    Job Summary:# ##The DRN role is a non-24/7 nursing role, that crosses all continuum of health care sites functioning in the role of expert clinician, educator, and nurse care manager for diabetes patients across the continuum of care. # In collaboration with the medical and ancillary staff, monitors the clinical care of patients and provides clinical support to improve patient care and outcomes, for both inpatient, outpatient, and across the continuum of care. Provides a significant level of disease-specific education to patient and hospital staff. Participates in assessing, planning, implementing, and evaluating health services of a disease specific population during hospitalization, post discharge, and site of service across the continuum. Performs tasks requiring independent knowledge and judgment. Implements performance improvement initiatives. Provides community outreach activities. Serves as a liaison and resource among patients, families, healthcare providers, staff, and community to provide a seamless transition throughout phases of care. Collects data for diabetes quality improvement initiatives and participates in implementation and management of performance improvement initiatives. Responsible for other Diabetes Services activities and assignments per Diabetes Services leadership as other duties as assigned. Qualifications:# # #Associates# degree from a recognized college or university required. Bachelor#s degree preferred. Unencumbered RN license to practice in NC (NC license or multi-state compact license). At least 3 years of Acute Nursing or Clinic Nursing experience required. Prefer 2 years of Diabetes Educator experience. Prefer 2 years of Educator or equivalent experience. At time of hire candidate preferred with Nursing Specialty Certification (Certified Diabetes Care and Education Specialist= CDCES). At time of hire 15 CEU#s are required within the 1st three months of employment per ADA guidelines and maintained per calendar year thereafter. CDCES is preferred by Service Line leadership and CaroMont Health for the American Diabetes Association (ADA) Recognition. BLS required. Require ability to work in a self-directed environment to work with the multidisciplinary team to communicate with all members on behalf of the patient. Excellent communication, organizational skills, problem solving, interpersonal, customer service, teaching, and coaching skills. Assist physicians and other health care providers with routine examinations and procedures, as well as procurement of equipment and supplies. 2. Critically review patient care documentation making note of any significant changes in the patient's condition and make an immediate report to a registered nurse or physician. 3. Provide a variety of clerical tasks to include but not limited to organizing and prioritizing workload, greeting and directing visitors, researching information as requested by nurses and physicians, reviewing orders for logic and clarity, revising orders as requested by the nurse, answering the telephone/ patient call lights, transcribing physician orders, and maintaining medical records. Perform routine quality assurance audits, safety checks, and inventory of supplies. 4. Provide administrative and clerical support including PI audits and data entry. 5. Provide clinical support assistance to include, but not limited to, cleaning the work area, maintaining office equipment, troubleshooting common software and hardware problems, changing linens, checking supply inventory, re-stocking supplies, transporting patients/ supplies/ specimens, and assisting in the orientation of new employees and students. 6. Provide direct personal care to patients as outlined in plan of care activities such as bathing, dressing, feeding, assisting with ambulation. Measure and record vital signs, and patient height and weight. May be responsible for specimen collection, skin preps/scrubs, catheter care, capillary blood glucose checks, simple wound care, range of motion exercises, and/or postural drainage. Performs Oxygen room set-up and monitors oxygen flow-rate, performs oropharyngeal and nasopharyngeal suctioning, tracheostomy care, fecal impaction removal, sterile dressing change wound irrigation, IV fluid assembly and monitoring, IV site care and discontinue peripheral IV, ostomy irrigation and care, urinary catheterization and irrigation, performs established oral, nasogastric, gastrostomy tube feeding 7. Reinforce routine teaching given to patients by the registered nurse or physician and document patient understanding of instructions.
