Responsibilities
Plans appropriate treatment programs for infants, children, adolescents and adults of all ages (depending on area of assignment) based on physician referral, evaluation results, patient/family goals and interests, sound clinical judgment and other pertinent available information. Responsible for maintaining a productive schedule with an average of 6.0 out of 8.0 hours per day scheduled for direct clinical care. Informs manager of openings and/or cancellations in their daily schedule.Administers daily treatment ensuring that the treatment plan is age appropriate and adjusted as necessary in order to the changing needs of the patient.Successfully completes all competency assessments pertaining to provision of patient care.Provides special services such as video swallow studies.Communicates with physicians, team members, other professionals, family members and caregivers for the purpose of sharing information that will assist in achieving optimum care for the patient. Establishes effective rapport with patients of all ages depending on assignment.Selects appropriate assistive devices for patients and provides appropriate instruction in the maintenance and use of such equipment.Provides education to the patient and family or significant others based on patient needs. Identifies and addresses learning readiness, preference and learning barriers.Determines when the patient will be ready for discharge. Recognizes the need for termination of treatment and effectively plans discharge which may include establishing home treatment plans and/or arranging for continued services.Administers and interprets initial assessments which determine the patient's status in the following areas of function: verbal expression, non-verbal communication, auditory comprehension, written expression, reading comprehension, cognition, swallowing skills and the need for swallowing precautions, articulation, fluency, voice and the need for augmentative systems.Selects appropriate assessment procedures to assess the abilities and limitations of patients of all ages, depending on area of assignment.Completes all documentation according to departmental policy.Performas other duties such as charge entry for all services rendered on the day delivered, maintaining a clean work environment and serving on departmental committees and in program development efforts as appropriate.May supervise speech pathology students and interns at the request of manager after obtaining Certificate of Clinical Competence.

Responsibilities
**Essential Functions**
Selects, administers and interprets diagnostic assessments across all communication areas with consideration of child's overall developmental abilities and speech/ language levels.
Works with parents and team members to obtain necessary information to develop speech, language, and listening targets that optimize child's use of amplification devices, and supports families through the referral process for additional services or testing.
Analyzes expressive and receptive semantic and syntactic skills at a variety of levels through formal and informal measures.
Writes treatment plans, updates progress weekly , completes diagnostic reports and maintains appropriate chart note documentation in the electronic medical record.
Provides repeated successful practice and utilizes audition as the primary pathway to expanding language and communication skills during therapy sessions.
Has or develops familiarity with basic workings of amplification devices (e.g., hearing aids, cochlear implants, FM systems, etc.), seeking guidance from audiologist when in doubt or when more comprehensive troubleshooting is required.
Acts as second tester for audiograms, perception testing and cochlear implant programming for patients of various ages. Activities include: working to develop consistent visually reinforced or conditioned play responses, transcribing and interpreting verbal responses, and providing input about performance at school, under the guidance of an audiologist.
Cooperatively observes individual patients at school sites and counsels staff about auditory, speech and language recommendations specific to the child's school placement.
Assists with additional responsibilities in program development and implementation as deemed necessary by ECHO Manager, CAOS Supervisor and/or ECHO SLP Supervisor.

Ability to work as a member of an interdisciplinary team. Drives reliable transportation and maintains state required insurance and valid Driver's License.
Responsibilities
Essential Functions
Administers prescribed speech therapy treatment programs for patient s to restore function, relieve pain, prevent disability and injury.
Anticipates and orders an appropriate quantity of supplies with sufficient lead time that patient does not run out of supplies between visits.
Communicates with all appropriate caregivers including as family members, physicians, therapists, and nurses. As appropriate, adds and documents all communication in patient medical record.
Communicates the requirements for collaborative team and care givers, assignments with scheduler, prepare written instructions for the assistant and supervises the assistant in the patient home.
Completes audits and other assigned tasks within assigned time frame.
Demonstrates understanding of Medicare Home Care benefits including covered services, recertifications, transfers, and non-recertifications. Follows regulatory requirements regarding documentation standards.
Develops and implements a setting appropriate plan of care with measurable and achievable goals that meet patients need. Collaborates with patients and relevant others in the planning process.
Documents and follows established department process to ensure follow-up visits are scheduled.
Documents evaluations and plans of care according to department policy, including goals, modifications of treatment plans, and patient's response to treatments and discharge plans. Ensures documentation is completed per regulatory and departmental standards.
Documents through comprehensive detail about patient visit, information is sufficient that following staff can determine exactly what and how care was rendered with a clear plan for the next contact.
Instruct, motivate, and assist patient in activity regimens and use of assistive/supportive devices.
Monitors, supervises, and adjusts the plan of care in order to ensure effective and efficient care.
Demonstrates knowledge of regulations for therapy and home care, and all departmental policies and processes. Comprehends OASIS and case mix index.

Assess homebound status of patients.
Completes comprehensive speech therapy evaluation in patient's home and develop plan of care.
Communicate with patients to schedule appointment times and manage daily schedule.
Educate family/caregivers on recommended home treatment program.

Compensation and Benefits
The compensation range for this position is $30.72per hour - $52.84per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model. Essential Functions
Collects case history information and integrates information from patients, family, caregivers, teachers, relevant others, and other professionals. Selects and administers appropriate evaluation procedures to meet patient needs. Recognizes the needs, values, preferred mode of communication, and cultural/linguistic background of the patient, family, caregivers and relevant others.
Conducts assessment procedures for speech and language pathology. Selects or develops and uses appropriate materials and instrumentation for prevention and intervention.
Develops and implements a setting appropriate plan of care with measurable and achievable goals that meet patients need. Collaborates with patients and relevant others in the planning process.
Interprets, integrates, and synthesizes all information to develop a treatment plan and make appropriate recommendations for intervention.

Administers standardized testing to access level of function compared to age related norms
Assess feeding and swallowing impairments
Educates patients and caregivers on home programs and activities
Coordinates with pediatric PT/OT on treatment interventions

Completes annual requirements as per therapy clinical ladder for senior status which includes supervision of students, completing an approved project, and one in-service training

A Speech-LanguagePathologist (SLP), also known as a Speech Therapist, is a licensed healthcare professional who evaluates, diagnoses, and treats individuals with speech, language, communication, and swallowing disorders. SLPs work with patients of all ages, from infants to the elderly, to help improve their ability to communicate effectively, swallow safely, and manage cognitive-communication disorders. They employ a variety of therapeutic techniques and evidence-based practices tailored to the individual needs of their patients.
Key Responsibilities:
Assessment and Diagnosis:
Conduct comprehensive evaluations to assess speech, language, cognitive-communication, and swallowing disorders.
Use standardized assessments, case history review, and observational techniques to identify speech and language deficits, as well as cognitive or swallowing difficulties.
Diagnose speech and language disorders, such as speech delays, stuttering, voice disorders, aphasia, dysphagia (swallowing disorders), and cognitive communication issues.
Developing Treatment Plans:
Develop individualized treatment plans based on patient assessment findings, goals, and specific needs.
Set short-term and long-term goals for each patient to improve speech, language, and swallowing abilities.
Modify treatment plans and interventions based on patient progress and changes in their condition.
Speech and Language Therapy:
Provide therapy to address issues related to speech articulation, language comprehension, and expression.
Use techniques to treat voice disorders, fluency disorders (e.g., stuttering), and aphasia (language disorders) caused by neurological conditions such as stroke.
Offer therapy for cognitive-communication disorders, including memory, attention, and executive function problems that arise from brain injury or neurodegenerative conditions.
Swallowing Therapy (Dysphagia Treatment):
Evaluate and treat patients with swallowing difficulties (dysphagia), working to improve their ability to eat and drink safely.
Conduct swallowing assessments to determine the cause and severity of swallowing disorders, such as aspiration or choking risk.
Implement therapy techniques, including oral-motor exercises, postural modifications, and safe swallowing strategies.
Recommend dietary changes or modifications (e.g., texture modification) and work closely with other healthcare providers, such as dietitians, to improve patient outcomes.
Patient and Family Education:
Educate patients and their families about the nature of speech, language, and swallowing disorders.

Documentation and Record Keeping:
