General Description
Permanency Resource Center CaseManager serves families with the unique needs of post-adoptive, post-guardianship, and kinship placements into their families. This position will provide supportive services in the home or community. The position will connect families to resources such as parent training, peer support, mentoring, navigation for cross-system needs, therapeutic services referrals, counseling, respite activities and other supports to help families address issues as they arise. The CaseManager will exercise casework assessment skills and considerable independent judgment, decision making, and creativity in assisting the individual's/family's personal growth and development while enhancing the stability of their social network. Supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment.
Essential Duties and Responsibilities
Provides therapeutic services to families primarily though home visits.
Services will include but are not limited to individual, couple, family, grandparent(s) or other relatives/child(ren) counseling; parent training/education; advocacy as needed.
Provides casemanagement, information and referral to additional supportive services that may include counseling, education and advocacy.
Serves as group co-facilitator for Adoptive and Kinship families utilizing select curriculum to strengthen familial relationships utilizing a trauma informed approach.
Develops and implement community activities for intergenerational families, adoptive families that will strengthen their relationships.
Works collaboratively with Monroe, Wayne, Livingston and Steuben Counties, the NYS Kinship Navigator and the NYS Office of Children and Family Services

Attends local community events to educate community and obtain program referrals.
Stays up to date on community resources and services that can benefit parents and children in the program.

The Permanency Specialist II Youth delivers permanency services using the Hillside Family Finding Model or the Wendy's Wonderful Kids Child-focused Recruitment Model. The Permanency Specialist II - Youth also facilitates Permanency Roundtables for youth in need of permanency who are receiving services from Hillside. Both require intensive collaboration with stake holders in order to develop emotional, relational, and/or legal permanency for youth.
This is a Full Time position - 40 hours per week. Hours will vary throughout the week as they are flexible based on client need.
Essential Job functions
Implement the Family Finding and/or Child-focused Recruitment Model to fidelity with assigned youth.
Facilitate, scribe, and/or coordinate Permanency Roundtables for referred youth.
Engage youth, caregivers, relatives, fictive kin and/or other appropriate adults to increase connections and build a lifetime network of support for each youth and their families.
Support teams in determining the type of permanency intervention needed for youth.
Educate youth, families, providers, and strategic partners (i.e. DSS, OCFS) about the positive impact of Family Finding and/or Wendy's Wonderful Kids permanency services.
Participate and provide expertise in Service Plan Reviews, Treatment Conferences, and Benchmark team meetings as it relates to permanency.
Develop, maintain, and update all required documentation in a timely manner utilizing effective written and oral communication.

Education & Experience
Associate's degree in human services or a related field with minimum 2 of years relevant experience; OR
Bachelor's degree in social work, psychology, or human services required; OR
Bachelor's degree in any field with minimum of 2 years of relevant experience required

KEY RESPONSIBILITIES:
Resident Assessment and Care Plan Development:
Conduct comprehensive evaluations in compliance with ALR/SNALR/EALR NYS DOH regulations of residents' cognitive, physical, emotional, and social needs upon admission and throughout their stay.
Collaborate with the Administrator, licensed nurses, medical professionals, care partners, and families to create personalized wellness plans that address the specific challenges of memory impairment, identified needs and goals.
Regularly review and update wellness plans in coordination with the RN CaseManager and the Administrator.
Family Support and Education:
Offer counseling and education to families, helping them understand the progression of memory-related diseases and strategies for caregiving.
Work closely with family members to ensure they understand the resident's wellness plan and assist them in coping with the challenges of caregiving and progression of memory loss. Provide education on aging-related issues, caregiving skills. Provide information about community resources, and long-term care options.
Facilitate family meetings to discuss wellness plan updates.
Behavioral and Emotional Support:
Provide emotional support and counseling for residents experiencing anxiety, depression, or confusion due to memory loss.
Develop strategies to manage difficult behaviors associated with dementia, including agitation, aggression, or withdrawal.
Offer one-on-one support and activities that stimulate cognitive function and promote positive emotional well-being.
Advocacy:
Advocate for residents' rights, ensuring they receive appropriate care and services.
Address any concerns from residents, families, or staff, ensuring that the residents' wishes and needs are respected in accordance with their wellness plan.
Act as a liaison between residents and staff to address concerns and resolve issues.
Resource Coordination:
Identify and connect residents and families with community resources, such as legal assistance, financial support, and healthcare services.

Crisis Intervention:

Training and Staff Support:

Offer guidance on how to maintain a supportive and therapeutic environment for residents with memory loss.
QUALIFICATIONS:
Education: Bachelor's or master's degree in Social Work (BSW or MSW) from an accredited program OR 10 + years of relevant leadership experience in assisted living, memory care.
Experience: Prior experience in healthcare, assisted living, memory care, community health, or social services setting is required.
Skills: Strong communication, problem-solving, and interpersonal skills. Ability to work in a team-oriented environment.
Other Requirements: Compassionate, patient, and empathetic toward the elderly. Knowledge of aging-related issues and elder care resources.

Duties and Responsibilities:

Provide support to students who have an Individual Education Plan (IEP) or a 504 plan
Participate in a Manifestation Determination Review as part of the disciplinary proceeding
Address underlying issues regarding a student's misbehavior in accordance with the RCSD Code of Conduct
Assist parents, students, hearing officers, and district staff (administrators, principals and teachers) in considering restorative interventions and supports that encourage educational success
Provide services for the following; informal hearings, informal conferences, short-term suspensions, restorative meetings, local MDR/CSE meetings, transition meetings and family or criminal court proceedings related to school suspensions

General Description
The Health CaseManager provides comprehensive health casemanagement for refugee clients identified as having special health needs. Working collaboratively with departmental colleagues, this role ensures that vulnerable clients have access to both mental health care and specialized services available in the community. The Health CaseManager fosters consistent, collaborative communication across program staff and maintains a trauma-informed care environment to support client wellbeing.
Essential Duties and Responsibilities
Maintain a caseload of clients and ensure all case files are complete and compliant with agency, departmental, regulatory, and funder guidelines.
Conduct relevant screenings and assessments, including but not limited to psychosocial, mental health, financial, and employment status.
Develop, implement, and monitor comprehensive, solution-focused service plans with measurable goals.
Assist in discharge planning to ensure safe and effective transitions.
Facilitates, transports and accompanies clients to relevant appointments, activities, or meetings as needed.
Support clients in understanding and meeting the expectations of service providers.
Connect clients with opportunities for social support, such as recreational activities, faith-based groups, or peer-support programs.
Provide educational guidance on topics that promote client self-sufficiency.
Collaborate with referring agencies, funders, and internal staff to effectively address client needs.
Maintain regular communication with service providers to coordinate benefits and address treatment needs.
Report urgent client concerns or incidents promptly and in accordance with established protocols.
Advocate for clients by completing letters of request, referral forms, and eligibility documentation for charitable organizations and public agencies.
Maintain current knowledge of community resources and connect clients to appropriate opportunities and services.
Document all case activity in agency-approved databases or online platforms in compliance with regulatory standards.
Meet or exceed productivity and documentation standards to support the agency's financial stability.
Notify supervisor of significant client-related events and follow the program's incident reporting procedures.
Participate in program, departmental, and agency-wide meetings, case reviews, and training sessions.
Engage in professional development through continuing education, workshops, conferences, and independent learning.
Maintain up-to-date knowledge of relevant regulations, standards, and best practices for serving the client population.
Attend interagency meetings and represent the agency in community and provider forums as needed.
Collaborates with referring agencies, funders, and all related internal staff to effectively support individual client needs.
Adheres to all applicable federal and state laws and regulations including, but not limited to, those governing confidentiality, privacy, program, billing, and documentation standards.
Awareness and active support of the Agency's Mission, Vision, Values and Strategic Plan.

Qualifications
Education: A minimum of associate's degree in social work, Human Services, International Relations or related field preferred. Equivalent combination of education and experience will be considered.
Credentials: Valid and clean NYS Driver's license and the ability to conduct home visits. Valid PPD screening available.
Experience: Preferred minimum 1 - 2 years of experience with casemanagement and community outreach in areas such as but not limited to persons affected by substance use disorder, geriatrics, mental health, and refugees. Knowledge of Medicaid, SNAP, Social Security, and legal systems are helpful. May require performance of duties in various community locations and community settings that may not be handicapped accessible.

RiseWell Community Services is seeking a Full Time Overnight CaseManagers to play a pivotal role on one the Governor's offices newly launched innovative Safe Options Support (SOS) team that that will provide comprehensive care to street homeless or subway dwelling individuals in Brooklyn & Queens.
In this role, you will work as a member of the Overnight multi-disciplinary team to assess health and social needs of the homeless population. Which will include late evening to early morning hours (including weekends and weekdays). This includes but is not limited to assisting the team with: community outreach/canvassing, specifically on the streets and subways throughout the Brooklyn/Queens area, coordinating participant's referral with appropriate overnight facilities, accompany, advocacy, and assist on ensuring warm hand offs and follow-ups are maintain with SOS day teams, etc. Maintaining collaborative relationships with community partners including the Department of Homeless Services, MTA, local law enforcement, community hospitals, existing outreach teams and housing drop-in shelter.
QUALIFICATIONS:
A minimum of 4 years of experience in casemanagement. Bachelor's degree in Human Services preferred with previous casemanagement experience. Clean valid NY State Driver's License.Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

The Role
CaseManager I is an entry-level counseling/casemanagement position. The CaseManager I provides casemanagement/substance abuse counseling/advocacy services to an assigned caseload and participates as a member of an interdisciplinary treatment team to facilitate the client recovery/rehabilitation process. These goals are accomplished by providing individual and group counseling, making appropriate interventions as needed and contributing to the healthy integrity of the therapeutic/program environment.
What You Will Do
Depending on site will perform some or all of the following:
Provides casemanagement/advocacy services to an assigned caseload.
Participates in monitoring therapeutic environment to ensure that behavioral guidelines are followed, a therapeutic milieu is maintained and interpersonal relationships remain positive.
Provides individual counseling and facilitation of caseload group meetings and other assigned groups.
Provides psycho-education and counseling germane to chemical dependency and relapse prevention.
Administers appropriate behavioral interventions as needed.
Participates in assessment screenings to determine client needs/preferences; formulates and implements treatment, transition and discharge/continuing care planning collaboratively with individual clients.
Facilitates re-socialization process of clients by serving as a role model. Provides role modeling regarding ethical and professional conduct.
Assumes staff-on-duty assignments as requested, including observation of urine specimen collection.
Assumes responsibility for medication management including the direct observation of clients during medication pass for adherence to their prescribed medication regime.
Conducts facility/safety runs and room checks as assigned to visually assess clients for use of alcohol/ drugs or environment of care for unsafe conditions.
Ensures maintenance of accurate, complete, timely and high-quality client records and reporting of client information that comply with external regulatory standards and agency policy and procedure.
Complies with 42 CFR confidentiality and HIPAA privacy and security regulations.
Adheres to all responsibilities and duties of a New York State mandated reporter.
Works cooperatively with other staff as a member of the interdisciplinary team.
Functions as a liaison/advocate to clients' families and/or other agencies as needed.
Provides outreach/education/prevention services to the local community, schools, groups, and agencies as necessary and as approved by management staff.
Provides crisis intervention/management, generating referrals to agency or external mental health providers or other service providers, as indicated.

Completion of 350 CASAC clock hours completed preferred.

Strong individual and group counseling skills.

Ability to participate in CPR and First Aid training.
