LSS Recovery Center Oconomowoc provides up to 30 beds for adult women in need of Medically Monitored Treatment and Transitional Residential Treatment. Services are gender specific and trauma informed. The facility does allow for infants up to age 3 months to reside with their mothers who are receiving SUD treatment services.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Role Description
As Case Manager, you will report to the Director of Utilization Review. This position is an integral part of the treatment team and functions in coordinating orientations, treatment planning and discharge planning with patients. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned:
Maintain an active caseload of up to 5-10 clients.
Enter client information into Orbis Youth Assessment Screening Instrument (YASI) system within 72 hours of intake
Collaborate and network with law enforcement, probation, employers, family members, schools, health providers and any other providers involved in the client's service planning.
Create and maintain Individual Case Plan for all youth involved in program.
Execute routes in either an agency vehicle or one's personal vehicle. This may include preparing the agency vehicles for daily use (I.e. filling up gas.)
Serve as group facilitator/co-facilitator for evidence-based curriculums.
Address individual youth and group behaviors using motivational interviewing and de-escalation skills.
Provide information to the Program Coordinator and Juvenile Justice clinician relevant to any unaddressed, perceived client needs as part of on-going assessment of referred youth.
Ensure safety and well-being of clients during all programming hours.
Assist in driving and maintaining vans, transporting clients to and from program sites, transporting food to program sites.
Complete all required paperwork in a timely and accurate manner.
Complete incident reports regarding client behaviors and other program related incidents before 9 am the following morning.
Ensure programming is implemented within the designated hours.
Prepare and submit all paperwork accurately and within the prescribed timeline.
Maintain Food Handlers Certificate for basic food handling.
Complete all Core Curriculum training(s) required for this position. Complete on-going training as designated by program requirements.
WORK HOURS
Willing to work varying hours based on the needs of the program. Hours may include evenings and weekends at any of the AMHW Reporting Centers.
CARE COORDINATION
Individuals in this position may have a leadership or shared role in coordinating care for clients within the program and a specific geographic area.

Major Responsibilities:
Assuring and Improving Quality of Care: Facilitates the patient and family's right to receive quality cost-effective care following Advocate's MVP of Stewardship.

Adapts practice to the latest standards according to evidence based literature. Utilizes appropriate tests and measures, and interventions to achieve outcomes.

Recognizes changes in clinical situations. Evaluates and implements changes to the plan of care.
Prioritizes and organizes patient care, ensures clear hand-off communication to achieve optimal health outcomes for patients, utilizing excellent communication techniques like SBAR and 5P's.
Initiates referrals and discharge planning for assigned patients and actively coordinates care utilizing the interdisciplinary approach. (As appropriate based on employment status).
Actively participates in interdisciplinary case conferences. (As appropriate based on employment status).

Licensure, Registration, and/or Certification Required:
Occupational Therapist. Speech-Language Pathology license issued by the state of Illinois
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required
Education/Experience Required:
Master's Degree in Occupational Therapy. Licensed in the State of Illinois (or eligible for licensure) Able to demonstrate compliance with state continuing education requirements. 1 year of clinical experience or demonstration of strong spectrum of clinical affiliation experience. Has graduated from an Occupational Therapy curriculum accredited by the American Occupational Therapy Association.
Knowledge, Skills & Abilities Required:
Basic knowledge of computer use (i.e. Microsoft Word, email access and use) Ability to learn and use computer based scheduling and documentation system. Ability to communicate professionally both verbally and through written reports, with strong documentation skills. Good interpersonal skills. Good time management and self organization skills. Proficient use of all home based therapy equipment and assistive/adaptive devices (balls, bands, weights, dressing aids, etc)
Licensed as Occupational Therapist in the State of Illinois
Physical Requirements and Working Conditions:
Extensive travel within geographical boundaries in all extremes of weather and on all types of roads including expressways. Must be able to provide/arrange own transportation to and from patients homes and branch office. Requires adherence to scheduled days and hours of work based on commitment to patient/client and work load assigned. Work environment includes patient or family homes, or assisted living facilities. Requires full range of body motion including: a. Bending, sitting and standing b. Climbing stairs while carrying objects c. Pulling, pushing, reaching, carrying and lifting; (must be able to lift/carry at minimum 25 pounds) d. Assisting and maintaining patient safety with all ADL activities, and transferring patients using standard pivot transfers or with use of transfer board or Hoyer lift e. Providing treatment on standard beds in the home environment
This requirement supersedes any previous lifting requirement effective 08/01/2014. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
COMPENSATION: $74,672.00-$112,112.00 annual base salary

JOB SUMMARY:
As a Field Medical Case Manager at ForzaCare, you'll help injured workers navigate their recovery and return to work. You'll act as the central point of coordination, connecting the injured worker, medical providers, employers, and insurance carriers to ensure timely, transparent, and effective care management.
This role is ideal for licensed nurses or certified rehabilitation counselors. While prior experience in workers' compensation is strongly preferred, those who have it will find their background especially valuable in this role.
KEY RESPONSIBILITIES:
Coordinate care between medical providers, employers, insurance carriers, and injured workers.
Attend appointments with the injured workers, which may include visiting employers and injured workers at their place of employment.
Develop, document, and monitor individualized recovery goals and return-to-work plans.
Provide consistent communication and detailed progress reports to clients and stakeholders.
Ensure all case management work meets or exceeds customer and compliance requirements.
Build and maintain strong relationships with clients, providers, and internal team members.
QUALIFICATIONS:
Education & Licensure:
Active Registered Nurse (RN) or Certified Rehabilitation Counselor (CRC) license with associated college degree is required.
Additional certifications such as CCM, CIRS, or other case management credentials are preferred.
Must comply with all state-specific licensure and certification requirements.
Prior experience in workers' compensation case management is strongly preferred.
Valid driver's license, reliable transportation, and auto insurance with ability to travel to appointments.
