DUTIES AND RESPONSIBILITIES
Reflects the company's mission and values in developing and maintaining professional working relationship with others.
Performs initial and periodic assessment of clients to determine care needs, implements interventions, and evaluates the effectiveness of care to achieve positive outcomes.

Ensures effective and appropriate financial management of the client's home health services are met.

PHYSICAL/ENVIRONMENTAL DEMANDS
Client care provided in individual client homes and may require transfers and lifting with adaptive equipment. Must be adaptable to a variety of environments and community settings. Must be able to drive or use public transportation in all types of weather.
Heavy. Amount of weight moved; 50 to 100 pounds occasionally; 25 to 50 pounds frequently; 10 to 20 pounds constantly. Physical Demand requirements are in excess of those for Medium Work.
QUALIFICATIONS
Currently licensed as a Physical Therapist (PT) in the State of Minnesota. The license must not have been revoked, suspended and without limitations or restrictions.
Proof of negative TB testing: Mantoux skin test, single TB blood test or chest x-ray.
Access to a dependable automobile in order to travel to multiple company business stops a day.
A valid driver's license and proof of current car insurance when using a personal vehicle for company business.
Have U.S. Citizenship or evidence of valid Alien Work Permit.
Disclose any conviction and criminal history records pertaining to any crime related to the provision of health services. A candidate who has been convicted of such crimes will not be hired.
Pass initial and ongoing background studies and screenings including but not limited to those of the Minnesota Department of Health and the Federal Office of the Inspector General's List of Excluded Individuals and Entities.

We are looking for an experienced casemanager to join the St. Cloud Youth Shelter! Use your casemanagement skills to support youth on their journey to safety and stability.
180 Degrees' youth shelters serve adolescents ages 12-19 who are seeking safety, stability, and support during critical moments in their lives. Youth may enter shelter through county placements, or because they are experiencing homelessness, running away from home, family conflict, exploitation, or safety concerns. Many youth have experienced trauma, instability, or disrupted relationships and come to us at a time when trust, consistency, and dignity matter deeply.
The Shelter CaseManager provides intensive, relationship-based casemanagement to youth residing in an emergency shelter. This role focuses on stabilization, safety, planning, and transition, working closely with shelter staff and community partners to support youth toward their next steps. CaseManagers practice trauma-responsive, youth-centered care that emphasizes advocacy, collaboration, and clear planning rather than control or enforcement.
This position does not have direct reports. This position is 40 hours per week, onsite.
Duties and Responsibilities include the following. Other duties may be assigned.
1. Direct CaseManagement with Youth
Provide 1:1 casemanagement primarily through in-person meetings, with phone, text, and virtual communication used as appropriate to the situation.
Meet with youth 1-3 times per week, with increased contact during intake, transition periods, or times of higher need.
Conduct thorough intakes, including required screenings and assessments that directly inform the youth's case plan.

Represent youth and advocate on their behalf in meetings with care teams, counties, and community partners.
Participate in external collaborative spaces, such as Safe Harbor or Continuum of Care (CoC) workgroups or case conferences, as assigned/appropriate to the role.
3. Family, Caregiver, and Worker Engagement
Serve as a liaison with parents, caregivers, guardians, caseworkers, probation officers, and other involved adults.
Support family and caregiver engagement and reunification efforts when appropriate and aligned with the youth's plan.
Provide regular, appropriate progress updates to families, caregivers, and workers based on youth needs, relationships, and circumstances.
4. Transition and Aftercare Coordination

When a Mobile CaseManager (MCM) is assigned, coordinate closely to ensure smooth handoff, clear communication, and continuity of services.
When a MCM is not assigned, provide appropriate aftercare follow-up within program scope until transition is complete.
5. Documentation and Data Management
Complete timely and accurate documentation in Power180 (electronic client record system), paper client records, and HMIS, REDCap, or other required systems.
Ensure all required client records are current, complete, and well-organized, including case plans and updates; transition and discharge documentation; safety and risk management plans; screenings; assessments; and progress notes.
Monitor youth progress toward goals and ensure documentation aligns with licensing, grant, and internal quality standards.
Following a youth's discharge, conduct a final review of all client records to ensure documentation is complete, accurate, and closed out appropriately.
6. Shelter Leadership and On-Call Responsibilities
Serve as a leader within the shelter, modeling trauma-responsive practice, professionalism, and clear boundaries.
Participate in on-call rotation as assigned.
Provide consultation and support to shelter staff during complex or high-needs situations, in collaboration with the Program Manager.

Job Description
The Mental Health Coordinator II (MHCII) helps manage many aspects of care and support for individuals in a group home setting. Responsibilities include:
Assisting with personal finances
Planning activities and programs
Supporting behavioral needs
Encouraging family involvement
Overseeing education or job programs
Managing health, nutrition, and the household
Creating staff schedules
The MHCII also helps the supervisor keep programs consistent and ensure good communication among staff and support team members. During direct care shifts, the MHCII also performs the duties of behavioral support staff.
This job is similar to roles like Lead Staff, Program Coordinator, House Manager, or Program Supervisor.
Qualifications

Experience:
