Salary ranges starts at $23.82 per hour. As a CaseManager, you will be responsible for:
Conduct casemanagement assessments to identify areas of need and service preferences;
Gather and integrate information from all available sources, including consumer self-reports, reports from family members, guardians and Adult Foster Care providers, documented medical and treatment history, needs surveys, assessments from other disciplines, etc.;
Utilize motivational interviewing techniques to assess and articulate the motivation of consumers to address specific needs identified during the assessment process;
Assist consumers in determining their eligibility for entitlements, community services and supports;
Provide consumers, family members and guardians with complete and accurate information regarding services, supports and other resources available to meet needs identified during the assessment process;
Consider and promote the utilization of group and support services;
Assist with the development of comprehensive and integrated Individualized Plans of Service with consumers and other members of the treatment team (consistent with principles of Person-Centered Planning, Integrated Dual Disorder Treatment, Self-Determination and current Medicaid Guidelines);
Provide face-to-face interventions to each assigned consumer at least monthly;
Conduct home visits and community outreach as needed to help engage consumers, identify needs and barriers, and assure access to services and supports;
Link consumers to community services and supports based on the needs and preferences identified in their Individualized Plans of Service;
Conduct skill building interventions with individual consumers and groups of consumers as indicated in their Individualized Plans of Service;
Maintain regular contact with treatment team members and other providers to monitor and coordinate service delivery efforts;
Reassess the service needs and preferences of consumers as needed, at a minimum every six months;
Coordinate the client's primary and other health care providers to assure continuity of care.
Coordinate and assist with crisis intervention and discharge planning, including community supports after hospitalization.
Facilitate the transition process from inpatient setting to the community, including follow up services.
Conduct crisis planning with client.
Provide clients with after hour crisis information.
Establish and maintain collaborative relationships with local providers and community organizations;
Develop, organize and routinely update a file of community resources;
Participate in regularly scheduled clinical supervision, case consultations, casemanagement and team meetings;
Document all face-to-face service activity and phone contact pertaining to consumers;
Perform within acceptable standards of productivity;
Complete all documentation within expected timeframes;
Respond in writing to routine chart reviews and audits;
Maintain appropriate state licensure or certification;
Complete 25 hours of program and/or position specific training on an annual basis;
Maintain appropriate professional ethics and boundaries;
Abide by all Guidance Center policies and procedures;
Other duties as assigned.
Requirements
To qualify for this position, you will need the following:
Candidates must be a human services professional with at least a bachelor's degree in a human services field.
Candidates must obtain the Qualified Mental Health Professional credential within 120 days of hire.
Must have one year of experience (may include time spent in fieldwork or internship) treating or working with people who have been diagnosed with mental illness.
Must have a valid Michigan Chauffeur License and a good driving record.
Must have the ability to engage adult mental health consumers, their families and supporters.
Must be an expert in helping consumers identify their service needs and preferences and helping them build on their individual strengths, resources and supports to navigate the network of services available at the agency and in the community.
Must have an orientation toward providing community outreach services.
Additional requirements include a nonjudgmental approach that reflects cultural competency and sensitivity to the unique needs and diverse experiences of adults with mental illness, their families and providers.

A DAY IN THE LIFE OF A CASEMANAGER
As a CaseManager, you assist individuals with developmental disabilities in navigating systems to better meet their needs. You understand that traditional services often aren't effective for this population and look for opportunities to increase access to additional supports. You find the gaps in support and help bring about improvement as you connect with multiple community partners and systems. Often, by the time you get involved, other systems and supports have given up or are close to it. Your job is to instill hope and reenergize efforts for recovery.
You assist consumers in identifying supports, building a team and designing a strength-based plan that focusses on recovery. The plan of service that you prepare is based upon discoveries about strengths and culture, a needs assessment and a crisis and safety support plan. WORK SCHEDULE
This person in this full-time position works a 37-1/2 hour workweek. The typical schedule is Monday - Friday, 8:30 AM - 5:00 PM with a one-hour unpaid lunch. There may be occasional times when he/she would need to meet with a client outside of these hours, but he/she would be offered a flexible work schedule to make up for these types of after-hour work.
The annualized salary for this position ranges from $53,976 (the starting annual salary) to $69,810 (each based on a 37-1/2 hour work week); the range is based on the number of years of service with the agency.

Overview

Position Summary:The Licensed mental health provider/Non-Clinical CaseManager shall provide year-round dedicated mental health and wellness services to Washtenaw Community College students. The primary goal is to facilitate student success and retention by providing short-term counseling and mental health support , non-clinical casemanagement, referral to internal/external counseling resources, crisis intervention, engaging students in self-care, promoting student success and wellbeing, and fostering behaviors that contribute to student degree completion. Respond to daily counseling and casemanagement functions including student emergencies, student referrals, walk-in requests, and faculty/staff/administrative consults. This includes regular responses to crisis and urgent mental health needs where wellness checks on students may be appropriate and follow-up necessary.
Ability to quickly make a strong therapeutic alliance with students of any background to support their emergency or day-to-day mental health needs.
Provide specialized support and intervention for WCC students experiencing complex mental health issues and crises. This includes comprehensive suicide risk assessment, safety planning (including lethal means management), and consistent application of WCC emergency mental health protocols.
Provide timely and appropriate referrals to internal and external support services, and coordinate intervention plans in collaboration with community resources using a casemanagement model
Assist with long-term needs of students including follow-up to ensure students have been successful in addressing issues/questions related to student crisis. This may include creating and managing post-intervention strategies for those students returning after a psychological emergency and/or hospitalization.
Monitor and track individual cases and student progress to ensure compliance with recommendations, including safety assessment and safety planning
Manage a caseload of students through an electronic documentation system and communicate with relevant campus partners regarding students of concern. This includes making sure all correspondence, notes and action items are accurately documented
Create and maintain mental health/wellness presentations, educational programs, and materials for faculty, staff, and students.
Perform outreach to faculty, staff, and students as well as other community stakeholders to support students in need
Prepare reports, proposals, and audits as required by the Executive Director of Student Success Services
Conduct research on best practices and evaluate benchmarks associated with the proactive assessment of initiatives supporting the mental health/wellness of current students.
Assist in the development of strategic goals that align with institutional goals relevant to student mental health/wellness needs
Collaborate as part of the college CARE team, a multi-disciplinary team designed to provide assessment and support to students of concern.

This is a PART-TIME position will support the Pathway Home 6 grant. This grant is expected to run for 42 months and provides funding for workforce development programs that support incarcerated individuals. The program goal is to improve employment outcomes and reduce recidivism by offering job training and other supportive services during incarceration and upon reentry into the community.

The Offender Services Team is comprised of competent and caring resource professionals who are innovative, resourceful, collaborative, and helpful in facilitating the reentry process for justice involved individuals who access resources at Kinexus Group. This responsibility requires that we be service oriented relative to the needs of all referred participants, while still assuring that all services adhere to federal, state and local policies and procedures, financial requirements as established through funding sources.

OUR DESIRED OUTCOMES:
The CaseManager will play a pivotal role in the success of the Pathway Home 6 (PH6) reentry initiative. Each CaseManager will be assigned to one of the three county jails and will support job seekers pre- and post-release. The primary goal is to help job seekers develop and follow Individualized Development Plans (IDPs), overcome reentry barriers, gain employment, and reduce recidivism through consistent, trauma-informed, and strengths-based support.
WHAT WE EXPECT FROM YOU:
As a CaseManager for the Pathway Home 6 Program, you will be the primary support for justice-involved individuals transitioning from incarceration to the workforce. You will lead the intake and assessment process, develop and manage individualized reentry plans (IDPs), and provide direct services both pre- and post-release. This includes coordinating training and employment opportunities, connecting job seekers to supportive services, and tracking their progress toward self-sufficiency and reduced recidivism. Your role requires strong communication, organizational, and interpersonal skills to collaborate with jail staff, service partners, and employers while maintaining accurate documentation and compliance with federal performance standards. Ultimately, you are expected to be a proactive, compassionate advocate who helps job seekers overcome barriers and achieve lasting success in the community.
CaseManagement & Participant Engagement
Manage participant documentation, enrollment files, and intake logistics
Provide one-on-one casemanagement to incarcerated and recently released job seekers.
Facilitate program intake, risk and needs assessments (e.g., LS/CMI), and career evaluations (e.g., JOFI).
Collaborate with jail staff and service providers to conduct in-jail visits twice weekly.
Develop and maintain Individualized Development Plans (IDPs) linked to training, employment, and support services.
Connect job seekers with supportive services (housing, legal aid, mental health, substance abuse treatment, etc.).
Service Delivery & Support
Assist job seekers in obtaining vital records such as ID, Social Security card, and birth certificate.
Provide job readiness training (resume building, interview skills, digital literacy, etc.).
Guide job seekers through job placement and post-release occupational training pathways.
Follow up with job seekers weekly for at least 12 months post-release to support stability and job retention.
Collaboration & Communication
Act as liaison between job seekers, jail staff, probation officers, public defenders, and external service providers.
Attend and contribute to weekly case coordination and staff performance meetings.
Work closely with the Program Coordinator to ensure timely, accurate entry of participant data into LS/CMI system.
Documentation & Performance Tracking
Maintain thorough and confidential participant records.
Collect employment verification, pay stubs, and training completion documents.
Monitor WIOA indicators and recidivism metrics, including participant rearrest and reconviction status.
Support data collection for quarterly and annual grant reporting.
Follow all policies and procedures related to casemanagement.
MINUMUM REQUIREMENTS:
Associate's degree or equivalent professional experience in human services, criminal justice, social work, or a related field (Bachelor's preferred).
At least two years of experience in casemanagement or direct services.
Strong interpersonal and motivational interviewing skills.
Comfort working in correctional and community-based settings.
Proficient in Microsoft Office and familiar with electronic casemanagement systems.
PREFERRED EXPERIENCE:
Experience working with justice-involved individuals or in reentry programs.
Familiarity with WIOA and LS/CMI casemanagement platform.
Understanding of trauma-informed care and wraparound service coordination.
WORK ENVIRONMENT:
Office-based with regular travel to jails, employer sites, and partner meetings.
Must pass background checks as required by jail facilities.
Flexibility in scheduling to accommodate access to correctional settings and participant needs.

General Summary:
The purpose of this position is to provide a wide range of community support and coordination of services to people experiencing serious mental illness and/or addiction challenges to ensure their needs are addressed. The CaseManager supports clients in the development of their short and long-term goals and acquisition of community resources using a person-centered approach. Essential Duties and Responsibilities:
Assesses client strengths and needs with the client and applicable team members, conducts comprehensive care coordination, and develops treatment plans. Monitors and evaluates client progress toward goal attainment. Reviews and modifies treatment plans as needed.
Maintains contact with the client as indicated in the treatment plan or as needed to ensure stability. Communicates with and provides support to clients in achieving goals and outcomes. Provides active outreach as necessary, including crisis intervention and management, and acquisition of appropriate community supports.
Links and/or refers clients to other services, programs, and agencies as needed. Assists in assuring clients obtain and retain applicable entitlements and follows up on same. Serves as an advocate for clients. With appropriate releases, communicates with family members and significant others as indicated.
Confers with supervisor and other team members regarding clients and, as appropriate, other agencies with emphasis on provision of most appropriate, effective, and cost-efficient services.
May provide face-to-face contact with clients who are hospitalized to facilitate discharge planning and coordination of services. Works with hospital and agency medical staff regarding same.
Assists clients in tracking and keeping appointments.
Assists clients with developing skills that lead to independence
Provides back up for other staff.
Completes required documentation in the electronic health record in accordance with timelines and standards required by the agency i.e. within 48 hours of interaction , regulatory bodies, and payors. Demonstrates excellent written and oral communication skills.
Carries out policies and procedures of the agency; may participate in community work, staff development, agency-wide projects; serve on agency committees/teams, attend meetings, and performs other related duties as requested.
Training requirements include CPR, CPI, new hire, Sanctuary and annual boosters in addition to any required trainings assigned during employment
Must be able to safely drive in all weather conditions in support of client needs

Provides services and activities under the direction of the Therapist and/or Program Manager in accordance with an Individual Treatment Plan (ITP) designed to meet the educational, vocational, residential, mental health treatment, financial, social and other needs of the recipient; provide ongoing revision of plan.
Ensures that delivered services and activities meet the goal of minimizing the negative effects of psychiatric symptoms or substance dependence that interferes with the client's daily living and personal development; Provides activities to support the client in the development of various skill building activities including: daily and community living, socialization and adaptation skills; development of leisure time interests and activities; symptom management; wellness and substance abuse education; employment readiness; behavior and anger management.
Deliver services pertinent to the ITP and specified goals including arrangement, linkage or integration of multiple services as they are needed or being received by the recipient, either between programs, within the area program, or from other outside agencies.
Documents services provided and progress against treatment plans and goals according to prescribed procedures including the name of the person supported, Medicaid identification number, date of service, purpose of contact, description of the intervention(s), the time spent performing the intervention(s), effectiveness of the intervention and the name of the Para-Professional.
Provides feedback and information for the QMHS or Therapist pertaining to the recipient's needs for casemanagement services; informs the recipient about benefits, community resources, and services; assists the recipient in accessing benefits and services; arranges for the recipient to receive benefits and services; and monitors the provision of services.
Participates in regular treatment team meetings to discuss clients; immediately reports any concerns to the QMHS or Therapist as it pertains to the welfare of the client and/or services rendered.
Performs other duties as assigned.
Completes all necessary documentation to service delivery. Provides written communication for necessary services inside and outside the Agency

What You'll Do

* Ensure foster parents, youth and casemanager needs are met during periods of staff transition.
* May require rotating on-call schedule.
* Participate in performance quality improvement process and execute any assigned tasks.
* Successful completion of all required training pertaining to job.

What You Will Do:
* Under the supervision of the Program Coordinator, provides community-based services to youth and adults with mental health and substance use disorders.

What You Will Do:
* Under the supervision of the Program Coordinator, provides community-based services to youth and adults with mental health and substance use disorders.
* Provides the scope of mental health and substance use related casemanagement services including, but not limited to: ongoing assessment of needs; skills to facilitate personal independence and daily living skills; coordination of the ITP including services identified within, linkage with natural support and formal community support service/systems; symptom monitoring; advocacy and outreach; interventions for symptom management; activities to report recovery including integration with self-help programs, etc.
* Provides crisis support and stabilization activities including de-escalating persons in crisis, assisting therapists with facilitating hospitalizations for clients when indicated, actively assisting with safety planning, and coordinating with treatment team and afterhours services as need.
* Provides Day Treatment services to adults as assigned by Program Coordinator.
* The scope and volume of duties as well as target populations served will vary based upon site/department needs and will be assigned by respective Program Coordinator.
* Renders all services within department guidelines and in compliance with rules/standards set forth by Medicaid/Medicare, HFAP, OhioMHAS, and CARF.
* Demonstrates a commitment to the provision of quality care through engagement in professional growth and agency-sponsored learning opportunities; utilizes best practices as appropriate to the conditions being treated; and provides all services in a manner that instills hope and meets patients where they are at in terms of their readiness for change. Uses best practices for patient engagement and retention throughout the episode of care.
* Performs tasks that provide for patients' safety including ongoing screening and documentation of risk factors; develops safety plans and crisis management plans when indicated; and coordinates with other members of the treatment team and supervisor as needed to promote patient care. Understands the role of accurate and timely clinical documentation in relation to quality care and patient safety.
* Promotes patient recovery through active and collaborative patient involvement in initial and ongoing treatment planning. Fosters recovery and independence by identifying and
* incorporating patients' strengths and abilities into the treatment process. Involves natural support persons, self-help resources, and other community integration activities as critical components to long-term recovery.
* Provides assertive outreach when clients are not engaging into treatment services. Extensive efforts are made to outreach clients who are identified to be at-risk if not involved in treatment services.
* Understands the role of Patient Care Standards (PCS) as a measure of our commitment to serving those in need and ensuring that access to care is provided. Ensures PCS standards are met and actively utilizes and responds to strategies provided through supervision when there are challenges in meeting these standards.
* Performs tasks necessary to support revenue including timely and accurate documentation within departmental guidelines; and maintaining current treatment plans. Performs tasks related to grants or other special projects as assigned.
* Is able to effectively and efficiently utilize EMR and video-conferencing software. Utilizes work email on a daily basis keeping current with FRMC and departmental communications. For new hires, these skills are in place by the end of the probationary period. Ensures all electronic forms of communication, that include PHI, are compliant with HIPAA and 42 CFR laws regarding confidentiality.
* Positively contributes to patient, referral sources, and employee satisfaction. Is a team player who is flexible, willing to assist team members, and positively influences team morale. Actively participates in and follows through with strategic planning, performance improvement and quality initiatives.
* Regularly demonstrates the ability to all behavioral health patients of all ages and of all cultural backgrounds in the routine performance of duties.
What You Will Need:
* Bachelor degree in psychology, social work, or related field required. Experience working with persons with mental health or substance use problems preferred.
* Heart Saver, Aegis Crisis Prevention, and First Aid certification within 6 months of hire.
* Ability to effectively understand and communicate in both written and oral form. Sensitive to the cultural needs of the individual and/or family served.
* Punctual attendance at assigned work location is required.
* Ability to be mobile and possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
