Required Qualifications
Required Education
Bachelor's Degree or 5 years of experience in the field of Long-Term Services and Supports (LTSS), which includes Developmental Disabilities or
A combination of education and relevant experience appropriate to the requirements of the position. Relevant experience is defined as: (a) Experience in one of the following areas: long-term care services and supports, gerontology, physical rehabilitation, disability services, children with special health care needs, behavioral science, special education, public health or non-profit administration, or health/medical services, including working directly with persons with physical, intellectual, or developmental disabilities, mental illness, or other vulnerable populations as appropriate to the position being filled; AND (b) Completed coursework and/or experience related to the type of administrative duties performed by case managers may qualify for up to two (2) years of required relevant experience.

Case managers collaborate with the larger team of case managers and clinicians and are part of the team's alternative response model. Alternative response involves responding to calls with other BHCRT staff that are lower in acuity than calls dispatched to clinicians and Boulder Police Department officers. The primary focus of the clinical case manager is supporting community members who are falling through system gaps, connecting them with resources, and preventing future crises.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Coordinates with Behavioral Health Crisis Response Team to connect with clients following community contact or referral from Public Safety personnel.
Provides short term case management to high-risk community members to support access to available resources, benefits, and care.
Provides in-person response, along with other BHCRT staff, to situations involving behavioral health concerns or crisis in a variety of community settings via Boulder Police & Fire dispatch, including homes, businesses, street corners, parks, congregate living facilities, and shelters.
Advocates for appropriate access to care and/or resources while supporting clients' strengths and resiliency.
Meets with clients and community members to determine case management needs and facilitate care coordination with appropriate providers. May involve intensive in-home or community-based contact.
Clinical case managers transport clients to medical appointments, food pantries, benefits offices, and other locations as needed.
Communicates, collaborates, and creates relationships with multiple collateral agencies, including resource providers, county services, schools, hospitals, family members, and significant others to facilitate referrals for needed community-based services and treatment.
Participates in clinical supervision and team meetings according to team policies.
Documents all contacts according to team policies.
MINIMUM QUALIFICATIONS

BHCRT operates seven (7) days per week including some evening hours. Is accessible to clients for supervision, guidance, and resources outside of scheduled meeting times.
Meet individually with each client at least weekly to evaluate progress in accomplishing case plan and sentencing requirements including all evidence based practices.
Assist client in developing budgeting, employment, treatment and other pro-social behavior plans and monitor adherence
Complete and maintain client files and records according to standards
Monitor drug and alcohol tests as required.
MINIMUM QUALIFICATIONS
EDUCATION and/or EXPERIENCE
EDUCATION: Graduation from an accredited college or university with a bachelor's degree in criminal justice, psychology, social work, counseling, a social science or other closely related field.
SUBSTITUTION FOR EDUCATION: Work experience in the areas of counseling, social work, law enforcement, corrections or community supervision which provided the same kind, amount, and level of knowledge acquired in the required education, will substitute on a year-for-year basis for the bachelor's degree.
SKILLS
Demonstrated proficiency in general population case management
Demonstrated experience in applying Motivational Interviewing skills and techniques
Ability to communicate effectively both verbally and in writing with a variety of constituents
Ability to complete assessments and develop case plans
Ability to enter data, generate reports and create communications from variety of software programs and databases
Ability to remain calm in stressful situations
Knowledge of the cultural and socioeconomic characteristics and appropriate techniques and practices for the client population.
Ability to interact effectively as a team member and independently with Company staff, stakeholders, and with diverse client base.
Must be granted access to state criminal databases

What we need for this job:
* Masters in Psychology, Social Work, Counseling or related field, plus 1+ year internship, and one (1) or more years experience in a clinical setting
* Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) or full licensed acquired within twenty four (24) months (Licensure Preferred).
