Classification Purpose: The primary purpose of the CaseManager classification is to determine a client's eligibility for and monitor a client's participation in a variety of public assistance programs and services, including employment services, financial assistance, food stamps, child care, medical care or other community and social programs and services.
Job Duties: Interview clients to assess needs and eligibility for TANF, Disability Assistance, Medicaid, Food Assistance, HealthChek, Food Assistance Investigation and Prosecution, Medicaid Transportation, SSI CaseManagement, At Risk CaseManagement Transportation, and/or Refugee Medical. Gather and document personal information, financial and employment information, verifications and other documentation as required by program rules and guidelines. Conduct interviews in person, over the phone, or in the field. Authorize or deny participation in and payments from public assistance programs and services.
Develop cooperative plan to meet client needs. Assist clients in obtaining, understanding and utilizing services. Provide information and answers questions regarding different programs. Advise clients regarding rights and responsibilities for participating in programs and services. Make referrals and recommendations. Establish goals and timelines; identifies service delivery problems or barriers and initiates problem resolution. Conduct case conferences, as required.
Provide ongoing casemanagement and oversight. Analyze the appropriateness and effectiveness of case plans and services utilized by clients. Monitor compliance with participation requirements. Research case inquiries, case alerts, match listings, case discrepancies or special reviews. Make adjustments to cash payments or participation in programs, as needed. Act as agency representative at state hearings. Prepare necessary documentation. Take action as instructed.
Maintain related documentation and reports as required by local, state and federal guidelines. Gather statistical information regarding work activities. Maintain contact logs. Document all work activities in case files. Attend conferences, workshops, and training, as required. Maintain regular and predictable attendance.

Incumbent is responsible for providing home-based intervention, advocacy, counseling, and education for families involved (or at-risk of involvement) in the Juvenile Court. Incumbent reports to the Family Advocate Coordinator. This unique opportunity offers the following advantages/benefits:

The ideal candidate will be a LISW, LPCC, or LCSW with licensure through the State of Ohio or MSW or LPC working towards independent licensure. Must assume a flexible work schedule, be available outside normal business hours, including evenings, and have access to reliable transportation. Work consists of complex, varied, non-standardized tasks, requiring the application of numerous laws, rules, procedures, and ethical principles.
* Provides home-based and/or teleconference intervention, psychoeducation, counselling and therapeutic support for court-involved youth and families;
* Assesses family function by observing family interaction and using evidence-based assessment tools;
* Utilizes Functional Family Therapy and/or other evidence-based practices with youth and families;
* Develops and updates case plans as needed;
* Complies with all documentation, supervision and training requirements designed for the program and licensure.
* Provides various data, reports outcomes, and documents ongoing program statistics and produces related reports.
* Maintains positive working relationships with juveniles and families of diverse cultural, racial, religious, and socioeconomic backgrounds;
* Collaborates with court staff, various county agencies and other service providers to ensure juvenile and/or family progress and success;
* Provides verbal progress reports at court hearings;
* Participates in group supervision and/or process meetings;
* Maintains Licensure through the State of Ohio.

Crisis Intervention Specialist
Duties: In this role, you will be provide crisis intervention services to individuals in community settings in Licking County. Services being a Public Health Officer, with the ability to write involuntary admissions, and completing Hospital Pre-Screens for Psychiatric admissions or community safety plans, as well as mobile crisis services for children and adults. Each staff is designated a specific shift and location, for a 40 hour work week, with the addition of a one (1), twelve (12) hour On Call shift a month, and on a rotation basis to provide a required holiday coverage, once or twice a year. Staff are compensated for On Call hours and overtime for any worked hours during that On Call shift. Each staff has access to the Crisis Supervisor 365 days of the year, 24/7, for consultation purposes, as well as to team members working the same shift. There are no productivity requirements or on going caseloads, and each day is varied, can be fast paced and often very interesting. Opportunities for additional training and advancement in professional growth are available, and encouraged.
