Case Managers serve as advocates for Fraser clients, helping them access resources and services within the community. Caseloads:
* DD Waiver & Non-Waiver Case Managers: Provide client-centered services to individuals receiving DD services. Caseloads average about 43 clients (a mix of children and adults). Case Managers meet with clients 2-3 times per year.
* CADI/BI Waiver Case Managers: Provide client-centered services to individuals receiving CADI/BI waiver support. Caseloads average about 35 clients (primarily adults, with some children). Case Managers meet with clients 2-3 times per year.
REQUIREMENTS:
DD Case Managers must have one of the following:
* A bachelor's degree in Social Work, Special Education, Psychology, Nursing, Human Services, or another field related to the education or treatment of persons with developmental disabilities or related conditions. If degree is in Social Work, Social Work licensure is required.
* One year of experience in the education or treatment of persons with developmental disabilities or related conditions (QDDP qualified).
* Completion of at least one course specifically focused on developmental disabilities (Fraser can provide this course free of charge).
CADI/BI Case Managers must have one of the following:
* A bachelor's degree in Social Work, Psychology, Sociology, or a closely related field. If degree is in Social Work, Social Work licensure is required.
* A bachelor's degree in any field and one year of experience as a social worker, case manager, or care coordinator in a public or private social service agency.
* If degree is in social work, licensure is required.
* Two or more years of experience working in service coordination or case management
Both DD Case Manager and CADI/BI Case Manager also require the following:
* Valid Minnesota Driver's License with acceptable record required.
* Effective communication in the English language, including verbal and written communication with both internal and external contacts required.

The HR Operations Associate Representative supports the Fraser Human Resources team-including HR Managers, HR Operations, and Payroll-by collecting, reviewing, filing, and maintaining accurate documentation for all active and inactive employees, in both electronic and paper formats. This role independently processes internal and external personnel changes and requests in UKG for applicants, current employees, and former employees. Responsibilities as a HR Operations Administrator:
When in-office, provide front desk support at the HR facility by answering phones, greeting guests, managing deliveries, troubleshooting office traffic, scheduling HR team meetings, sorting and organizing mail, and maintaining updated contact lists, phone numbers, office tags, and other essential information.
Responsible for transactional data entry, auditing, and processing employment-related documentation based on personnel actions. This includes tracking new hire activities, status changes, terminations, and training requirements.
Ensure accurate filing, scanning, and digital storage of documents in employee personnel files within UKG and other external databases, including I-9 forms and additional documentation as outlined by HR guidelines (e.g., spreadsheets and workbooks).
Collaborate with HR teams-including Compensation, Talent Acquisition, HR Business Partners, Learning & Development, and Payroll-to ensure accurate documentation of staffing changes, organizational growth, and management structure updates.
Effectively manage a high-volume workload and respond promptly to requests from managers, HR team members, employees, applicants, and former employees.
Provide administrative support by creating employee badges, completing verification of employment (VOE) requests, setting up templates, and coordinating with other Fraser sites as needed.
Support post-orientation onboarding by meeting with new hires to collect I-9 documentation, gather required paperwork and signatures, and schedule any necessary training.
Qualifications:
HS Diploma or equivalent required. Post-secondary education preferred.
One to two years administration experience required.
Onboarding experience is preferred.
HRIS and/or other HR database exposure required, UKG experience preferred.
Computer proficiency using HRIS, Microsoft Word and Excel with proficiency in keyboarding skills required.

Requirements:
Case Managers must have the following:
Bachelor's Degree (at minimum) in social work, special education, psychology, nursing, human services or other fields related to the education or treatment of people with developmental disabilities or related conditions.
If degree is in social work, social work licensure is
required.
Either one year of experience with DD (Development Disability) populations or offer will be contingent on taking/passing the Relias I/DD course (paid expense by Fraser).
Experience (or at least a strong interest) in working with adults with serious and persistent mental illness
Commitment to promoting diversity, multiculturalism, and inclusion with a focus on culturally responsive practice, internal self-awareness and reflection.
Other Requirements:
Valid Minnesota Driver's License with acceptable record.

Fraser is looking for Case Managers to work with individuals on the CADI/BI & DD waivers. Case Managers serve as an advocate to Fraser clients and connect clients to resources and services within the community. The ideal candidate will demonstrate compassion for clients and commitment to success. We seek candidates who are well organized, have experience working with developmental or intellectually challenged individuals and have excellent documenting and case noting skills.
CADI/BI WAIVER Case Managers: provide client-centered case management services to individuals receiving CADI/BI waiver services. Case Manager typically have about 35 clients on their caseloads and meet with clients two to three times per year. Caseloads are a majority of adults, with a small mix of children as well.
DD WAIVER & NON-WAIVER Case Manager: provide client-centered case management services to individuals receiving DD services. Case Managers typically have about 43 clients on their caseloads and meet with clients two to three times per year. Caseloads is a mix of children and adults.

Fraser is currently seeking Case Managers to support individuals on the CADI/BI and DD waivers. Case Managers serve as advocates for Fraser clients, helping them access resources and services within the community.
