Essential Functions

Performs examination, selecting reliable and valid examination methods relevant to the chief complaint, results of screening, and history.
Produces documentation in accordance with the Clinical Process Policy (CTSCP67).
Screens patients using procedures to determine the effectiveness of and need for therapy services, if appropriate.
Uses support personnel according to legal standards and ethical guidelines. Monitors the care delivered by support personnel and provides regular feedback.
If applicable, completes annual education as assigned to comply with DNV Hip and Knee Replacement Program requirements.

Job Summary
Home Base, a Red Sox Foundation and Massachusetts General Hospital program, is dedicated to healing the invisible wounds - including post-traumatic stress, traumatic brain injury, anxiety, depression, co-occurring substance use disorder, family relationship challenges and other issues associated with Military service - for Veterans of all eras, Service Members, Military Families and Families of the Fallen through world-class clinical care, wellness, education, and research.
The Massachusetts General Hospital seeks a dynamic Licensed ClinicalSocialWorker (LCSW)/Licensed Independent ClinicalSocialWorker (LICSW) to serve on the Special Operations Team. In this role, the LCSW/LICSW will provide and oversee the provision of psychiatric, psychosocial, and overall mental health services and referrals for military Special Operators into the Home Base program. They will also be dedicated to supporting the Special Operations Forces (SOF) program and will provide coverage to other clinical operations as needed. The LCSW/LICSW may help cover approximately 2-3 weekend shifts per year.
Summary
This position will collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community.

Essential Functions
* Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues. Formulates biopsychosocial assessment, disposition, and treatment plans.
* Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
* Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
* Coordinate care for Special Operations Forces (SOF) veterans and military-connected family members seeking care at Home Base with the appropriate internal or external clinicians in a multi-disciplinary team, coordinated care model.
* Serve as a contact point for accepting and triaging SOF referrals from internal clinicians, outside clinicians and self-referred patients.
* Serve as primary case manager for patients in SOF admissions process in collaboration with Admissions Coordinator, including, but not limited to, the following responsibilities: conduct pre-screening to assess treatment and evaluation needs for SOF patients referred; coordinate signing of Release of Information forms, acquire relevant medical records, and coordinate with SOCOM where applicable; serve as point of contact for ongoing questions from the patient as they arise.
* Monitor patient's status in admissions process using appropriate documentation.
* May be asked to collaborate with SOF/ComBHaT team, patients, clinical team, and Resource Specialist to identify clinical and psychosocial needs in patients' home communities and identify appropriate resources.
* May provide direct clinical services to Veterans and/or family members, which may include group or individual therapy.
* May provide psychoeducation regarding PTSD, TBI, and related conditions to Veteran patients and family members as clinically appropriate within the context of the Intake and triage duties.
* Collaborate with an inter-disciplinary team of clinical staff (socialworkers, psychologists, nurses, psychiatrists, physiatrists, physical therapists, etc.) to coordinate patient care and plan for discharge/aftercare when needed.
* Consult with team to facilitate mandated assessments when abuse is suspected (child, disabled adult, elder), and safety assessment when violence is reported. Collaborates with MGH resources (HAVEN and Child Protection Team).
* May assist with crisis intervention and management.
* Documents timely and relevant information in patient electronic medical record and Home Base database.
* Provide coverage for social work responsibilities in Outpatient Clinic as needed.
* For LICSWs: provide clinical supervision to LC-level and/or MSW students as needed.

We are growing! Seeking a licensed therapist with training and interest in general outpatient psychotherapy with children & adolescents. The ideal therapist for this position will be comfortable with individual, group, and family therapy modalities, utilizing an evidence-based approach to treating a wide range of presenting problems in children and adolescents. Referral sources are physician colleagues from across the hospital system, as well as integrated care. Many opportunities for involvement in collaboration groups and ongoing training are available.
Gundersen Health System, based in La Crosse, Wisconsin, is seeking a full-time Clinical Therapist (LPC, LCSW, LMFT) to join the Behavioral Health Department. The applicant must have completed an accredited program and be eligible to obtain a license to practice in our multi-state region with no restrictions or limitations. Advanced practice clinicians work under the direct supervision of a physician. Clinician staff may need to provide outreach service as part of their role in the system. Teaching students, residents, fellows or clinical staff is cornerstone to our Medical Education philosophy and an expectation of all clinicians.

Attend meetings with leadership regularly with community resources to build a bridge for communication between our department and the community.
Evaluates patients for abuse/neglect and makes appropriate referrals, & assist ED staff with completing mandated reports of suspected abuse and/or neglect of minors, disabled adults, and seniors. Cooperate and collaborate with the investigating bodies for safe plans.
Involves patient and family in case planning decisions.

Formulates, develops, and implements a plan utilizing appropriate social work interventions. Provides education and counseling to clients and families around issues related to adaptation to the client's illness and/or life situation. May utilize crisis intervention, brief and long-term individual, group, and family therapies.
Facilitates education/training modules to assist RN staff managing basic social work needs.
As requested/required, participates in community coalition in support of NFP as well as promotes public awareness of Healthy Beginnings Services and the Mobile Health Clinic.
Assesses physical, emotional, social, spiritual, and environmental needs of clients and their families as they relate to the NFP domains.
Assists clients and their families in establishing goals and outcomes through therapeutic relationships by providing education, support, and referral resources.

Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.
Develops a bereavement plan of care to address family member/care giver needs.

This position will be a part of the City of Peoria Social Services Unit (SSU). SSU personnel are tasked with assisting those who come into contact with, or are referred to, the SSU. One of the goals is to promote safer encounters between citizens in need of crisis services and police department personnel while maintaining a high level of professional service. The other main goal is to refer to, and encourage participation in, appropriate mental health, substance use, or other needs-based services that may fit the individual. These duties are critical to an overall healthier, safer community and are aimed at facilitating more stable community members. This position provides crisis intervention services/assessments/evaluation and stabilization/case management to people in a primarily community-based environment in collaboration with the Peoria Police Department (PPD). This role will include field response into the community in collaboration with the PPD, likely several times per day, to provide mobile mental and behavioral health services to citizens within the city of Peoria, Illinois. This role will consist of supervision of Licensed SocialWorkers and Licensed ClinicalSocialWorkers, clinical oversight for staff, continued follow up and case management to connect citizens to ongoing services and supports.
