Required to work 4 hospitals in the Southside or Western Tidewater regions. Location and unit to be assigned based on greatest need.There are three region options.Must be within 75 miles and 90 minutes of all four hospitals in assigned region.
• Operational hours = 8:00 AM - 4:30 PM 7 days per week
• Flexi - Hours: The team member must enter at least 48 hours of prescheduled time per calendarmonth.
• Facility supported: Members of the team are required to work assigned shifts in all units within the casemanagement specialty. Required to work in 4 hospitals in the Southside or Western Tidewater regions.
• Operational hours = 8:00 AM - 4:30 PM 7 days per week
• Flexi - Hours: The team member must self-schedule at least 48 hours of prescheduled time per calendar month in API. Of these prescheduled hours, 16 hours are to include availability to be scheduled on a weekend.

• Peninsula Locations only (Obici, Careplex, Williamsburg Regional, Albemarle Medical Center)

Education:
*Master's degree in Social Work
Certification/Licensure:
*Certification in CaseManagement (CCM, CMAC or ACM-SW) required within one year of eligibility.
*BLS required within 90 days of hire.

As an Inpatient Care Manager, responsible for the provision of medical social work services to patients receiving care in the hospital setting. Assesses the social, psychological, cultural, environmental, and financial situations, as well as disposition needs of each referred patient. Collaborates with patients, families, healthcare team members, and community organizations, and uses strong advocacy skills to identify needs and resources in the hospital and community.
Develop and evaluate effective transition plans taking into consideration the client's diagnosis, prognosis course of treatment, past and present services, short-term and long-term goals, provider options, and/or available health care benefits as appropriate to the continuum of care. Position Responsibilities:
Provide ongoing casemanagement services to assigned clients.
Comprehensive documentation of interventions, services, and behaviors that each client exhibits in relation to the Individualized Service Plan (ISP).
Facilitate psychosocial and psycho-educational interventions to individual clients
Establish referral networks with other agencies to ensure comprehensive care.
Attend weekly support team meetings with management.
Coordinate treatment teams and complete all discharge, planning, and monthly and annual reports for full client caseload.
Assist in the completion of Activities of Daily Living (ADL)/Independent Activities of Daily Living (IADL) personal care for clients
Advocate for clients on caseload; recommend appropriate services or level of care as needed.
Transport clients to community activities after successful completion of vehicle training.
Be willing to work with a volatile community, occasionally dealing with physical confrontations, bodily functions, etc.

The Behavioral Health Professional will work as part of a multidisciplinary team with all other disciplines on the Behavioral Health Unit to provide a proactive and collaborative approach in serving the patient population in a correctional environment.
MENTAL HEALTH PROFESSIONAL MINIMUM REQUIREMENTS
Must be a Licensed, Master's Level Behavioral Health Clinician who can administer assessments, make clinical decisions, provide Mental Health treatment, and hold any of the following licenses: CMHC, LCPC, LPC, LCSW, LMFT, ACMHC, ALMFT, CSW or equivalent. The Behavioral Health Professional co-leads group therapy, one on one therapy, and crisis therapy.
MENTAL HEALTH PROFESSIONAL ESSENTIAL FUNCTIONS
Professionals will provide crisis stabilization, complete intakes and on-going evaluations, and provide consultation to in-house and community services.
Participate in monthly behavioral health calls, work collaborative with the medical team to best determine overall treatment for each individual.
The Behavioral Health Professional performs risk assessment of inmates; coordination of care with interdisciplinary team including medical staff, community providers, psychiatric hospitals and corrections; and discharge planning.
The Behavioral Health Professional assists in planning and implementing the goals and objectives of programs and projects.
The Behavioral Health Professional provides the necessary preparation of documentation, necessary records and reports.
The Behavioral Health Professional utilizes clinical skills such as prioritizing treatment needs; crisis management; brief clinical interventions including CBT and Motivational Interviewing.
Excellent and timely documentation skills required.
The ideal candidate is able to make independent clinical decisions, while utilizing assistance and supervisory intervention, as needed.
MENTAL HEALTH PROFESSIONAL REQUIRED EDUCATION, CERTIFICATIONS, EXPERIENCE
Graduation from a Master's level program in a behavioral health related field.
Minimum of one year employment in a social work capacity in a public or private agency or equivalent.
Must receive a satisfactory background investigation report.

The EHS Manager will be a key player supporting the implementation and administration of the ITAC EHS Program. The EHS Manager will fully support our commitment to safety and must exemplify our Core Values.
