Current active unrestricted license such as RN LCSW LMHC LICSW LPC (as allowed by applicable state laws) LMFT LMSW (as allowed by applicable state laws) or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States required.
Preferred Skills, Capabilities, and Experiences:
Previous experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders.
Managed care experience preferred.

Job Summary
Under the general direction of the Assistant Dean of Students, the Case Manager performs a variety of functions and responsibilities related to the delivery of non-clinical case management to students through outreach, advocacy, support, and helping students connect to university and community resources.
Essential Functions
Provides a continuum of case management services to students navigating complex and/or extenuating circumstances related to personal, academic, mental health, physical health, and psychosocial adjustment. This includes, but is not limited to, conducting non-clinical assessment of concerns/referrals, meeting with students, connecting students with campus and community resources, collaborating with family members and campus partners in supporting students.
Reviews referrals from families, faculty, staff, and other members of the campus community regarding new and existing concerns. Autonomously manages a caseload of students, providing individualized case management, tailored intervention, support and connection to campus and community resources, as well as consistent follow up to students until the concern is resolved or the student has successfully connected to an appropriate source of support. Connects with parents/guardians and/or designated emergency contacts as appropriate.
Uses sound professional judgement and university policies to determine the appropriate follow up for the student and the referring party, referral to the CARE or BIT teams are needed, if emergency services need to be alerted, and/or if emergency contact needs to be notified.
Serves as a primary point of contact for emergent calls and walk-ins regarding students in distress. Provides consultation to faculty, staff and families who are seeking guidance and advice about Dean of Students Office and campus services for students who may be experiencing distress or managing extenuating circumstances.

Compensation
Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $19.50 - $24.00. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.

Link youth to appropriate community services and monitor progress as appropriate within the case plan.
Model, teach and role play life skills necessary to maximize the development of a stable living arrangement, to prepare the residents for future independent living.
Develop a thorough and current knowledge of all community service resources available to the Compass House client population.
Advocate for the residents as needed with other agencies to eliminate barriers leading residents to their goals.
Accompany residents on job searches, interviews, Department of Social Services, DMV office, etc. as needed.
Make sure clients maintain clean and orderly bedrooms and common areas. Teach the residents as needed how to complete chores and maintain a clean and safe living environment.

This is a grant-funded position, with reappointment dependent on grant renewal and funding availability.
Essential Functions
Provide short term peer counseling; crisis intervention; medical, legal and academic advocacy; safety planning; case management; and facilitate related support groups throughout Hampshire County.
Responsibilities can include meeting and communicating with other service providers about clients.
Comply with all relevant regulations, policies, standards, and protocols including those regarding privileged communication between rape crisis counselors and victims, and mandated reporting of abuse.
Assist to administer and participate in the daily operations of 24 hour rape crisis services including ensuring scheduling and staffing of hotline, and providing and/or scheduling back-up to ensure coverage. Participate in training and selection and can provide supervision and support to program staff.
Consult and participate in implementation of violence prevention outreach, education, and training for diverse populations.
