The Caseworker is responsible for integrated client services from initial placement to permanency as well as foster family services from family application, opening, and on-going family assessment and support. This is a ministry position where the main responsibility is to meet the above definition through service while sharing the Gospel in speech and action in alignment with all employment qualifications outlined in the employee handbook. What you'll do Foster Placement Specific Responsibilities: · All placement services from the time a child is placed through a child's final day of care with Connected. Including: o Placement in an appropriate Connected Foster Home per policy. Meet with foster parents/placement providers to assess the provider and clients' needs to address any concerns at least monthly. o Facilitating and participating in case “staffings”: both internal and held by external providers. o Attend court hearings as approved by your supervisor. o Establish necessary services for clients as part of a team with the clients' foster placement, therapy providers, and other providers to meet client's needs in a timely manner. o Plan and calendar a minimum of 1 face-to-face visit per month for each assigned client in the client's placement. o Assist with visitation plan for client(s) and biological or adoptive family according to policy. o Serve as the primary point of contact for any and all placement changes including new placements in, discharges out, and disruptions from the home. Essential Duties and Responsibilities: The Bilingual Family Intervention Specialist: * Carries a caseload of 4 to 6 families * Holds individual and family sessions with each family 3 times a week, scheduled at the convenience of the families * Conducts on-going assessment of youth to determine their needs from a strength-focused, solution-based perspective * Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development and quarterly booster trainings to enhance clinical skills * Drives up to 60 to 80 miles to meet with families in their homes and communities (additional travel may be required) * Collaborates with other providers, case workers, and courts to formulate a collaborative treatment plan * Provides on-call crisis support to the youth and family (schedules vary by location) * Completes accurate and timely documentation in an electronic medical record system (EMR) * Performs other duties as assigned Additional Information: * Schedule is flexible and non-traditional as it is based around the availability of youth and families served. * Applicants must possess a current, valid driver's license, an automobile for work purposes, and proof of auto insurance. * Community-based staff will be reimbursed for applicable mileage. As requirements for certification are met and caseload increases. After 60 days and when caseload increases to 4 pay will increase to $43,000. The Family Centered Treatment (FCT) Clinical Caseworker provides structured, goal oriented and time limited Family Centered Treatment (FCT) in the natural environment to families who need assistance recovering for a variety of issues including but not limited to mental health needs, substance abuse, physical, sexual, and emotional abuse and neglect. Our community-based Clinicians focus on the families' strengths and empower them to make changes that have a lasting impact. Clinical Caseworkers will receive training and certification in the Family Centered Treatment model. This is an innovative evidenced-based approach which achieves effective outcomes. What you'll do FCT DUTIES: 1. Complete and maintain all relevant FCT training and make consistent progress toward achieving the FCT Provider Certification from the Family Centered Treatment Foundation, including video recorded demonstration of competencies in accordance with the established training contract. Clinical Caseworkers will complete all activities necessary to maintain certification from FCTF including recertification every two years. 2. Provide at a minimum two, multi-hour face-to-face sessions per week with each family system with the exception of titration at discharge. Work on a schedule that is flexible and able to meet the needs of each family they serve. This will require meeting in the evenings and on the weekend when necessary to meet client needs. 6. Work as a member of a provider team that meets weekly. Participation in team meetings is mandatory and requires clinical caseworker to serve as facilitator and recorder on a rotating basis 7. Participates in weekly case staffings, child/family team meetings, and other meetings as needed with multi-disciplinary teams 8. Provide thorough documentation including FCT fidelity documentation and progress notes, monthly reports and assessments. The FCT Clinical Caseworker position is expected to provide 40 hours or more per week including evenings and weekends as needed. Qualifications A bachelor's degree in a human services field. Two (2) years' work experience in a human services field preferred
