Located in our new Crisis Recovery Center on the DuPage County Campus in Wheaton, Crisis Services provides 24/7 crisis mental health services to county residents experiencing psychiatric crises or needing support for mental illness symptoms. In this on-call position, you will be required to work at least three (3) shifts per month, having evening and weekend availability, working no more than 1000 hours per year. We offer a $2.00 per hour shift differential for our second shift and $2.50 for our overnight shift.
Responsibilities
Supports maintenance of a safe, therapeutic environment for clients seeking services through Crisis Recovery Center programs on the Substance Use Stabilization Unit, Adult Mental Health Unit, and Youth Mental Health Unit, as well as in Crisis Residential.
Facilitates client safety and quality care in Crisis Residential and Crisis Recovery Center Programs, through completion of client rounding and belongings checks
Assists with client medication observation in Crisis programming as needed
Performs tasks related to maintaining environmental standards in crisis programming
Assist in de-escalation of clients experiencing a mental health crisis through engagement with client and coordination with team members providing clinical intervention.
Completion of required documentation within established timeframes, through use of an Electronic Client Record
Participates in team meetings to assess clients' progress.
Participates in emergency response activities as assigned.
Requirements
A qualified candidate must possess a valid State of Illinois Driver's license and have a good driving record. New employees are required to attend paid on-site, HR New Hire Orientation at our Central Public Health Center in Wheaton, during our day shift generally, from 8:00 am to 4:30 pm, for one week. Must have flexibility to work on the day shift for two weeks of additional training on site at the CRC.

Provides Medical Case Management to individuals through in person and telephonic communications with the patient, physician, other health care providers, employer and others.
Utilizes their medical and nursing knowledge to discuss the current treatment plan with the physician and discuss alternate treatment plans.

KNOWLEDGE & SKILLS:

Experience as a RN, Medical Case Manager is ideal, or a clinical background in orthopedics, neurology, or rehabilitation is preferred.
Ability to meet with the patient, their physicians, other healthcare providers, attorneys, and advisors/clients and coworkers.
A cost containment background, such as utilization review or managed care is helpful.
Strong interpersonal, time management and organizational skills.

As a Medical Case Manager you will make a meaningful difference in the lives of injured workers and their families. Your responsibilities include working closely with injured workers to facilitate their recovery. You will work collaboratively with the patient, their family, medical providers, members of our team, and others. This is a heavy local travel role responsible for working with a caseload of workers compensation injured workers within a defined jurisdiction.
ESSENTIAL FUNCTIONS & RESPONSIBILITIES:
Provides in-person and telephonic Medical Case Management to individuals, involving the patient, physician, other health care providers, the employer, and the referral source
Utilizes their medical and nursing knowledge to discuss the current treatment plan with the physician and discuss alternate treatment plans
Provides assessment, planning, implementation, and evaluation of patient's progress
Evaluates patient's treatment plan for appropriateness, medical necessity, and cost effectiveness
Attends doctors, other providers, home and in some cases, attorney's visits
Attends hospital and/or long-term facility discharge planning conferences, etc. for the purpose of determining appropriateness of care and developing an effective long-term care strategy
Conducts home visit for initial evaluation
Implements care such as negotiating the delivery of durable medical equipment and nursing services
This role requires regular travel, dependent on the injured worker's injuries and needs. The employee must be available for local travel up to approximately 60% of the work week/month
This role may require overnight travel

KNOWLEDGE & SKILLS:
Effective communication and multi-tasking skills in a high-volume, fast-paced, team-oriented environment
Ability to meet with the patient, their physicians, other healthcare providers, attorneys, advisors/clients, and coworkers
A cost containment background, such as utilization review or managed care is helpful
Strong interpersonal, time management, and organizational skills

Ability to work both independently and within a team environment
EDUCATION & EXPERIENCE:
Experience as an RN Medical Case Manager is ideal, or a clinical background in orthopedics, neurology, or rehabilitation is preferred
Graduate of accredited school of nursing
Current RN Licensure in state of operation
Certification as a CCM, CIRS, or other Case Management certifications preferred
A valid driver's license, reliable transportation, and ability to travel to assigned locations is required
PAY RANGE:
CorVel uses a market based approach to pay and our salary ranges may vary depending on your location. Pay rates are established taking into account the following factors: federal, state, and local minimum wage requirements, the geographic location differential, job-related skills, experience, qualifications, internal employee equity, and market conditions. Our ranges may be modified at any time.
For leveled roles (I, II, III, Senior, Lead, etc.) new hires may be slotted into a different level, either up or down, based on assessment during interview process taking into consideration experience, qualifications, and overall fit for the role. The level may impact the salary range and these adjustments would be clarified during the offer process.

TITLE Medical Case Manager The Medical Case Manager assists individuals living with HIV in accessing and adhering to health care services linking them with core clinical, psychosocial and other related services. MINIMUM QUALIFICATIONS
Bachelors Degree in social services, public health, health administration, human services field or an Associates degree with appropriate certifications (i.e., CADC, LPN) with at least three (3) years experience in social services (case management experience preferred). Must be knowledgeable about HIV/AIDS. Experience serving homeless, ex-offenders, individuals living with HIV/AIDS and/or substance abuse population is preferred. Must have computer literacy and data entry skills. Strong organization and time management skills required as well as the ability to work independently. An automobile, valid Drivers License and vehicle insurance coverage is required. Occasional late or weekend hours may be required.
RESPONSIBILITIES
* Manage an assigned caseload of client with HIV/AIDS requiring comprehensive Ryan White Medical Case Management Services.
* Conducts initial intake and ongoing assessments, medication and treatment adherence assessments, treatment/service plans and medical assessments
* Monitors progress toward improving/stabilizing health status
* Development of comprehensive care plans with clients that integrates medication and treatment adherence into services
* Work in conjunction with clinical providers (e.g. case conferencing, huddles); ensuring all clients are case conference at least twice per year.
* Provides and tracks referrals and linkages to services with special emphasis on core clinical services, access to medical, income benefit programs and access to emergency financial assistance.
* Monitors treatment and medication adherence for all core clinical services
* Maintains client file, records all activity, and maintains appropriate documentation of eligibility
* Inputs all data into the client level database.
* Completes the Case Management Competencies training with a passing grade as determined by the AIDS Foundation of Chicago training policy
* Attends at least 12 trainings during the contract year to enhance skills and HIV knowledge; including at least four clinical training
* Submits monthly/quarterly/annually reports on program/client activities in a timely fashion.
* Attends monthly team meetings and all other relevant program meetings as scheduled.
* Conducts presentations and/or educational workshops on behalf of CCHC.
* Conducts peer chart reviews to ensure continued quality of services.
* Completes at least one telephone contact with each client monthly.
* Meets with clients at other agency clinic locations as necessary.

Responsibilities
The Crisis Intervention Specialist will perform duties including, but not limited to:
Perform all work in a manner consistent with the National Youth Advocate Program's mission, values, and philosophies.
Provide live crisis support for those at imminent risk of suicide, those in need of immediate resources; or those in need of emotional support.
Provide crisis intervention, emotional support, or transportation services for parents, youth, and adults.
Establish rapport with crisis hotline callers in a non-judgmental and calm voice.
Assess risk through assessments including the “Ask Suicide-Screening Questionnaire.”
Contact first responders if the caller is at imminent risk of suicide.
Schedule appointments for intake and outpatient follow-up care.
Provide transportation when needed.
Administer groups and training for support staff.
Completes all follow-up care and home visits within the required time limits.
Reassess safety and provide referrals for behavioral health and other needed services.
Performs other duties as requested.
Minimum Qualifications
Bachelor's degree in social work, Counseling, Psychology, or related field from a CSWE accredited college or university.
Previous experience in crisis response, substance abuse, juvenile justice, advocacy, mental health, or equivalent.
An awareness or acceptance of the NASW Code of Ethics and is an effective and efficient communicator.
