Responsibilities for Senior Case Manager: Provides case management - Community Psychiatric Support Treatment (CPST) services and Therapeutic Behavioral Services (TBS) to persons with severe mental illness in an outpatient facility and in the community. May provide treatment to persons with substance use disorder as a secondary diagnosis. Initiates, coordinates, and monitors evaluations necessary to identify each client's needs in areas to include: living arrangements, financial issues, benefit status, legal status, physical/psychiatric health, ac tivities of daily living, vocational/educational, social supports, leisure and recreation. By assertive outreach, attempts to engage the client in identifying and expressing his/her own needs, offers services to assist in meeting these needs, and re-assesses outcomes with the client on a regular basis. Gains knowledge of the following: mental health treatment and basic needs resources, informal and formal linkages with agencies that provide services, the client's use of services, and applicable costs of use, and the opportunities for alternative resources. Serves as a liaison with other service staff. Delegates services to secondary and supportive service providers; and monitors and evaluates the outcomes of these services. Assists the client to achieve goals and maximize independence and productivity through support, teaching, and training in the use of personal and community resources. Reviews and monitors services and activities and documents progress according to organization process Assists in onboarding and initial training of new care managers. Serves as a resource to less experienced care management staff May serve as a representative of GCB in partnerships and shadowing experiences, such as CIT police shadowing. Provides services to individuals in Cincinnati/Hamilton County or Amelia/Clermont County. Position Requirements: Bachelor's or Master's degree in a Human Services field or High School diploma and a minimum of three years experience in Behavioral Health, Mental Health or a Human Services related field and two years of experience as a Mental Health Care Manager. Must be certified as a CDCA (Chemical Dependency Counselor Assistant) in the State of Ohio Knowledge, Skills and Abilities Required Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. Records maintenance skills. Ability to gather data, compile information, and prepare reports. Ability to monitor, assess and record client progress against care plan, and make adjustments to plans and services. Ability to assess recovery status in clients and to develop individual treatment goals and plans. Knowledge of community health care and vocational services. JOB RESPONSIBILITIES: As Case Manager, CDCA, you will: Facilitate client access to community resources, including locating housing, food, clothing, school programs, vocational opportunities or services, providers to teach life skills, and relevant mental health services; assist client to develop natural resources and make contact with social support networks. Coordinate and monitor services, including comprehensive tracking of client activities in relation to treatment plan. Assist clients in developing goals and areas of need and assist in developing treatment plans which are assessed regularly; conduct medication and recovery status assessment and determine required level and frequency of services. Document all client encounters and contracts made on behalf of clients; complete and submit billing documentation as appropriate; maintain comprehensive client files, which may include documents held for safekeeping on behalf of the client. Identify and provide emergency crisis services as necessary; make immediate clinical assessments and respond according to accepted crisis intervention methods and techniques; coordinate other services as appropriate. Assess clients, evaluate effectiveness of treatment plan and progress made by client, and participate in client treatment planning and case review with client care providers.
