Responsibilities:
As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

Utilizes information gathered through investigative interviews with shelter staff, clients, and prospective sponsors, home visit observations and the review of informational documents, including background investigations for all adults living in the potential sponsor's home for the purpose of making determinations and recommendations.
Conduct a collaborative psycho-educational process with the youth and sponsor designed to identify areas where support, resources or information are needed to ensure successful reunification.

Post Release Services

Perform ongoing case management with a focus on screening and making appropriate referrals to address the youth's needs in the following domains: safety, placement stability, guardianship, education, health and medical, individual mental health and family stabilization, legal/immigration proceedings, substance abuse, gang prevention, independent living, juvenile/criminal justice, trafficking assistance (as applicable) and child enrichment activities.
Facilitate the development of a youth and family care plan ensuring that the plan demonstrates youth and family voice and includes clear needs statements, strength-based strategies, and outcomes that are measurable and specific.
Collaborate with youth and sponsor, as well as available informal supports, to develop a crisis and safety plan to ensure the safety of the youth, family, and community.

Job Summary
We are seeking a part time Adult Case Manager to join our team. In this role, you will work collaboratively with adults with intellectual/developmental disabilities and their families to determine their needs, develop the best course of action, and monitor progress to ensure the delivery of high-quality, individualized support. The ideal candidate is compassionate, patient, and knowledgeable about behavioral healthcare and developmental disabilities.
Responsibilities
Conduct in person, virtual and phone contacts for referrals, meetings and troubleshooting
Collaborate with individuals, families and mentors and facilitate communications
Coordinate scheduling
Develop individualized care plans
Work with mentors to monitor progress toward goals
15 to 20 hours per week - primarily mid-afternoon/early evening hours
Qualifications
Previous experience as a Case Manager or similar position preferred
Experience with intellectual/developmental disabilities preferred
Strong problem-solving and organizational skills
Ability to manage multiple projects or tasks and prioritize appropriately
Ability to work in fast-paced situations and make sound decisions quickly
Excellent interpersonal skills and high level of compassion
Strong verbal and written communication skills
