Manages the day-to-day healthcare operations of the community to ensure resident's healthcare needs are met. Ensures residents are treated with respect and dignity and ensures quality care as resident's healthcare needs change. Supervises licensed nurses and other direct care staff within the community. Provides training, supervision, and monitoring of associates in the administration of medications as described/allowed in Nurse Practice Act, to include auditing of medication administration records.
Provides training and education to resident care associates on an ongoing basis with classroom in-services, and situation-specific training.
Supervises the maintenance of resident charts and reviews documentation performed by care giving staff.

Performs ongoing assessment/observation of residents' physical and psycho-social needs and coordinates with other departments to assure quality, proactive care.
Evaluates residents; documents changes in condition, and notifies executive director, physician, and resident's legally responsible party/family of resident's condition and reactions. Prepares Physician Visit Form and reviews and updates resident chart.
Facilitates continuity of care for those residents receiving home health care, hospice services, and other third party healthcare- related services.
Participates in or leads meetings relevant to resident care issues, such as Service Plan meetings, involving appropriate parties as needed.
Ensures in-house ancillary medical services such as podiatrist, doctor visits, dental visits, psych visits, lab, X-ray, ambulance, etc. are scheduled and followed through.
Participates in department quality improvement activities.
Assists in keeping the environment safe for associates to reduce the occurrence of Workers' Compensation claims by appropriately evaluating resident transfer and mobility needs and involving therapy services as indicated.

**Job Description:**
Provides grief counseling, disease adjustment support, crisis intervention, goals of care planning support, and de-escalation services for patients as appropriate. Assesses patients' and families' wholistically for discharge planning needs in the inpatient, observation and/or emergency departments, including prior functioning, support systems, financial, and psychosocial in a timely fashion to avoid delays in discharge planning. Reviews the medical record, including medications, history and physical, labs, and progress notes and incorporates the clinical, social, and financial factors into the transition of care plan. Develops discharge plans with appropriate contingency plans throughout the hospital stay to ensure timely care coordination and progression of care, making arrangements for post-acute care services and facilities as well as community care for social needs. Leverages technology and follows standard work and best practices to communicate with post-acute care services and facilities to ensure patient care information is communicated for continuity of care, medical records are complete, and discharge reconciliation is accurate. Actively participates in multi-disciplinary rounds to review changes in patient status, progression and level of care, and discharge plans for all assigned patients to identify resources necessary at discharge and ensure a timely transition, escalating care delays to leadership as appropriate. Communicates with and educates patients and families regarding emotional, social, and financial impacts of illness and mobilizes family/community resources to meet identified needs while advocating for patient and family empowerment in making health care decisions and accessing needed services. Organizes and facilitates patient and family care conferences with the multidisciplinary team. Documents discharge planning evaluation, ongoing assessment, discharge plans, MDRs, barriers to progression of care, avoidable days, and patient and family needs according to standard work. Other duties as assigned. Assesses readmitted patients for the patient's and family's perceived reasons for the readmission.

RESPONSIBILITIES
Serve as lead sports educator and counselor for Cherryvale Day Camps.
Manage and maintain all sports equipment, including inventory before and after the season.
Create and implement age appropriate sports activities for our campers.
Enthusiastically participates in all camp programs and activities.
Interface with the leadership team (Assistant Camp Director, Program Director, Camp Director) as needed.
Assisting in camp groups when staff support is needed
Maintain a safe and healthy environment for campers, abiding by all camp rules and policies. Assist any camper with empathy and patience when needed.

Position Summary:
This full time, Medical CaseManager Position is responsible for providing individualized, comprehensive casemanagement and advocacy services to people living with HIV/AIDS in the northern region of Colorado. Essential Job Functions:
Provides intensive CaseManagement, based on levels of acuity, to a caseload of assigned clients.
Process emergency financial requests
Maintains ongoing, proactive contact to assess client's physical and psychosocial status and needs through personal visits, telephone and written contacts.
Conducts comprehensive intake and needs assessments to identify client needs, including but not limited to:
Access to healthcare and the client's ability to self-manage and actively participate in healthcare
Adherence medication and medical treatment
Short term and long term housing needs
Income stability including employment needs and eligibility for public benefits (SSI, SSDI, Medicaid, Medicare, etc.)
Mental health and substance abuse issues using program specified assessment tools.
