.
Other Minimum Qualifications: Thorough knowledge of medical terminology and human anatomy and physiology. Working knowledge of the disease/condition under study, the clinical trial process and good clinical practices. Proficient in the application of federal regulations and the Good Clinical Practices governing clinical research. Must be competent in word processing and spreadsheets. Proficient with data base management systems and statistical software, research methodology, and statistical analysis. Must have effective interpersonal and problem-solving skills. Must be self-directed and excel in attention to detail. Must use good judgment with regard to priorities and deadlines. Must have demonstrated leadership among a research team.
