    The RN Case Manager provides patient care management across the continuum of care. The RN Case Manager is responsible for ongoing assessment of patient needs and coordination of the delivery of patient care through the unit manager, Social Worker, and the multi-disciplinary health care team. The RN Case Manager is accountable to ensure patients meet criteria for admission, continued stay, and discharge. They focus on facilitating the development of discharge plans and the appropriate movement of patients from one level of care to another. In collaboration with the Social Workers, they insure a smooth transition to other levels of care. The RN Case Managers function as a liaison with insurers to advocate for patients based on defined needs. There is a significant focus on patient care outcomes and the RN Case Manager oversees the evaluation of outcomes (treatment and financial outcomes). The practice of the RN Case Manager is highly collaborative with physicians and other health care team members. They are involved in quality improvement utilizing trended data to focus on issues identified for improvement. RN Case Managers are teachers to patient/families and members of the multi-disciplinary team. RN Case Managers are accountable for the training related to the principles of care and case management. They assist the Social Worker with the complicated medical issues. The RN Case Manager is responsible for completing Interqual Reviews for initial and concurrent reviews. The Human Trafficking Protective Order Advocate will be primary located at the Family Safety Center in Downtown Tulsa. This position will assist survivors of human trafficking with protective orders and other court related matters. Primary job responsibilities include assisting human trafficking survivors in filing for protective orders, providing court advocacy, connecting survivors with community resources, and performing follow-up and clerical responsibilities. The Human Trafficking Protective Order Advocate will have enough flexibility to serve as a resource to other DVIS programs that encounter survivors of human trafficking. Responsible for attending community outreach regarding human trafficking. Priority 3: Collaboration Assist clients with protective order filings. Conduct initial assessments for clients, matching their needs with appropriate services. Utilize the Oklahoma Supreme Court Network for legal matters with client or harm doer. Aid clients in completing emergency protective order paperwork and explain court procedures. Collaborate with clients to assess risks and goals, incorporating this into Safety Plans. Make referrals to community agencies as necessary. Maintain client document files according to DVIS guidelines. Compile daily statistics for reports to funding sources and courts. Work both independently and in team settings. Perform additional assigned duties. Other Duties/Responsibilities: Other assistance to support the overall goals of the department and organization.
