Join our team as a Behavioral Health Consultant in Woodburn, OR. The Behavioral Health Consultant provides primary care-based behavioral health services to clients with complex or chronic needs depending on the assigned program as determined in the treatment or care plan of the client. Counseling or therapeutic services are provided as part of a team, with primary care-based services being integrated into the care plan directed by the primary care provider. We offer this position at either PsyD/PhD clinical psychologist, or independently licensed Master's-level mental health therapist.
This is a part-time 20 hours/week position that includes our full benefits package.

As a Behavioral Health Consultant you have the opportunity to :
Responds to requests from primary care providers to address behavioral health needs of medical clinic patients to maintain a warm hand-off for most patient encounters.
Provides brief intervention for presenting problems and triage for ongoing treatment services as needed. Completes encounters and associated documentation, including coding and billing.
Presents case studies and reports to funding sources or as requested by outside organizations.
Measures change and adjusts treatment as needed, using appropriate screening tools.
Provides cognitive behavioral therapy to individuals, families, groups, or communities as assigned. Integrates cognitive behavioral interventions in multiple settings.
Enters documentation into the electronic medical record (EMR).
Responds to crisis situations, assessing urgency of patient's needs, provides care, and obtains appropriate services as needed.
Assists patient and family in obtaining additional services and other resources as needed. Documents all referrals made to other resources.
As assigned, participates in various community meetings and serves as a liaison to numerous community agencies and organizations, including school districts, hospitals, law enforcement, Division of Children and Family Services (DCFS) staff, and tribal authorities.
Presents mental health/behavioral health findings and recommendations in verbal or written format to appropriate professionals either upon request or as part of a regular case presentation for peer review or clinical meeting.
Leads staff training sessions on new therapeutic techniques and practices. Provides training and education on clinical topics to team members, staff, and the community, as needed.
Performs other duties as assigned.
Performs clinical responsibilities in alignment with The Joint Commission (TJC), Health Resources and Services Administration (HRSA), and YVFWC requirements.
Qualifications
Clinical Psychologist Requirements
Education: Doctor of Philosophy (Ph.D.) or Doctor of Psychology (Psy.D.) in Clinical Psychology.
Experience Preferred: One year's clinical experience working within a multidisciplinary team in primary care or other medical settings providing diagnostic and psychological assessments, for primary care-based positions. One year's clinical experience working with children, adolescents and families for specialty behavioral health settings.
Licenses/Registration: Licensed Psychologist within the state of practice. First Aid and Cardiopulmonary Resuscitation (CPR) Certification within 45 days of hire.
Master's Level Therapist Requirements

The Foundational Community Supports (FCS) program provides targeted Medicaid benefits to assist eligible individuals with complex health needs to obtain and maintain stable housing, which will ultimately address the interrelation of health problems and housing stability. To achieve this work, the Housing Specialist will have two primary scopes of work: 1, home and community-based supports to eligible individuals- which includes coaching, advocacy, information and referral, linking and coordinating, and ongoing supports; and 2, identifying housing opportunities in the community by doing outreach to private market, and scattered-site landlords. AM Counselors are generalists who are flexible in their job duties serving a population that has a variety of needs including treatment for substance use disorders, co-occurring mental health disorders, and support and education for their family members. AM Counselors function within a multidisciplinary treatment team to support members that present in all phases of readiness for change.
Essential Responsibilities:
Provide high quality clinical services (individual, group, and family) to our adult and adolescent members through screening, assessment, diagnosis, and formulation/implementation of service plan in outpatient or residential settings. Provide all care in accordance with accepted best practice standards and department protocols (including supervision). Provide case management through appropriate triaging and referrals, consultation with other providers, contact with members and their families, and coordinating with outside agencies. Coordinate care for patients receiving medications from the Addiction Medicine Medical Team.
Document all member care in the electronic medical record in accordance with accepted medical/legal standards.
Provide other member or department services, such as program development, in-service training, and participation in process improvement activities.
Basic Qualifications:
Experience
Minimum one (1) year working in a setting that is specific to substance use disorder treatment within the last five (5) years OR completion of KP Addiction Medicine department internship before hire/transfer.
Education
Masters level degree in counseling, social work, or closely related field (M.S.W., M.S., M.A., M.Ed.) required at time of hire/transfer.
License, Certification, Registration

Strengthen and improve Case Management Services: Establish strong relationships with clinicians and other health care team members. Communicate data on population case managed, utilization and outcomes. Education of staff/clinicians. Development and distribution of education, tools and materials for member coordination. Clackamas County Health Centers is seeking a Bilingual (Spanish) Case Manager to provide skills training, case management, care coordination, and medication education for individuals with mental health conditions or co-occurring mental health and substance use disorders. The Case Manager works with families and community partners to provide education and coordinates access to resources, benefits, and support.
This position will work within the Diabetes Integration Program (DIP) providing services and support to adults diagnosed with diabetes or pre-diabetes. DIP has provided integrated physical, dental, and behavioral health care for Clackamas County Health Centers' patients with diagnoses of diabetes and severe mental health. This bilingual DIP case manager position will expand services to under-served Spanish speaking communities who experience diabetes at greater rates than majority communities.
Specific case management focuses may include, but are not limited to, providing group, individual, and family services identified in the primary care and/or mental health treatment plan; offering skills training, case management, and care coordination; supporting individuals diagnosed with Severe and Persistent Mental Illness (SPMI) and diabetes; and conducting outreach as indicated in the client's care plan. The Case Manager must be skilled in engaging with a diverse client base while maintaining professionalism in a high-need environment.
Successful candidates will provide culturally relevant services, have an anti-racist lens, engage in personal reflection and growth in their equity journey, demonstrate a strong commitment to equitable services, and show sensitivity to and respect for the diverse populations served. WORK SCHEDULE
This position works 40 hours during a standard workweek of Monday through Friday.

If selected as a final candidate, you will be required to submit all State Professional Licenses/Certificates, DEA, National Board Certifications, Educational Commission for Foreign Medical Graduates (ECFMG), BLS and/or ACLS level CPR, and degrees (as applicable). Credentialing and Provider Enrollment processes will be completed prior to hire with the help of our Health Centers Credentialing team.
This position will require possession of a current certification as a Qualified Medical Health Associate (QMHA) by having the minimum education requirements as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO) and/or State of Oregon.
