Horizon Health is seeking a certified therapist for our BRAND NEW opioid treatment center in Petersburg, VA The Opioid Treatment Program (OTP) Therapist/ Counselor is responsible for providing counseling, educational, and referral services to enrolled patients and their families as defined by OTP protocols. Counseling services include individual, group, and family counseling. The OTP Therapist/ Counselor is responsible for providing a range of cognitive, behavioral, and other substance use focused counseling, reflecting a variety of medically necessary evidence-based, individualized, person-centered care. The OTP Therapist completes individual psychosocial assessments and assigns treatment objectives and goals accordingly. The OTP Therapist works closely with other members of the interdisciplinary team to provide comprehensive care to enrolled patients.
Responsibilities:
Acts as primary counselor to address substance use disorders.
Develops individualized, PCP and its ongoing revisions in coordination with the Client and ensures its implementation.
Provides ongoing assessment and reassessment of the Client based on their PCP and goals.
Provides individual and group counseling based on the Client's individualized, PCP.
Provides crisis interventions, when clinically appropriate.
Provides substance use, health and community services education.
Provides coordination and consultation with medical, clinical, familial, and ancillary relevant parties with Client consent.
Ensures linkage to the most clinically appropriate and effective services along with arranging psychological and psychiatric evaluations.
Informs the Client about benefits, community resources, and services.
Advocates for and assists the Client in accessing benefits and services.
Monitors and documents the status of the Client's progress and the effectiveness of the strategies and interventions outlined in the PCP.
Other duties as assigned.

Required Education and Experience
Bachelor's degree in a human services or related field.
Registered with the Virginia Board of Counseling as a QMHP, or be registered with the respective boards as a QMHP-T or QPPMH, under supervision.
Registration with the Board of Counseling as a Certified Substance Abuse Counselor (CSAC).
Two (2) years of experience providing substance use treatment to adults with co-occurring serious mental illness and substance abuse disorders in a mental health setting.
Valid Driver's License with a point balance of -6 or higher.
Preferred
Three (3) years of experience working with dually diagnosed adult consumers in a mental health setting.
Required Knowledge, Skills and Abilities
Must be well versed in substance use treatment modalities and stages of change.

ACT is an essential program, operating 365 days of the year. Applicant must be able to work some evenings (until 6:00pm), weekends, and holidays, based on program needs, and work with a rotating on call schedule.

Required Education and Experience
Bachelor's degree in human services field or related field.
One (1) year of position-related experience providing community-based mental health and casemanagement services to the SMI and SUD populations.
Must be registered as a QMHP or QMHP-T with the Virginia Board of Counseling.
Valid Driver's License with a point balance of -6 or higher.
OR
Have any equivalent combination of education, training, and experience that provides the required knowledge, abilities, and credentials for the role.
Required Knowledge, Skill and Abilities
Knowledge of seriously mentally ill and co-occurring populations; psychosocial rehabilitation of the SMI; casemanagement services; casework documentation; available community resources and service delivery systems.
Excellent written, oral and interpersonal communications.
An ability to effectively consult and connect clients to emergency/crisis intervention services.
Ability to exercise sound judgment and demonstrate initiative.
Ability to sustain cooperative/effective working relationships with Greater Reach CSB and non-Greater Reach CSB personnel.
Knowledge of the laws and regulations related to providing casemanagement services.
Demonstrates proficiency in basic computer skills and programs.

Key Responsibilities & Duties
In this role, you will be responsible for the overall coordination and management of services for assigned participants across multiple programs. This includes reviewing intake documentation, gathering additional information as needed, and developing individualized service plans (ISPs) in collaboration with individuals, legally authorized representatives, caregivers, support coordinators, and other stakeholders. The role involves completing annual assessments, maintaining accurate and comprehensive documentation, monitoring progress toward ISP goals through documentation review and on-site observation, and completing required quarterly reports. The CaseManager serves as the primary point of contact and advocate for participants and their support networks, attends and facilitates planning and problem-solving meetings, interfaces with community partners and agencies, and maintains current knowledge of community resources.
Requirements & Qualifications
Qualified candidates will have a bachelor's degree in a related health or human services field, which is preferred; however, a combination of education, training, and experience may be considered instead of a degree, with a minimum requirement of a high school diploma or equivalent. Candidates must have relevant specialized training, certifications, and work experience, including at least two years of experience working with individuals with disabilities or medical concerns, with demonstrated experience in service management and related documentation. Strong knowledge of community resources, human services systems, and applicable regulations is essential, along with excellent communication, analytical, and documentation skills. The ability to work independently, manage multiple priorities, and travel between program locations using personal or alternative transportation is required.

Attend weekly support team meetings with management.
