  • Supervisors are responsible for assuring that the DBH Practice Guidelines are put into operation to the extent possible within the general operations of the TCM duties.
  • The primary responsibility of this position is to provide daily on-site supervision to Teams of Blended Case Managers. This supervision should entail Team meetings in which the proposed daily contact for each staff is discussed. During this discussion supervisors are expected to utilize the team member's to address any problems that the Team member feel they need help with and use the members to cover interactions/tasks that one BCM might not be able to complete by themselves.
  • Included in the general supervision duties is the requirement that every effort be made to provide weekly individual supervision to each assigned staff and that these sessions will be documented for review. During this individual supervision a chart review should be completed using the updated Utilization Review sheet with particular attention paid to whether or not a PGP exists to cover the time period since the last CBH review.
  • Daily monitoring of the 30 day contact board and if a contact is due make sure that the contact is completed before an infraction occurs.
  • Assure that all Pay 4 Performance criteria are being met:
  • Supervisors are responsible for assuring that the staff under their supervision are making every attempt to complete the documentation in a timely fashion and that it meets the general guidelines of legibility, it is signed, dated and that the content follows the DAP format and explains the nature of the contact.
  • Supervisors are expected to monitor the productivity of the staff assigned to them using the reports provided through the TCM director.


Requirements:

  • Masters Level degree in social work, psychology, rehabilitation, activity therapies, counseling or education and
  • Three years mental health direct care experience - one of which includes supervisory experience.
  • Applicant must have knowledge of and the ability to put into practice the Blended Case Management Standards of Care.
  • On-call responsibility & completion of BHTEN 10-week case management orientation, as well as a valid driver's license, is required.
  • Bilingual (Spanish/English) is required.

The Case Manger Supervisor is an integral part of the whole residential program overseeing the team of Case Managers who work with our clients to develop transition plans, provide group counseling and work toward client discharge and aftercare services. The Case Manager supervisor will oversee staff in all areas of staff development, performance and policy, case management, and reporting data by thoughtfully assigning workload to challenge and grow individual skills. As the Case Manager Supervisor you will also interpret and review the Case Management data, create insightful and comprehensible reports and lead the facilitation of various groups or meetings.

ESSENTIAL FUNCTIONS include but are not limited to

+

+ Supervise Case Manager staff, relating to both security and case management.

+ Oversee staff response to all shift problems and emergency incidents.

+ Review all client related paperwork, regarding content, timeliness and quality. Review all client files on a monthly basis for organization and content.

+ Monitor client compliance with treatment stipulations and recommend modifications to program director.

+ Monitor staff performance and provide regular feedback to program director on all staff.

+ Perform all duties and responsibilities of case manager, including carrying a caseload as required by program needs, as relates to counseling, case record management, intake assessment, discharge preparation, aftercare services, security, resource management and facility maintenance.

+ Assist program director in developing, leading and supervising client group meetings, program services and counseling sessions. Monitor and evaluate group activities on an ongoing basis.

+ Assist program director in training staff in areas of security procedures, client supervision, client paperwork.

+ Supervise the completion of urine collections, telephone spot checks and employment and home site visits of clients to verify and document client movement and record information.

+ Monitor staff compliance with all policies and procedures pertaining to the safety and security of clients, Agency property and the community.

+ Assist program director in setting agenda and participate in weekly staff meetings.

+ Participate in on call agency duty officer rotation as assigned.

+ Bachelors degree in a social science field and two years experience in case management or similar field. Education may be substituted with experience on a year for year basis.

+ One year experience in management and supervision of staff or three years of case management experience.

+ Excellent oral, written and interpersonal skills desired.

+ Ability to engage and develop collaborations with community agencies.

+ Previous assessment experience preferred

+ Experience with Microsoft Office

+ COVID19 vaccination required or a negative test prior to start

About your role:

The Care Management Supervisor is responsible for the successful provision of Tailored Care Management services to clients with complex behavioral health and healthcare needs. This position will supervise a team of no more than 8 Care Managers and indirectly supervise Care Manager Extenders. The Care Management Supervisor will partner with regional and state leaders to ensure the compliance with regulatory requirements and to meet or exceed established key performance indicators and desired client outcomes.
