The CaseManager shall assume the following job responsibilities:
Conduct intake and provide an orientation for new arrival.
Complete IPP (Individualized Program Plan) within 10 days of resident arrival.
Prepare Individual Re-Entry Plan for all assigned residents with their assigned US Probation Officers.
Coordinate reentry programs to ensure continuity of care for the resident and facilitate services for residents with special needs. This coordination will include consultation with the SCTC (Supervisory Community Treatment Coordinator) for treatment that is provided under CTS (Community Treatment Services) contracts.

Document approved prescribed medications.
Network clients with community services.
Attend and provide input at PRT meetings and mandatory monthly staff meetings.
Process incident reports.
