The Lead Care Manager ensures quality care and services are being provided and documented, in a consistent manner and in alignment with each resident's individualized Service Plan (ISP) and Sunrise policies, to residents in our care neighborhoods. The Lead Care Manager leads by example in all job responsibilities, and may work variable hours to likely include some weekend and holiday duty. RESPONSIBILITIES & QUALIFICATIONS Essential Responsibilities : • Provide supervisory support by taking attendance at the beginning of every shift; managing call-offs/no shows; sending open shift messages and responding to shift requests • Support team members as they provide Activities of Daily Living (ADL) care and services to residents in accordance with their Individualized Service Plans (ISP). This may include bathing, grooming, personal and dental hygiene, bowel and bladder functions, dressing, hair care, escorting to and from meals and activities and tidying residents room • Support team members as they encourage personalized and meaningful resident participation in life enrichment programming and ADLs • Assist with meal service in dining room. Coach team members on hospitality standards during meal service • Accurately document care and services provided to residents. Review care manager documentation throughout shift. Identify and bridge gaps in documentation by coaching team members • Ensure compliance with Sunrise's Timekeeping and Meal/Rest Period policies. Monitor team member break schedule and duration; ensure care managers clock in/out using correct job codes. Coach team members as required • Display and encourage flexibility in work schedule by working holidays, evenings, weekends and additional shifts when necessary • Prepare for shift by obtaining resident group assignments before care managers arrive. Print and hand out daily assignment sheets to appropriate team members • Participate in crossover meetings by sharing/receiving pertinent information with/from other Lead Care Managers • Carry a resident group assignment on overnight shifts and pick up group assignments in the event of an unfilled call off on day and evening shifts Other Responsibilities • Cross trained and credentialed (including certifications where required by state) to safely administer medications if needed during times of high volume or unexpected absence • Lead by example when clocking in/out and taking proper meal/rest breaks • Participate in group interview process including job preview video and community tour • Support onboarding of new team members including shadowing and skills demonstration • Maintain a safe and secure environment for all staff, residents and guests by following established safety standards; actively support safety practices • Report incidents and complete appropriate paperwork immediately • Communicate any observed or suspected resident change of condition to the department care coordinator; assist care coordinators, when required, in development and/or updates to ISPs and communicating with residents and families • Prevent and handle conflict by working with the team towards a solution. Communicate issues to department care coordinator when necessary • Contribute to care manager annual performance review by providing feedback to department care coordinator • Project a positive, professional and friendly image through action, words and dress Qualifications: • High School diploma or GED strongly preferred and may be required per state regulations • Ability to read, write, communicate effectively with Care Managers, Community Managers, residents and family members • In states where appropriate, must maintain applicable certifications • Must be at least 18 years of age • Medication Management Certificate required; ongoing training and re-education per Sunrise and state requirements • Previous experience working with seniors and desire to serve and care for seniors • Demonstrates leadership competencies • Ability to delegate assignments to the appropriate individuals based on their skills, roles and interests • Ability to make choices, decisions and to act in the residents' best interest • Ability to appropriately react to and remain calm in difficult situations • Ability to handle multiple priorities • Competent in organizational and time management skills • Demonstrate good judgment, problem solving and decision making skills • Competent in computer skills, Microsoft Office, and Sunrise applications with the ability to learn new applications • As applicable, all Sunrise team members who drive a Sunrise vehicle must sign the Driver job description and understand the key essential duties for safety and regulatory compliance ABOUT SUNRISE Sunrise Senior Living has championed quality of life in senior care for more than 30 years. Members who need assistance call DAN's emergency hotline where queries are triaged by medical consultants who work with the treating physician to determine the medical needs and possible medical transportation needs of the member. Once a plan is finalized, the case is transferred to the TravelAssist Case Manager who is responsible for the operational execution of the mission. Missions often begin with an emergency medical transportation to the closest medical facility. Other services include monitoring the member's condition and the mission as it progresses, keeping family members informed of the members situation, arranging follow up medical transfers, hyperbaric chamber treatments, physician visits, and more. Once the medical emergency passes, the TravelAssist Case Manager will help arrange transportation home, or to another medical facility for further care or treatment. The TravelAssist Case Manager must exercise independent thinking and critically evaluate information to make appropriate decisions regarding mission parameters to ensure the best outcome for members. Responsibilities: · Coordinate and provide safe, timely, effective, efficient, and member-centered care. · Promote quality and cost-effective interventions to improve outcomes. · Handle case assignments, draft service plans, review case progress, and determine case closure. · Facilitate multiple care aspects (case coordination, information sharing, etc.); interact with members to monitor their progress and ensure satisfaction. · Work with other DAN departments to develop and maintain an international database of local providers (air ambulance, hospitals, etc.) to supplement the existing Hyperbaric Chamber and Referral Physician databases. · Work with the DAN Medical Services department to develop clear lines of responsibility for case management and efficient SOPs for the management and transfer of cases. · Inform members of available DAN membership and insurance benefits so the member can make informed decisions. · Record case information, accurately complete all necessary forms, and produce reports as needed. · Adhere to professional standards outlined by DAN's protocols, rules, and regulations. Minimum Requirements: · Proven working experience in travel assistance and/or medical case management, including expertise and knowledge in emergency/critical and acute care; medevac transport services, and understanding of flight physiology is a plus. · Excellent knowledge of case management principles, healthcare management, and reimbursement. · Critical, independent thinking where outcomes are often time-sensitive; must be self-motivated and able to work in high-pressure situations. · Effective communication skills, verbal and written, to build and maintain internal and external relationships. · Excellent organizational and time management skills, with the ability to multi-task, problem solve, prioritize, delegate, and meet critical deadlines. · Compassionate, with teamwork skills. · Typing and computer skills with proficiency in Microsoft Office software. · Bachelor's degree in the healthcare field or equivalent experience. The Supervisor of DCM's will monitor and evaluate DCM's performance and effectiveness through feedback from LTRG teams and Clients, Construction Cost Analysts, Volunteer and Materials Management and the “Funding Table or Unmet Needs Table” members. Be responsible for the overall success of Survivor recovery in a timely manner through monitoring the DCM process documents and the client spreadsheet updates for such indicators as #Cases Open, #Cases Active, #Cases Referred and #Cases Closed. Principal Duties and Responsibilities (Essential Functions) include: Help to recruit, interview, train, develop and evaluate the DCM staff Explain and enforce existing DCMP forms, policies and procedures Implement and monitor performance goals and objectives and performance expectations for DCM's May provide supervision of other local staff, such as Construction Manager, Materials and Volunteers Coordinator in direct coordination with the Program Manager and facilitates standard documentation practices and consistent identification of applicable resources for individuals and families Work closely with the local LTRG's and communities Ensure that the DCM's are presenting case information in a confidential manner to obtain funding support for necessary unmet needs for disaster survivor's recovery Be responsible for maintaining up-to-date client digital records Regularly monitor client files in the Data Management Systems to ensure that DCM's and resource providers have access to timely information about survivors and their progress towards recovery Responsible for maintaining the Data Management Systems database and record keeping of documents such as survivor leases, estimates, supporting documentation Ensure case managers develop and maintain 35 client case load Will perform periodic performance reviews for DCMs assigned to the team thru BambooHR Ensure all time is entered and notes appropriate to the DCMP into the time keeping software on a daily basis Other duties as assigned and required Qualifications and Skills Preferred: Minimum of five (5) years of experience in a related capacity.
