We're seeking an exceptional Clinical Lead Care Manager (CLCM) to join our team. For Pre-Procedure Evaluations: Drafts the PPE questionnaire, obtains the reports, bills and
estimated cost or each procedure, obtains the defendant's policy limits, runs and ISO on the
client, and provides all information to the Lead Attorney for their review. For surgery evaluations runs a Criminal Background Check (CBC).
-Contacts client to over the ISO and/or CBC with client to confirm the information and/or obtain any pre-ret treatment regarding any previous accidents. Adds a note regarding the ISO/CBC review.
-CM will obtain medical provider full contact information, prior attorney information and provide this to their respective RC for cases where there are pre-existing accidents as they will request the records.
Consults and advises clients regarding loss wages claims and credit issues.
Resolves problems. Consults with Pre-Lit Manager regarding claims process and special, unusual, or complex circumstances.
As required and under the direction of an attorney, assists in the negotiation process for a
client's case (case by case)
Seeks opportunities for clients to submit favorable online reviews about their experience with the firm.

System Proficiency:

Organ Allocation Optimization:
Maximize organ allocation effectiveness by ensuring timely offers, conducting organ-specific testing, managing logistics for courier services, activating recovery surgeons, and providing clear communication with transplant centers.
Recovery coordination:
Coordinate and arrange transportation for recovery surgeons when necessary.
Dispatch and direct appropriate recovery personnel to organ recovery sites.

Verify and document all recovery activities, including time of death, anatomical details, and diagnostic testing results.
Oversee courier operations and confirm safe and timely delivery of recovered organs.
Local and National Organ Placement:
Coordinate organ allocation for local, regional, and national transplant centers, including Heart, Lungs, Liver, Kidneys, Pancreas, Small bowel, and other organs for transplant or research.
Organ Biopsy Coordination:
Facilitate organ biopsy processes and ensure timely reporting of biopsy results to the relevant Organ/Tissue/Research Centers.
Accurate Documentation:
Maintain accurate and real-time documentation in systems like Digital Donor, UNET, and other assigned platforms for all cases coordinated.
Ongoing Communication:
Maintain continuous communication with Procurement Staff, Family Care Center, Family Services, and Leadership/Administrator on Call (AOC)
Notify the Manager/AOC of any unusual circumstances during organ placement and recovery processes.
Adverse Lab Value Reporting:
Timely report adverse lab values, such as positive cultures, to the appropriate Organ/Tissue/Research Centers.
Post-Recovery Follow-up:
Conduct post-recovery follow-up, including feedback on UNET, potential transplant recipients (PTR), and organ disposition.
Organ Disposition:
Ensure that unsuitable organs are discarded appropriately in accordance with established guidelines.
Shift Handover and Communication:
Conduct thorough and accurate shift exchanges with oncoming staff to ensure smooth continuity in organ allocation and recovery.
Staff Training:
Assist in the training / precepting of new staff and provide ongoing education under the guidance of management.
Medical Terminology and Medical Knowledge Resource:
Utilize experience in the medical field and education to triage medical questions from transplant centers to appropriate medical director.
Review and interpret laboratory and radiologic donor findings during case review to provide quality review of donor report to transplant centers.
General Expectations

Summary:
Through the use of evidence-based practices such as harm reduction, housing first, and trauma-informed care, the CaseManager supports Clients' housing stability, self-sufficiency, and community integration. The CaseManager partners with Clients who are currently homeless to assess and address barriers to housing placement. Once Clients locate best-fit housing, The CaseManager works to implement interventions with recently housed Clients to support housing retention and further coordinates multiple services to meet individualized needs. The CaseManager collaborates with Clients to learn independent living skills and other life-skills vital to successful transition from homelessness into permanent housing.
Essential Duties and Responsibilities:
Maintain a caseload of approximately 20 high acuity Clients for intensive casemanagement services (up to 40 if low acuity).
Develop relationships with Clients with a focus on facilitating independence and maintenance of improved physical and mental health.
Collaborate with LA County Department of Mental Health (DMH) and Department of Health Services (DHS) programs to enhance coordination within Clients' treatment team.
Engage Clients and implement interventions with an approach emphasizing Client choice, harm-reduction, and psychosocial rehabilitation within a setting guided by trauma-informed care, housing-first principles, and professional boundaries.
Assist in the screening, intake, and enrollment of Clients including orientation to program policies and goals.
Complete comprehensive biopsychosocial assessments and functional needs assessments to guide individualized treatment planning.
Complete quarterly collaborative goal plans with Clients with the purpose of targeting areas that will improve quality of life and health outcomes.
Write and maintain accurate, complete, and up-to-date progress notes and data in electronic databases as required by the program and its funding sources.
Ensure all Client documents within physical charts are complete, accurate, current, and filed as guided by agency, department, and funder expectations.
Maintain confidentiality of Client information per HIPAA and internal agency guidelines.
Assist Clients in applying for and obtaining all benefits to which they are entitled.
Coordinate appointments, transportation, and follow-up services for Clients to improve access to primary health care, mental health care, substance use services, and other local resources in order to build sustainable community connections and reduce the need for emergency health care services.
Directly transport Clients in personal vehicle to various agencies and healthcare providers in the greater Los Angeles area to decrease barriers to access and increase community reintegration.
Support Clients by teaching and modeling life skills involved such as budgeting, housekeeping, meal preparation, appointment scheduling, health maintenance, and socialization within communities where they become housed.
Communicate effectively with property managers and other third party partners.
Work cooperatively and cohesively with multidisciplinary team including participation in weekly staff meetings, direct individual supervision, and trainings.
Adapt workload as directed by supervisor in response to funder expectations.

Qualifications:
High School Diploma or GED or Equivalent
At least one year of experience working with homeless individuals AND have a social work/mental health-related bachelor's degree OR have a minimum of two years of experience providing direct mental health or intensive casemanagement services, unless otherwise approved by the funder.
Demonstrated knowledge of issues faced by a culturally diverse population
Proficiency In Word, Excel and Outlook required
Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record.
Willing and able to transport Clients in your personal vehicle.
Preferred Qualifications:
