Job Summary: The Case Manager is responsible for providing comprehensive case management services to clients, including but not limited to intake assessments, benefit assessments, goal setting, case plan development, home visits, progress monitoring via standardized testing tools, client advocacy, and referrals while maintaining compliance with established state and federal standards and requirements.
Essential Functions:

4. Attend training as assigned by the program director.

6. Compile, prepare, submit, and maintain accurate records, files, forms, and additional information in accordance with agency policies, licensing, and/or contract requirements.
7. Participate in scheduled meetings and team decisions and operations.
8. Participate in workshops, seminars, education programs, and activities promoting professional development.
9. Obtain and enroll in continuing education/training required for recertification(s).
10. Maintain BCFS HHS professional and ethical standards of conduct outlined in the employee handbook, including demonstrating respect for agency staff, children, and community members and complying with the required dress code, at all times.
11. Provide crisis intervention according to training provided by BCFS HHS when needed to maintain a safe environment.
Measurable Deliverables:
1. Maintain a client caseload of 35-40.
2. Maintain an updated client roster to be sent to program management weekly.
3. Document client visits, interactions, outcomes, and education provided to participants in Salesforce within two business days.
4. Conduct routine client contacts based on requirements.
5. Conduct routine in-office and/or virtual visits based on requirements.
6. Meet all federal and state regulatory guidelines and standards applicable to this position.
7. Meet all deadlines by the program director and federal partners.
8. Increase client's knowledge and positive changes in attitude as measured by standardized tests.
9. Demonstrate the ability to:
a. Respond sensitively and competently to the service population's cultural and socio-economic characteristics.
b. Communicate effectively in writing and verbally in English.
c. Work in a fast-paced environment.
d. Maintain computer literacy required to meet the responsibilities of the position.
e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
f. Travel as needed.
10. Other job duties as assigned per grant requirements.
Requirements:
1. Maintain certificates required for the position including but not limited to First Aid and CPR.
2. Valid Texas Driver's license with a clear driving record and reliable transportation.
3. Pass a pre-employment drug screen and random drug screens.
4. Provide proof or work eligibility status upon request.
5. Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years).

Provide training to the patient and as appropriate, all others involved in that patient's rehabilitation. Analyze and document the result of any training.
Perform a thorough evaluation, assessing the needs of the patient, and set measurable objectives that are formulated in conjunction with the rehabilitation team. Adjust treatment to achieve maximum results.
Qualifications:
Masters Degree in Rehabilitation Counseling or the equivalent plus a minimum of two years vocational counseling experience with individuals with neurological impairments is required.
Knowledge of complex neurologic injury and orthopedic rehabilitation techniques generally required.
Maintain professional license and/or certification as required by the state in which the therapist practices
Good organizational and communication skills are needed.
Ability to function with minimum supervision.
Reliable transportation is required.
