Develop therapeutic relationship with team and client. Supports and advocates on behalf of client in the procurement of services and benefits entitlement, assisting with required paperwork as necessary.
Maintains complete case documentation and records, as required by Catholic Charities, the funders, or licensure/regulatory bodies. Ensure confidentiality of all records and interactions.
Coordinate services both internally and externally including county casemanager, family/guardian of client, work personnel, medical personnel, etc.

What will you do?
The Mental Health casemanager will offer direct assistance to a diverse range of individuals, fostering trusting relationships while being attuned to their experiences and traumas. This role includes administrative responsibilities such as completing essential paperwork, documentation, and billing processes, as well as offering referrals and coordinating cases for clients seeking a range of services, including mental health, medical, financial, social, educational, vocational, housing, and legal assistance.
Responsibilities are executed in compliance with Rule 79, which includes documentation, functional assessments, crisis plans, releases, and personalized community support plans, all while adhering to established timelines and performance standards.
Regular interaction with clients is expected, occurring at least once a month or as required. Additional responsibilities may include building relationships with county and state officials, family members, employers, and other organizations to improve the support available to clients. Over time, this position will promote personal development and skill enhancement while aiding in the growth of our company's division.

Assists in the care coordination of complex patients to facilitate the delivery of cost-effective health care and identify the appropriate utilization of resources. Coaches patients and parents/caregivers in developmentally appropriate positive parenting style.
Works collaboratively with interdisciplinary staff internal and external to the organization. Participates in quality improvement and evaluation processes related to the management of patient care.
RESPONSIBILITIES:
Explain policies about teen confidential services to patients and parents during the first visit

General Job Statement:
The Adult Services CaseManager focuses on supporting African American men and single adults in their journey toward personal and professional growth, improved health outcomes, and improve quality of life. Minimum Qualifications:
Bachelor's Degree in Psychology, Sociology or closely related field, preferred. Associates Degree with two (2) years of casemanagement experience in a non-profit setting focusing marginalized communities, or a High School Diploma or GED with four (4) years' experience in casemanagement in a non-profit setting working with marginalized communities.
Two or more years of proven experience and the ability to perform intensive casemanagement and programmatic functions such as tracking outcomes, implementing service coordination and assisting with program development and outeach for adults.

Job Summary: The Ryan White Non-Medical CaseManager (NMCM) provides supportive casemanagement services to individuals living with HIV, focusing on improving access to and retention in needed core medical and other support services. In the context of an integrated collaborative team, this position independently carries a caseload of clients, leads the delivery of social services and benefits navigation, and is responsible for ongoing documentation and data reporting to funders. The NMCM provides coordination, guidance, and assistance in accessing medical, social, community, legal, financial, employment, vocational, and/or other needed services. Services are delivered within a harm-reduction model, utilizing a nonjudgmental approach to clients' substance use and other factors. This position offers an opportunity to make a significant impact in the lives of individuals living with HIV, contributing to the efficiency and effectiveness of their care and support systems.
Duties and Responsibilities:
Provide the following services to clients:
Conduct initial assessments for eligibility determination and service needs identification.
Develop individualized care plans that respect the client's cultural values, addressing barriers to care including homelessness or injection drug use, in a nonjudgmental manner.
Continuous client monitoring to assess the efficacy of the care plan
Re-evaluation of the care plan at least every 6 months with adaptations as necessary
Provide education and guidance to clients about HIV care and treatment options, ensuring timely and coordinated access to medically appropriate levels of health and support services and continuity of care
Assist clients in navigating benefits and healthcare systems.
Assist clients in accessing available resources, such as food access, social supports, housing resources, and specialty healthcare.
Support clients in transportation needs, and accompany patients to appointments as appropriate.
Advocate for clients and review their utilization of services, ensuring they are client-specific.
Ongoing assessment of the client's and other key family members' needs and personal support systems
Consistently and efficiently tracking and reporting Ryan White required data
Work in coordination with another Ryan White Medical and Non-Medical Casemanagers
Work collaboratively with an interdisciplinary team, including directly with the HIV clinical care team.
Ensure a high level of confidentiality and sensitivity while dealing with clients' personal information.
Engage with clients using various methods (phone, in-person, outreach, during clinic visits) including harm reduction-based community outreach.
Maintain ongoing documentation in compliance with Ryan White program requirements.

Qualifications:
