JOB SUMMARY:
Performs professional nursing care for patients in a primary care setting (the home or alternate-site setting) according to nursing practices, agency policies and regulations. Provide nursing services to patients and families in accordance with the scope of the RN as defined by the North Carolina Board of Nursing.
MAJOR JOB RESPONSIBILITIES:
Plan, provide, supervise and document professional nursing care utilizing the nursing process for patients in accordance with the NC nurse practice act, physician orders and established policies and procedures. Uses professional nursing judgment to individualize the plan of care based on assessment of the patient's baseline needs and response to care.
Utilize comprehensive assessment skills to plan priorities and to set realistic outcomes; collaborate with healthcare team in the evaluation and cost effectiveness of patient care.
Assess educational needs of patients/families concerning alterations in health, the disease process, and plan of care.
Develop and implement an effective discharge plan for patients to begin at the initiation of services.
Obtain or develop appropriate educational resources for patients/families; plan and implement appropriate educational interventions; evaluate effectiveness and outcomes of patient/family teaching.
Role model professional nursing through conduct, appearance, communication, mutual respect, ethical decision-making, critical thinking, and problem-solving skills.
Participate in weekly interdisciplinary team meetings and case conferences, as necessary, to assure appropriate care and service are provided to the patient.
Maintain up to date knowledge of Medicare/Medicaid/JCAHO standards thru inservices and workshop attendance.
Assesses the severity of patient symptoms, communicate to the physician and record significant findings, intervenes as appropriate.
Document care and services provided in accordance with DHCH Policy and Procedures and regulatory standards.
Complete documentation accurately, timely, concisely.
Medication reconciliation and education to prevent adverse events and readmission to hospital
Observe patient's reactions to medications and reports significant incidents to the physicians.
Communicate regularly with team members, caregivers, and physicians.
Follow infection control standard precautions and uses personal protective equipment as required.
Delegate and supervise care given by NCA's/LPN's.
Develop plan of care for aides, assign care to aides based on the skills of the aide, the availability of the aide for patient care continuity, patient preference (when possible), and other considerations as determined by the patient's care needs.
Identify personal learning needs and implements corrective action: assists in planning and conducting staff development programs to improve DHCC effectiveness.
Participate in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate assistance or educational offerings.
Implement agency policy, procedures and objectives.
Work closely and collaboratively with agency billing staff to ensure accuracy of billing.
Maintain confidentiality of employee and patient information.
Exhibit an attitude which promotes harmony and goodwill in the workplace.
Display a clean, neat, professional appearance.
Participate in Performance Improvement Program.
Promote quality, comprehensive services through a team approach.

Job Summary:
New Season operates a number of nationally recognized and accredited methadone treatment centers dedicated to helping individuals with prescription drug abuse and/or opiate addiction regain control of their lives.

Essential Functions:
Works with patients to complete all intakes, admissions, discharges, and transfer paperwork.
Documents patient progress through counseling and interaction through groups.

Performs individual, group, and family counseling as required.

Criminal Justice Services (CJS) is responsible for promoting improvements in the criminal justice system through interagency cooperation, coordination, and planning. It was formed in 2010 by the Mecklenburg County Manager's Office in response to findings from a Justice & Public Safety Task Force.
Position Summary
Provide professional level casemanagement to ensure compliance with court orders and enhance public safety in area of responsibility. This position, under general supervision, may be responsible for Recovery Courts (RC) work, providing offenders with the skills necessary to function in the community and not become repeat offenders, through participation in case planning and the creation and implementation of a unified case/service plan. May be responsible for pretrial work with accused defendants awaiting case disposition, providing direction about where to receive needed services from community resources. May also be responsible for re-entry services work with successful reintegration of residents returning to Mecklenburg County following incarceration at local jails or state/federal prisons. Duties include linking and arranging of services and advocacy.
This position will be located at the Mecklenburg County Court House, 832 E 4th St., Charlotte, NC 28202.
Essential Functions
Be able to provide some casemanager support in the community
Meet with social service/criminal justice involved individuals on a regular basis to monitor, case plan, collect urine analysis, crisis management, complete risk needs assessments and SA/MH assessments.
Interviews and screens individuals referred for participating in RC; reviews mental health and/or substance abuse history; conducts case consultations with applicable individuals, coordinates discharge planning with team partners
Evaluates individual's immediate circumstances, including environmental situations and availability of services/support
Facilitate assigned courts by serving as a liaison between the court and treatment providers
Manage housing program for re-entry clients
Document contracts and maintain client records
Enter and maintain progress notes in computerized chart and conduct chart reviews to ensure that all documentation meets with best-practice standards; reviews cases with supervisor to ensure compliance
Prepares a variety of reports and documents related to the individual's services, progress, and/or other related issues/concerns
Monitors individual's progress on a regular basis via office visits, phone contact, or care team meetings
Assists individuals in resolving problems related to treatment or rehabilitation issues and various living problems in home, work, or social environments; provides support as individuals cope with negative consequences of specific mental health and substance use issues
Develops case/service plan in concert with individual and partnering agencies
Assist Program Manager in assessing basic resources accessed by re-entry
Educate and recruit housing providers and support providers through the vendor set-up process
Monitor housing placement for suitability and livability and report issues to provider and Program Manager

Schedule: Monday, Tuesday, Thursday, Friday 8am-4:30p, Wednesday 1pm-9pm
Summary of Job Description:
Help to develop and implement treatment plan goals and targets for Members in collaboration with the multi-disciplinary team. Participate in crisis management, program activities, and provide direct oversight for all members. The CM works closely with members on goal attainment and supports members as they transition into less structured living arrangements.
Specific Responsibilities:
Collaborate with admissions to support the admission and acclimation of new Members into the program setting.
Complete Incident Reports in accordance with processes outlined by the Managed Care Organization (MCO), State and Project Transition/ PCS Mental Health Policy and Process.
Collaborate with members of the multidisciplinary team to develop treatment plans for identified Members.
Complete weekly individual CaseManagement sessions with Members and document in DAP format.
Facilitate/ Co-facilitate all groups as assigned and complete documentation.
Support with transporting Members to appointments per the directive of the Program Director.
Support with observation of Member drug and alcohol screenings and document observations (if applicable).
Participate in phone coaching/ crisis management process, which includes (but is not limited to):
Orienting Member and family to phone coaching process.
Carry and respond to coaching phone calls on days assigned, including being available to go onsite for emergencies during hours when there is no or minimal staff coverage.
Provide coverage for staff callouts.
Following incident reporting and documentation process for all on-call emergencies.
Support Members with links to services within the community.
Complete family contacts for assigned Members per requirements.
Ensure all Members have an emergency contact documented.
Ensure all consents are completed and updated yearly.
Review all Member's Advance Directives annually.
Support Members with activities of daily living including (but not limited to) the following:
Daily oversight and coaching of daily living activities.
Observing Member self-administration of medication.
Support with the development of socialization/ communication skills, conflict resolution, de-escalation skills, and utilization of crisis supports.
Support with meal planning, menu development, food shopping and meal preparation.
Encourage and support health and home safety practices.
Support with navigation of public transportation services.
Completion of residential roving and checklist to ensure safety.
Support with Discharge Member Follow-Up.
Complete continuing educational requirements as aligned with regulatory compliance.
Attend individual supervision sessions and program meetings as scheduled by direct supervisor.
Ensure documentation of all Member engagements is completed within 24 hours.
Other duties as assigned by the Program Director
Additional Performance Expectations:
Participate in multidisciplinary treatment team and will support and implement interventions and directives as directed by the Team.
Always demonstrate compassion and concern when supporting a Member through embracing Project Transition/ PCS Mental Health's Mission and Core Values.
Approach Member engagement from a non-judgmental stance understanding that a Member's behavior is driven by experience, which may include trauma.
Treat and speak to Members with supportive kindness even when a Member demonstrates intense behavioral or emotional actions. Staff will show Members dignity and respect for their values and lifestyles.
Seek out appropriate support, consultation with Clinician or Psychiatrist (if applicable), in conjunction with the Program Director or obtain supervision, when they are uncertain about how to respond or support a Member effectively.
Report back to the Treatment Team any observations of Member behavior that suggests Member may need additional treatment interventions and/or support.
Engage with all external parties/ individuals with professionalism and with a positive customer service approach, understanding that they are always representing the organization.
An understanding of an agreement to value the concepts of a Trauma Informed workplace.
Qualifications:
Bachelor's degree and 1 year experience in Mental Health, Behavioral Health or Drug and Alcohol field.
Excellent communication, interpersonal, organizational and time management skills.
Demonstrated ability to work as a part of a multi-disciplinary team.
High energy individual with strong work ethic and ability to multi-task.
Must be able to work in a self-directed manner.
Ability to maintain confidentiality.
