Case Managers are responsible for supporting individuals with disabilities by providing clinical management and oversight of the professionals who work with them, as well as building relationships and overseeing the families and individuals on their caseload.

SUMMARY OF RESPONSIBILITIES
* Conduct intakes: meet with families and/or individuals in an effort to explain the Organization's services.
* Provide ongoing supervision and training to unlicensed direct care staff.
* Assign appropriately skilled Direct Support Professionals (DSPs) to ensure services are being provided at the highest quality.
* Attend quarterly, yearly, or as-needed meetings for all clients on caseload.
* Review staff monthly and quarterly notes for completion, and update as necessary.
* Create and maintain clinically appropriate goals.
* Work with families and internal supportive teams to initiate, coordinate, and facilitate, smooth positive transitions when individuals enter the program.
* Work in partnership with colleagues, families, and individuals to establish positive, trusting relationships, and consistent clear communication.
* Utilize effective problem solving skills and act as a model/mentor for others in demonstrating professional ethics, standards, and practices.
* Supervise unlicensed staff.
REQUIRED EDUCATION AND EXPERIENCE
* Minimum of a Bachelor's Degree in Psychology, Social Work, or a related field.
* Minimum of 1-3 years' experience working in the field of human services conducting intakes, assessments, and in-home services.
* Consistent access to a reliable vehicle.
* Willingness and availability to travel or drive throughout the region as needed
* Must be a self-motivated and independent worker who can manage a flexible schedule.

Responsible for following Amcord Care Inc. policies and procedures for document maintenance, confidentiality or records and employees' rights.

Qualifications:

Qualifications: Required:
• High school diploma or equivalent
• Has maintained at least 12 months of successful full or part-time paid or volunteer experience or has obtained at least 24 credit hours of postsecondary education.
• Successful completion of the Peer Support Specialist training will be required upon hire, if not previously obtained. • Identifies themselves as a mental health consumer in recovery or as a primary support person for an individual diagnosed with a serious mental illness.
• In recovery with at least two years sustained, current recovery history.
• Valid driver's license, proof of auto insurance, and Act 33/34 and FBI clearances. EOE/ADA.

Northeast Counseling Services is the mental health crisis service provider for central and southern Luzerne County. We are currently seeking individuals to become part of our team providing assessments on mental health clients seeking help. Positions available are for per-diem work, great for professionals who want to pick up a few additional hours a week. These positions offer flexibility in scheduling, and the ability to work a range of different shifts.
The Mobile Worker On-Call (COC) provides in person assessment and intervention for adults, adolescences and children experiencing psychiatric emergencies in the Northeast Counseling service area when phone intervention by the Emergency Worker is not adequate, during non-working hours of the center. The Mobile Worker can provide intervention either individually or as a team based on the presenting problem and individual need of the client.
THE POSITION REQUIRES AN INDIVIDUAL TO HAVE ONE OF THE FOLLOWING:
Have a bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, education or a related field.
Be a registered nurse.
Have a high school diploma or equivalency and 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years of experience in public or private human services with 1 year of mental health direct care experience.
Have a high school diploma or equivalency and 3 years of mental health direct care experience in public or private human services with employment as a mental health staff person prior January 1, 1992.
Be a consumer or a family member who has 1 year of experience as an advocate or leader in a consumers or family group, and has a high school diploma or equivalency.
Staff persons employed by a provider who have 5 years' experience as a supervisor of mental health services in a mental health agency prior to January 1, 1992, are exempted from this section.
Must poses a working knowledge of the philosophy, concepts, mandates and operations of community health services and other human services agencies. They must have a knowledge of crisis intervention techniques and be proficient in writing and verbal communication skills.
