Position Summary
This position is responsible for assessing, documenting and monitoring the overall functional, physical and behavioral health status of members assigned to them. Based on the assessments, the casemanager, collaborating with the member and his/her support system, develops a service plan that meets member needs in the most cost-effective and most integrated setting.
Core Functions
Is the primary contact for the ALTCS member, explaining the program to members, including their rights and responsibilities, the grievance and appeal system and other information according to regulations.
Comprehensively assesses and documents the member's bio psychosocial functioning in accordance with AHCCCS time frames, identifying the individual's strengths and needs.
Develop and implements a service plan based on the member's strengths, needs and placement preferences, authorizes and coordinates with provider agencies.
Assists the member to define personal goals, identifying barriers to achieving these goals and encouraging the member to resolve the difficulties identified.
Acts as a facilitator and/or advocate for the member in dealing with issues with providers, community programs or other organizations.
Acts as a gatekeeper to ensure that the member is receiving the most appropriate, cost-effective services in the most appropriate setting.
Facility based while remaining within budgetary allowances. Internal customers: all levels of nursing management and staff, medical staff, and all other members of the interdisciplinary healthcare team. External customers: physicians, payers, community agencies, provider networks and regulatory agencies.
Minimum Qualifications
Knowledge, skills and abilities as normally obtained through the completion of a bachelor's degree in social work, and two years of experience serving persons who are elderly and/or persons with physical disabilities or who are determined to have a Serious Mental Illness (SMI).
Preferred Qualifications
Bilingual, preferred in some assignments.

Your Role at Axis for Autism:

Who You Are:

What your day will look like at Prisma Community Care:
You will conduct comprehensive psychosocial and program / service eligibility assessments and reassessments of casemanagement clients, showcasing timely and coordinated access to medically appropriate levels of health and supportive services, as well as continuity of care for those living with HIV, as indicated by the Ryan White Program managed within Maricopa County, Arizona.
You will curate and complete individualized service plans with clients to set goals and strategies to overcome barriers to maintaining medical care, which could include but is not limited to mental health, chemical health, housing stability, or criminal history. Service plans will include continuous client monitoring to assess the efficacy of the care plan, with re-evaluation every six months and adaptations as necessary, and the primary objective to become self-managed in their care.
You will close gaps in medical care through effective guidance and support that empowers clients to attend medical and wellness visits, ensuring access, follow-up, and adherence to treatments and other requirements, including supportive services that can enhance harm reduction, behavior change, and maintenance.
You will manage an active caseload of approximately 50 - 80 individuals that require casemanagement services, ensuring a productivity level of at least 60% of time spent is billable to the Ryan White Program.
You will demonstrate culturally appropriate and culturally reactive engagements with all individuals within the community, performing services, documenting records and referrals, as well as communicating with clients via telephone, email, and in-person.
You will provide clear, understandable, current, and accurate information to clients, community members, and volunteers. Provide prevention and harm reduction information in a way that shows respect, listening skills, empathy, and patience in a status-neutral approach.
You will record all contacts and activities into all system databases and EMR within 24 hours of the encounter, completing and submitting all relevant information in accordance with state policy, including documentation into appropriate databases and reporting systems, and finalizing all documentation within 72 hours of actions being completed.
You will submit billable units from each previous calendar month into the appropriate state database by the third day of every month.
You will provide information and referrals for other partnering agencies that provide additional support and services to include social and medical needs that cannot be met by Prisma.
You will eliminate barriers to care through insurance navigation and enrolling patients in qualifying patient assistance programs that increase access to care. Insurance navigation includes assisting patients in the processes of enrolling in State Medicaid (AHCCCS).
You will stay informed, engaged, and current with emerging research, trends, and best practices in the field of sexual health and wellness, gender identity, intersectionality, and wellbeing of all.
You will work collaboratively with other teams to further develop and evolve the needs of the department and organization, including but not limited to, policy reformation, cross-training, and identifying opportunities for growth and improvement within teams.

Your Role at Axis for Autism:

Conduct face-to-face supervision for assigned clients
Communicate regularly with parents to facilitate the clinical objectives of the client's treatment plan

The Adult CaseManager provides referral to appropriate ABHS services and acts in collaboration with assigned clinical team. This position coordinates appropriate services between the client, client support system, community, and other healthcare and social service professionals. The Adult CaseManager is responsible for provision of transportation services for clients and families, assisting with participation in medically necessary services. This position will also act as an advocate for the client with other service providers as appropriate.
DUTIES AND RESPONSIBILITIES:
Provides support services assistance to ABHS clinical staff, including, but not limited to, transportation, community visits, appointment setting/coordination, outreach services for clients having difficulty accessing services, and/or for reengagement in treatment services.
Facilitates groups, including, skill training, peer support, and education programs in coordination with ABHS clinician.
Adheres to ABHS vehicle policies and maintains appropriate vehicle utilization records.
Documents contact with clients to assess needs, maintain required paperwork, and engage family members.
Provides prevention education services including presentations, workshops, and other forms public education on a variety of mental health topics, including, but not limited to, substance abuse treatment education and life skills instruction.

Ability to address a variety of diverse spiritual, emotional, and social needs in multiple environments of an often-high-risk population, using a trauma-informed care approach while maintaining a high degree of professionalism and cultural awareness.
Collaborates with court/probation, tribal/community agencies, families, and other natural support systems to ensure client's needs are met. Develops a strong working relationship with community partners.
Provides services at the clinic, in the community, adult shelters, residential facilities, hospital settings and in client's homes to ensure client obtains all benefits and services necessary.
Participates in interdepartmental clinical team meetings to assist and encourage consistent and timely provision of services. Participates in weekly clinical staffing with other members of the clinical team to review client progress and service requirements.
