This will also consist of referrals to the Rescue Mission of Utica's Representative Payee services when deemed appropriate. PRIMARY DUTIES: Conduct face-to-face and home visits to all clients to assess client safety, risk, and to determine client needs at least once per month. Screen program referrals from OCDFCS on the date of referral. Meet with OCDFCS staff member and client within three (3) days of referral. Determine client current needs for day-to-day living (i.e. food, clothing, housing, medical care, and social supports) and assist clients in meeting those needs. Meet with OCDFCS to discuss client status and progress on a monthly basis, or more as requested. Provide monthly status reports of all clients to include financial and personal situation details. Maintain accurate client files and provide progress notes to OCDFCS for their permanent case record. The notes shall be updated and provided no later than ten (10) days from an added event.
