Position Description: The Integrated Team Based Care (ITBC) Older Adult Manager is responsible for providing clinical oversight, support, and professional development for assigned staff. This position is part of a matrixed leadership structure and is responsible for collaborating with ITBC leadership to create a robust system of support and accountability for all ITBC staff. This role works collaboratively with other Integrated Team-Based Care leadership to ensure staff meet program obligations including compliance with state licensing standards, county requirements, and internal policies. This role is also responsible for developing best practices, and serving as the subject matter expert for assessments, treatment, harm reduction, case presentations, and diagnostic methodologies. This position must understand and appreciate differences within our Cascadia community. As such, it is expected that this position promotes integrated care, our vision of trauma-informed and person-first approaches and helps create a work environment of safety and acceptance. General: In collaboration with leadership team set annual goals and objectives aligned with integrated treatment and the agency's mission; participate in on-going review and revision of processes. Assist with the development of an operating budget to support the programs goals and objectives. Serve as Leader of the Day (LOD) weekly, handling client calls and crises. Provide resources and tools to supervisees to ensure quality work. Collaborate with leadership to develop clinical guidelines and operational workflows. member of ITBC leadership, Provide clinical consultation to team as needed Coordinate with the leadership team to offer one monthly clinical training for staff. Advocate and collaborate with leadership teams, executive management, and other departments. Attend both internal and external meetings to problem-solve, advocate for programs, and maintain positive working relationships with employees and stakeholders. Oversee and leverage clinical data to monitor performance and assist in decision making. Serve as a liaison between departments, facilitating collaboration among interdisciplinary teams, and communicating with stakeholders. Perform other duties as assigned. Regulatory & Compliance: Comply with laws, regulations, policies, and procedures under which the program and company operates. Comply with and manage relevant Contracts, Memorandum of Understanding (MOU), and Grants. Assist in crisis situations as appropriate; follow protocols and procedures in coordination with the treatment team. Respect and honor client rights and responsibilities and demonstrate professional boundaries and ethics. Maintain accurate and up-to-date documentation that ensures compliance with all agency policies and procedures and local, state, and federal regulations, and billing requirements. Adhere to mandatory abuse reporting laws and HIPAA requirements. Understand safety regulations and evacuation procedures; participate in regular safety drills in compliance with program policies and procedures. Complete assigned employee training in compliance with program and company requirements. Provide guidance, support, and training to staff in alignment with company mission and values. Participate in all scheduled staff meetings, supervision sessions, and other departmental and company meetings. Supervisory Oversight: Provide regular supervision, coaching, and annual training oversight for direct reports. In collaboration with program leadership, determine staffing requirements, recruit, hire, and train staff in a manner that encourages and maintains Cascadia's Values. Establish work plans and priorities for staff members, assess the progress of staff members. Promote and complete annual employee training requirements on a timely basis. Demonstrate commitment to continuing professional development. Provide direction and coach staff in Standard Operating Procedures (SOP), Electronic Health Records (EHR), Trauma-Informed Care (TIC), Feedback Informed Treatment (FIT), evidence-based practices, community resources, crisis management, harm reduction, Oregon Administrative Rules (OAR), HIPAA, and mandatory reporting. Participate in on-call rotation, providing after-hours support when necessary. Leadership: Take the initiative in thought leadership, innovation, and creativity. In collaboration with leadership team develop, review and revise clinical guidelines and operational workflows; as well as set annual goals and objectives aligned with Cascadia's mission. Create, model, and maintain a work culture that supports Cascadia's Values. Attend and participate in all meetings as assigned, including but not limited to staff and team meetings, consultations with staff, individual and group supervision. Develop and maintain strong relationships with internal and external customers to promote quality and efficiency in Cascadia operations. Perform other duties as assigned. Qualifications Education: Required: Master's Degree in a behavioral science field from an accredited college or university. Must demonstrate the competencies as outlined in the Oregon Administrative Rule's (OAR) to become credentialed as a Qualified Mental Health Professional (QMHP). Experience: Required: Minimum five years of experience in community mental health or related field; 3 years clinical experience providing community behavioral health services AND a minimum of 2 years staff supervisory experience. Preferred: Clinical experience working with an older adult population. Specialized Knowledge, Skills, & Abilities: Understand the unique clinical needs of individuals served with complex medical conditions. Understanding of recovery model, motivational interviewing, illness management and recovery, evidence-based practices, and strength-based case management philosophies and strategies. Understand and align with harm reduction philosophies, cultural responsiveness, and trauma informed care. Operate computer hardware and software at a level needed to effectively perform job functions. Other: Ability to commute to other locations. Ability to perform BLS, First Aid, and effectively use AED equipment. BLS/ First Aid certifications are required and must remain current. Working Conditions Environmental: Work is performed in one or more of the following: administrative and clinical offices, acute care settings and community setting including client residences, correctional facilities, shelters, hospitals, public places and other social service sites. Meal periods and breaks are subject to interruption. Universal Precautions and remaining alert to the environment are critical to address the potential for violent behavior, exposure to disease, biohazards, noise, and contaminants. Mental Demands: The work assigned is diverse and involves addressing new and unusual circumstances. Program Description: As part of Cascadia Health, the Child and Family Program provides services to youth and families across the Portland metropolitan area, with primary locations at each Cascadia Health Center. Position Description: As the licensed medical provider for the Cascadia's Child and Family program you will provide psychiatric assessments and ongoing medication management to consumers under 18 years old and to Transitional Age Youth receiving services at Cascadia clinical programs, while supporting a Trauma Informed Care environment. With the high prevalence of trauma among individuals receiving mental health services, it is required the Licensed Medical Practitioner (LMP) understand the effects of trauma on health, coping, and other aspects of the lives of those we serve. Depending on the program served, services may range from one time consultations to internal staff or external partner providers/agencies to ongoing services. Provide formal and informal case consultation to case managers, therapists, nursing staff, and other treatment team members. Coordinate with primary care and other external care providers serving individual consumers, as per community standards. Provide case consultation and support to family and/or other members of individual consumer's support systems, as needed. Compensation: Varies by credential and experience. PMHNP/DNP: $72.03/hr. ($149,794/yr.) to $79.22/hr. ($164,782/yr.) Psychiatrist: $123.75/hr. ($257,385/yr.) to $136.11/hr. ($283,113/yr.) Document medical supervision of cases by signing comprehensive mental health assessments, updates, and individual support and service plans. Provide medication consultation and management for consumers/parents/guardians, including supplying prescriptions and providing ongoing assessment of medication effectiveness, safety, and appropriateness. Provide urgent back up services for consumers enrolled in the same services/program whose primary LMP is unavailable. Respect and honor consumer/parental/guardian rights and responsibilities and demonstrate professional boundaries and ethics. Assist in crisis situations as appropriate, following emergency protocols/procedures and coordinating with the treatment team with interventions which support a trauma informed environment for clients/residents and staff. Adhere to mandatory abuse reporting laws and HIPAA requirements. Deliver timely, accurate, and professional documentation. Provide input on all relevant documentation and procedural requirements. Complete annual employee training requirements on a timely basis as indicated in the licensing requirements for the facility as well as agency requirements. Attend seminars, training and other educational opportunities in order to develop professional skills and abilities. Participate in all scheduled staff meetings, supervision sessions, and other departmental and agency meetings. Perform other duties as assigned. Qualifications Education: Must have either of the following: Active state medical license, including maintenance requirements and DEA certification. Certification (or eligibility for certification) by the American Board of Psychiatry and Neurology for general psychiatry, OR Certification (or eligibility for certification) by the American Board of Psychiatry and Neurology for child and adolescent psychiatry. OR Active State Nursing License as a PMHNP/DNP, including maintenance requirements and DEA certification. Certification by the American Nurses Credentialing Center. Experience: 2-5 years of clinical experience with a community mental health clinic population is preferred. 2 or more years of child and adolescent psychiatry experience preferred. Specialized Knowledge, Skills and Abilities: Sufficient computer skills to complete documentation in an electronic medical records system in a timely manner Provide care in a setting consistent with the principles of collaborative care informed by stages of change models Other: Ability to perform CPR, First Aid, and properly use AED equipment. CPR/ First Aid certifications are required and must remain current. Working Conditions Work is performed in a typical clinical setting. As such, the setting is not secure, and the LMP will be responsible for creating a safe environment within the meeting area. Employee will be provided with a private space, as needed, in which to meet with consumers, and have access to appropriate tools with which to meet the requirements of their work (such as computer, printer, scale, and other). Mental Demands: Assigned work includes interacting with a variety of consumers, some of whom may present emotional and/or behavioral challenges. The LMP is expected to respond with appropriate professional interventions. Busy schedules will demand a high degree of time management skills, including triage and prioritization skills.
