Primary Objectives of Position: Functions as the central focus of services to job seekers and referred customers. Provides eligibility determination, orientation, and assessment. SUMMARY DESCRIPTION OF PROGRAM
Heartland Bridges houses two programs, Bridges Transitional Living and Bridges Crisis Care. Transitional Living is a short-term transitional housing program enriched with supportive services designed to assist people with a diagnosed mental health or co-occurring condition while they transition to stability in the community. Crisis Care is a residential stabilization service designed for individuals who need a safe, secure environment for three to five days while stabilizing from a crisis before returning to the community where they live.
SUMMARY DESCRIPTION OF WORK
The CaseManager III serves as a direct care staff that provides intensive casemanagement services to Bridges clients. This role facilitates supportive services designed to meet the needs of the client. Decision making, problem solving, and highly developed interpersonal skills are critical.
Compensation: between $19.74 and $23.54 per hour (wage is determined by total years of relevant experience)
Work Hours: 24 hours per week (Monday, Friday, and Saturday, 7:30 a.m. to 3:30 p.m.)
MINIMUM QUALIFICATIONS

MJE CaseManager: Geriatric Coordinator -- responsible for planning, coordinating, and monitoring care for older adults to ensure they receive comprehensive, person‐centered services that address physical health, mental health, social, and environmental needs. The goal of the role is to promote independence, enhance quality of life, reduce avoidable hospitalizations, and support aging in place when possible.
Elicit and document healthcare goals and treatment preferences for patients aged 65+, incorporating what matters most to them into the interdisciplinary care plan and discharge planning.

Essential Duties and Responsibilities
Provide essential support to clients through trauma-informed casemanagement and advocacy.

Collaborates with and engages in Multi-Disciplinary Teams to include (but not limited to) Pottawattamie County Sheriff's Department; Pottawattamie County Jail; Pottawattamie County Court Judges; various attorneys; Iowa Department of Corrections; County Probation; Medical offices; Clinicians; Practitioners.
Travel to and meet with clients and supports for accurate assessment and progress updates.
Attend community meetings and legal court proceedings, including providing court testimony when necessary.
Establishes a positive working relationship with and knowledge of therapists, medication providers, justice system personnel, community providers, schools, and other professionals assigned to each case.
Maintains confidentiality of clients and families.
Maintains required records as per contract, compile data spreadsheets.

This casemanager will serve as a direct care staff/casemanager in a residential substance abuse treatment facility that serves women and their children. This position provides supervision and support to residents as a part of the 24-hour direct care staff, as well as casemanagement services for assigned families. The casemanager will be part of the team that provides education and support programming for women, their children and family members.
Compensation: between $19.74 and $23.54 per hour (wage is determined by total years of relevant experience)
Work Schedule: between 15 and 20 hours per week, multiple shifts available including nights and weekends (four openings available)
MINIMUM QUALIFICATIONS
Bachelor's Degree in Human Services, Psychology, Social Work, Sociology or a related field preferred
Two or more years' experience in human services field
Alcohol/drug abuse experience preferred
Valid driver's license/ acceptable driving record
Bilingual-Spanish speaking abilities preferred
Essential Duties and Responsibilities
*Provides supervision and support for program participants as a part of residential program staffing for 24 hour per day, 7 day per week schedule.
*Assists staff in implementing casemanagement activities and goals with and for WCC clients.
*Participates in interdisciplinary family/case staffing to obtain information, provide information, and assist with service planning as needed.
*Completes intake information with incoming clients.
*Reviews casemanagement activities, goals and rules with Program Coordinator.
*Documents client activity, progress and needs per established record keeping procedures.
*Participates in interdisciplinary family/case staffing to obtain information, provide information, and assist with service planning.
*Implements casemanagement plans and provides support services for program participants under the direction of Lead CaseManager and Program Coordinator.
*Does follow-up with individual clients and updates goal planning as client goals change.
*Responsible for maintenance and upkeep of facility, including monitoring residents' activities.
*Orients clients to WCC rules and addresses client non-compliance as necessary to create a safe and secure environment for other residents.
Works as a team member with other program staff to provide trauma-informed care to clients.
*Assists in facilitating educational participant support groups and children groups as assigned.
Networks with community providers and other agency programs in order to assist clients in accessing needed community resources.
*Remains knowledgeable of support group services that provide support services for persons with substance abuse problems and their families.
Evaluates efficiency and effectiveness of community service providers on an ongoing basis to ensure clients are receiving quality care.
*Develops supportive relationship with clients.
Advocates for clients in the community.
Able to work independently with little supervision.
Maintains required records and statistical information.
Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
