We are currently seeking a full time Forensic Case Manager at our new Outpatient Restoration Long Term Structured Residence (ORLTSR), located in Easton, PA. The ORLTSR is a therapeutic residential program that works with justice-involved adults. The individuals in the program have a diagnosis of serious mental illness and have been prosecuted on criminal charges. The program will provide mental health treatment to those who will be admitted on Conditional Release or those in need of competency restoration. The Forensic Case Manager works from a recovery framework within the team providing providing care management services, therapeutic interventions, cultural competence and trauma informed care to help individuals obtain their recovery goals. The Forensic Case Manager will coordinate all services and follow up related to Northampton County competency hearings and mental health court hearings and communications; view individuals form a strengths approach in preparation for their court hearings. This position is responsible for acquisition and coordination of all services required by residents in order to attain treatment goals. Additional responsibilities of the Forensic Case Manager includes, but is not limited to; supervision of resident fund management, engagement in resident advocacy with internal and external programs and agencies, and collaboration with the Director and Assistant Director in relation to all admission screenings and negotiations regarding referrals, transfers and discharges. Ensure that all required documentation is completed efficiently and thoroughly maintaining compliance with all internal and external requirements and regulations. Crosstrain with Residential Services Advocates to provide trauma informed support to Safe House residents and their children by assisting in maintaining a welcoming, safe, and supportive community living environment. Conduct safety/security rounds in the building during assigned shift; complete necessary verbal and written reports; complete logs as required; follow up on safety-related issues and concerns; and contact medical, law enforcement, and fire personnel in the event of an emergency. Utilize language access services to effectively communicate with LEP individuals. Maintain records in accordance with agency standards and complete and submit required documents as assigned. Arrange transportation for clients when appropriate. All other duties as determined by the Department Director. This position is considered essential. To ensure continued 24/7 operations this position is required to work onsite during inclement weather, outages, emergencies, and/or authorized administrative office closures. Crisis Intervention Specialists are available 24/7 on a rotation basis to assist our consumers in times of need. You'll work to determine the immediate safety steps to be taken and provide appropriate interventions and resources that address the current mental health needs. Duties and Responsibilities During non-operating hours of the clinic, participates in on-call rotation to include evenings, holidays, and weekends in accordance with program expectations. Telephonic and/or Mobile Crisis Intervention responses are required during these time periods. Obtains and/or reviews initial intake/referral/payer information and documents in Electronic Health Record (EHR) system. Conducts follow-up calls to consumers of Crisis Intervention services as requested by Clinical Supervisor and in accordance with practice standards. Supports, educates, and assists with training/mentoring other on-call staff responsible for working in the crisis program. Maintains proper documentation of all consumer related contacts and ensures documentation is concise, timely, and congruent with practice standards for crisis service delivery. Hours of Work Schedule and hours are varied as part of an on-call rotation.
