ESSENTIAL FUNCTIONS
Conducts agency orientation in person or by phone with potential families for foster licensing or kinship placement; follows orientation with screening interview of interested applicants in person; provides general information and initial foster care application packet.
Assesses the family and home environment for appropriate placement through the home assessment process. Complete the required assessment report to include recommendations.
Ensures foster families complete the required training (Foster Parent College) for licensing, helps identify community resources that will meet the advanced training requirement.
Remains in contact with families during the application and pre-service training process; provides support, assistance, and assures that prospective foster parents progress through each step towards licensing in a timely manner.

Responsibilities
What You'll Do:
The CaseManager is responsible for managing the care of individuals, including both youth (adolescents) and adults, who are in crisis and stabilizing from acute episodes. This role involves facilitating admission intakes, treatment planning, discharge planning, obtaining collateral, assisting in the legal process, and initiating personalized care plans. The CaseManager works closely with an Interdisciplinary Treatment Team to coordinate care and discharge planning for both youth and adult individuals, their families, schools (for youth), local providers, and other referral sources. * Collaborates with the Crisis Therapist to develop and implement an individualized person-centered Treatment Plan that is appropriate for brief intervention. Completes the signed Treatment Plan and reviews with each patient and any other involved parties.
* Gathers collateral from the patient and involved parties to provide a more thorough assessment of what the BHR's immediate needs are for treatment and discharge.
* Actively coordinates care for patients to ensure services are coordinated with facility staff. Identifies and addresses gaps in service needs for participating parties in service delivery to the individual and makes appropriate recommendations to meet individuals needs upon discharge.
* Collaborates with Interdisciplinary treatment team to engage, monitor, and communicate with the individual ensuring that safety is maintained while meeting the individual's needs.

The CaseManager I for La Jornada is responsible for the completion and submission of timely assessments and service plans, ensuring document uploads into ORR's UC Portal and maintaining required comprehensive case files compliant with ORR Policy and NYAP's CQI Team. RESPONSIBILITIES
The CaseManager I will perform duties including, but not limited to:

Ensure casemanagement assessments are completed within ORR's allotted timeframes.
Conduct on-site admission and initial intake interviews of youth to include gathering familial, possible sponsorship information and to establish age of the youth.
Conduct interviews of sponsors/family members, friends of family to vet sponsor's ability to care for the minor(s).
Act as UC case POC for assigned Federal Field Specialist, Case Coordinator and Contract Field Specialist.
Prepare case for, conduct and lead pre staffing process with multidisciplinary team of professionals including casemanagers, clinical counselors, medical coordinators and other designated stakeholders.
Attend staffing(s) with Case Coordinators, Federal Field Specialists and other ORR stakeholders as needed.
Coordinate with local pro bono attorneys for the timely provision of “Know Your Rights” presentations and legal screenings to children in care.
Work with program administration, clinical, medical and educational staff in identifying best casemanagement practices while maintaining a collaborative multi-disciplinary environment.
Work to ensure children in care are provided a safe environment and safe and timely release from ORR care pursuant to ORR MAP Section 2; this may include completing online address searches, obtaining birth certificates to prove relationship, income verifications, background checks, and other actions to ensure proper vetting of the Sponsor, household members and adult caregivers.
Document all actions taken and contacts with youth, sponsor, and stakeholders in the form of progress notes as required by NYAP.
Complete and submit reunification packets and Release Requests for initial review to Lead CaseManager or Program designee.
Submit completed reunification packet with appropriate referral made by CaseManager for the timely release of youth to designated sponsor, including referrals for home studies and post release services (PRS).
Provide weekly face to face updates to youth and telephonic updates to family members/sponsor.
Ensure the provision of two weekly telephonic contacts with family in the US or COO, primary caregiver and/or sponsor.
Facilitate incoming calls to minors with the appropriate family members and other approved caregivers.
Facilitate attorney to client contact as requested by youth.
Coordinate weekly treatment team meetings with representatives from all departments at the Program.
Establish and maintain a strong relationship with assigned foster parents, attend meetings with foster parents and act as program liaison with foster caregivers.
Drive children to facilitate program services, may include transportation to court appointments, attorney visits, other appointments as needed per contractual duties; this may also include transporting youth within the United States for reunification purposes.
Coordinate casemanagement and family reunification services for children, including active involvement in discharge planning.
Actively participate in documenting safety plans and Post 18 age out plans.
Oversee and/ or actively participate in the process of reporting significant incidents (SIRs) in accordance with existing policies and procedures.
Ensure maintenance of UC electronic and physical files, including uploading documents in UC Portal, Evolve, and maintaining the corresponding physical file.
Perform other duties as assigned.
MINIMUM QUALIFICATIONS
Bachelor's degree required in Social Work, Psychology, Human Services, Counseling or other social service field. One (1) year experience preferred working with children and adolescents or in the youth services field. (volunteer and internship experienced included).

Qualifications
License or Certification:
Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required for one's discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM or ACM™ certification upon entry into this position OR within two years of entry into the position.
CCM or ACM™ certification required OR must be obtained within two years of being placed in the CaseManager II position.
Minimum Qualifications:
For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.
For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor's degree and graduate degree is preferred.
2 years of rehabilitation experience preferred.

What You'll Do:
The CaseManager is responsible for managing the care of individuals, including both youth (adolescents) and adults, who are in crisis and stabilizing from acute episodes. This role involves facilitating admission intakes, treatment planning, discharge planning, obtaining collateral, assisting in the legal process, and initiating personalized care plans. The CaseManager works closely with an Interdisciplinary Treatment Team to coordinate care and discharge planning for both youth and adult individuals, their families, schools (for youth), local providers, and other referral sources. This position also carries a caseload and serves as an ambassador to community partners.
Collaborates with the Crisis Therapist to develop and implement an individualized person-centered Treatment Plan that is appropriate for brief intervention. Completes the signed Treatment Plan and reviews with each patient and any other involved parties.
Gathers collateral from the patient and involved parties to provide a more thorough assessment of what the BHR's immediate needs are for treatment and discharge.
Actively coordinates care for patients to ensure services are coordinated with facility staff. Identifies and addresses gaps in service needs for participating parties in service delivery to the individual and makes appropriate recommendations to meet individuals needs upon discharge.
Collaborates with Interdisciplinary treatment team to engage, monitor, and communicate with the individual ensuring that safety is maintained while meeting the individual's needs.
Maintains the clinic record, including documentation of activities performed as part of the service delivery process. Obtain signatures from patient regarding treatment, as necessary. Documents all services and patient activities in the medical record regarding brief interventions, coordination of care, discharge planning, treatment planning, etc.
Maintains a working knowledge of the Title 36 Involuntary Commitment process. Monitor the petition process to ensure timeliness is adhered to, and forms are complete and thorough. When subpoenaed, attends COE hearings.
Maintains a working knowledge of the Maricopa County Regional Behavioral Health Authority (RBHA) system and potential funding sources.
Participates in individual and group supervision as required. Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency. Demonstrates competency through post-testing, skill observation, and performance as assessed by direct supervisor.

JOB SUMMARY:
As a Field Medical CaseManager at ForzaCare, you'll help injured workers navigate their recovery and return to work. You'll act as the central point of coordination, connecting the injured worker, medical providers, employers, and insurance carriers to ensure timely, transparent, and effective care management.
This role is ideal for licensed nurses or certified rehabilitation counselors. While prior experience in workers' compensation is strongly preferred, those who have it will find their background especially valuable in this role.
KEY RESPONSIBILITIES:
Coordinate care between medical providers, employers, insurance carriers, and injured workers.
Attend appointments with the injured workers, which may include visiting employers and injured workers at their place of employment.
Develop, document, and monitor individualized recovery goals and return-to-work plans.
Provide consistent communication and detailed progress reports to clients and stakeholders.
Ensure all casemanagement work meets or exceeds customer and compliance requirements.
Build and maintain strong relationships with clients, providers, and internal team members.
QUALIFICATIONS:
Education & Licensure:
Active Registered Nurse (RN) or Certified Rehabilitation Counselor (CRC) license with associated college degree is required.
Additional certifications such as CCM, CIRS, or other casemanagement credentials are preferred.
Must comply with all state-specific licensure and certification requirements.
Prior experience in workers' compensation casemanagement is strongly preferred.
Valid driver's license, reliable transportation, and auto insurance with ability to travel to appointments.

Scope of Work: The Housing CaseManager Coordinator (HCMC) provides comprehensive casemanagement services to clients living with HIV/AIDS and supervises a team of casemanagers who provide these services. The House CaseManager Coordinator reports to the Housing Services Manager and
works with a broad spectrum of people living with HIV/AIDS, works with clients' families, and assists clients with accessing medical care, housing through Section 8 Voucher Programs, and connecting with community resources. The Housing CaseManager Coordinator position is a full-time exempt position. Duties include but are not limited to:
Functional Responsibilities:
Supervises a team of Housing CaseManagers (HCM) in the Ryan White Part B program and:
Works with the HCMs to address and coordinate daily client issues for clients in service or at risk for leaving service.
Works with the Housing Services Manager (HSM) to address client crisis and/or high-level clinical issues.
Is responsible for training, and supervising the Housing CaseManagement team, in coordination with the HSM and Associate Director of Care and Housing.
Is responsible for daily program implementation and coordination of housing casemanagement services.
Carries a case load that provides a range of client-centered services to support persons living with HIV/AIDS in obtaining or maintaining primary medical care and housing including:
Initial assessment of service needs including Self-Sufficiency Matrix and Acuity Scale.
Complete the Vulnerability Index- Service Prioritization Decision Assistance Tool (VI-SPDAT) for clients requesting housing.
Development of a comprehensive, individualized service plan.
Coordination of services required to implement the plan.
Client monitoring to assess the efficacy of the plan.
Periodic re-evaluation and adaptation of the plan as necessary; and
Client-specific advocacy and/or review of utilization of services.
In collaboration with the client, develop a goal plan, which outlines goals that address transition needs for potential housing program clients and long-term housing stability needs of housing program clients in their residence.
Actively participate in weekly group supervision meetings.
Conduct home visits to assess and support clients and/or identify unmet needs and institutional visits as applicable.
Attend and participate in treatment team meetings with medical providers and network with other agencies and community service providers to secure appropriate, quality services for agency clients.
Monitor and note medication adherence and treatment compliance.
Maintain timely and accurate documentation of all client contact.
Serve as member of HIV Housing Department, working in close coordination with the medical casemanagement and housing teams.
Attend agency staff meetings.
Comply with SAAF employment and quality assurance policies and procedures.
Qualifications
Minimum Qualifications:
Minimum of two years prior administration experience in social services, housing, or property management related field.
Minimum three (3) years of Management experience, managing a team of five (5) or more with paid staff and/or volunteers.
Appropriate fingerprint clearance through the Arizona Department of Public Safety (paid for by SAAF).
Health-related field or 2 years' work experience in social services related field.
Proficiency with Microsoft Office.
Must have access to reliable transportation, a valid Arizona Driver License, current insurance and a driving record that will be supported by our liability insurance provider.
Preferred Qualifications:
Knowledge of HIV/AIDS issues.
Three years' casemanagement experience working with people with substance abuse and/or mental health issues.
Knowledge of supportive housing programs and issues related to homelessness.
Previous experience in a team leader or supervisor role.
Experience working with Tucson human service providers.
Effective oral and written communication skills.
Proficiency with database systems.
Strong organizational skills.
Capable of problem solving and working independently.
Excellent interpersonal skills, sensitivity to cultural and personal diversity.
Bilingual (English/Spanish).
Working Conditions/Physical Demands:
Operate in a professional office environment and requires the ability to sit for long periods of time with intermittent walking, standing, bending, squatting, pulling, and pushing. Requires the ability to use standard office equipment such as scanners, computers, keyboards, phones, shredders, photocopiers, and filing cabinets. Requires travelling on occasion as needed between SAAF campuses and other locations.

This job is a sedentary job, as defined by the Social Security Administration, CFR.
