The ideal candidate should have verifiable experience working directly with physicians in the care of patients and will involve outpatient-only responsibilities and treatment of adult and adolescent patients.
# Job Summary Provides psychosocial assessments, plans and implements various types of counseling, conducts and coordinates care planning for complex cases and participates in a multidisciplinary team process.
#The Therapist#shall work closely with our team of professionals, including physicians, nurses and staff, to provide counseling, education and resource identification while providing quality services related to emotional and mental health.
Must be knowledgeable of State and Federal laws as they pertain to adult and child abuse reporting and domestic violence.
The Therapist#will act as a resource consultant for providers, clinics, and navigators with individualized patient care planning.

Utilizes technology and reports such as EHR.
Qualifications Master#s or Doctorate Degree in Counseling or Social Work from an accredited college or university with a minimum of 3000 hours of experience in social work required; extensive community and mental health experience preferred.
North Carolina State Certification required.
Thorough knowledge of the principles and methods of social work, counseling,#knowledge of State and Federal laws as they relate to social work reporting practices.
Exemplary verbal and written communication, including creative thinking.
Proficient with computer and MS Office software.

POSITION SUMMARY
Facility and community-based, the Team Leader is in constant face-to-face contact with persons served. HCCH clinics participate in the National Health Service Corps loan repayment program Our clinics are located in the mountains of western North Carolina. Job Summary and Responsibilities:
The School-Based Behavioral Health Specialist (SBBHS) is responsible for providing and coordinating behavioral health care for patients at High Country Community Health (HCCH), a fully integrated primary care setting. Services may be provided in a clinic or in a school-based setting. This clinician provides evidence-based behavioral health assessment and treatment services to referred patients, predominantly children and adolescents. The clinician monitors symptoms and response to treatment, and works closely/collaborates with the primary care provider, educational system, school staff, consulting psychiatrist/PMHNP, and resource specialist. There is also the opportunity to supervise graduate-level trainees, including doctoral students from the local university.
Primary Job Responsibilities:
Assess strengths, weaknesses, and presenting problems through completion of assessment tools and intake screening interview
Uses assessment information to formulate a diagnosis and treatment plan
Provide individual and family therapy, as well as consultation with caregivers and teachers related to patients' needs
Collaborate with supervisor, school staff, medical provider and other treatment staff regarding observations and treatment interventions
Complete necessary documentation in accordance with service records requirements, Medicaid guidelines, and other insurance requirements
Creates, reviews, and modifies crisis plan as needed
Monitors, evaluates and records clients progress according to treatment plan
Requirements
Requirements/Qualifications/Skills/Experience
Independently licensed psychological associate, clinical social worker or clinical mental health counselor. Applicants should have experience treating children and adolescents. The candidate will possess experience working within a team-based, interdisciplinary treatment environment.
The ideal candidate will have experience working within integrated primary care and/or school- based treatment settings.
Effective written and verbal communication skills. Demonstrated ability to establish rapport quickly with a wide range of patients.
Ability to work under supervision and collaboratively in an inter-disciplinary team setting. Possess a pleasant, open attitude and a professional awareness of boundaries with patients, co-workers, and supervisors.
Seek supervision and consultation as needed with peers and supervisors.
Demonstrate an ongoing interest in learning. Competent computer usage. Desire to work with an ethnically and culturally diverse population.
Excellent working knowledge of DSM-5-TR diagnoses and evidence-based treatments for mental health conditions, behavioral and relationship challenges.

* Assesses emergencies as appropriate with recommendations for treatment.
* Records actions and interventions in the patient record accurately and completely reflecting the nature of the contact, the condition of the patient and the care or treatment provided or recommended.
* Educates patients and family in the nature of behavioral health related conditions and in the general promotion of positive and supportive lifestyles.
* Serves on assigned committees
* Assists with the preparation of reports on Behavioral Health department issues, concerns, program status, initiatives with the Director of Behavior Health.
* Assists in the provision of continuing education, on-the-job training, and the orientation of RHS staff as requested.
* Utilizes appropriate Behavioral Health policies, protocols and procedures which are in compliance with the most current accepted professional standards. Collaborates with management staff to ensure smooth operation of behavior health services.

Qualifications
Required
* Master's or doctoral degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or related behavioral health field.
* Active, independent licensure in South Carolina (LISW-CP, LPC, LMFT, Psychologist).
* Strong clinical skills in rapid assessment, brief therapy techniques, and behavioral health consultation.
* Familiarity with PCBH and CoCM workflows, MAT treatment protocols, and team-based integrated care.
* Comfort working across a range of behavioral health and chronic disease presentations.
Preferred
* Experience in FQHC or rural health settings.
* Bilingual (English/Spanish) or multilingual capabilities.
