Attends appropriate local, regional and national meetings on homeless issues to seek out new current information. Prepares and presents training and educational programs and publicity materials. Participates in team efforts to make La Red a welcoming environment for clients utilizing our services including persons participating in our homeless case management. Creates written statistical technical reports for review to the Behavioral Health Administrator Performs other duties and responsibilities as assigned.

RESPONSIBILITIES

• Must meet visitation and treatment team compliance goals for Community Hospital Coordinated Care as directed by the Team Lead and auspice of the Division of Health and Social Services. • Attends daily and weekly treatment team meetings as well as monthly residential house meetings if applicable.
• Responsible for proper completion of agency-required forms, assessments, and documentation in a timely manner, i.e., Progress Notes, Recovery Plans, Assessments, etc.
• Ensures that there is a process in place to trigger an immediate increase in service for a person served who experiences a crisis.
• Ensures that persons waiting for services at a higher level receive interim care that addresses their most acute concerns.
• Works in conjunction with the directors of other services internally and externally to ensure that all persons served receive the services they need and achieve a seamless transition from one service level to the next in accordance with their needs.
• Manages the benefits application process and coordination with appropriate State Service Centers, ASSIST, Social Security, LogistiCare, Division of Vocational Rehabilitation, Division of Disabilities Determination, etc.
• Acts as an ombudsman to other community and State service providers, ensuring that a process for suggestions and complaints is in place and that suggestions, complaints and grievances are responded to respectfully and in a timely manner and that there is a mechanism for referrals back and forth.
• Can work independently and responds to crisis on-call when assigned on a rotating basis.
• Works to facilitate transition of clients to and from the Delaware Psychiatric Center and other acute care inpatient units, and to and from outpatient, and lower care levels as appropriate.
Performs other duties as requested or assigned, verbally or in writing.
REQUIREMENTS
• Bachelor's degree in psychology, counseling, human services, social work, nursing, rehabilitation or related field from an accredited college or university.
• Three years of clinical/administrative experience in providing support to individuals with severe and persistent mental illness.
• Computer literacy in MS Office products; Word, Excel, Outlook, Power Point.
• Agency Electronic Medical Record and other software programs

Investigate reported incidents of child abuse, neglect, or dependency, determine whether the incident is substantiated, and develop recommendations to a Juvenile Court or County Director for disposition.
Assess the level of risk to the child for additional injury or harm, including whether the child is in imminent danger, and may remove the child from the family as the situation warrants, placing a child in a protected environment.
Perform needs assessments to determine treatment options for families and children reported to be abused/neglected.
Testify and prepare pre-dispositional and progress reports, maintain case files, and develop briefing reports for the community child protection team. Develops “informal adjustment” plans with families and children to divert children from the juvenile justice system.

JOB REQUIREMENTS for Social Service Specialist III
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:
Three years experience in health or human services support such as interviewing clients and assessing personal, health, social or financial needs in accordance with program requirements or coordinating with community resources to obtain client services.
Three years experience in applying laws, rules, regulations, standards, policies and procedures.
Six months experience in making recommendations as part of a clients service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
Six months experience in record keeping.
Six months experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data.
