    Full-time Description Schedule: Full Time: Monday- Friday 8:00 am- 4:30 p.m. and one rotating Saturday every 4 to 6 weeks Provides adequate contact with clients in the apartment program to ensure the health and safety of residents and to ensure appropriate support is given for other community services. ESSENTIAL DUTIES AND RESPONSIBILITIES Immediately reports maintenance concerns/issues to the Apartment Program Director. Assists in the provision for the physical needs of each resident by the following means: assist and monitor each resident with obtaining adequate food, clothing and shelter, monitors the self-administration of medication by residents and the adherence to physician's instructions. Assist/Instruct residents with how to properly store medications and valuables. Reports to the supervisor all pertinent information concerning illnesses, accidents, untoward events, staff difficulties and repair needs. Monitors food, supplies and upkeep in the apartment helping the resident make appropriate purchases or assisting in the cleaning whenever necessary. Develops and monitors service plans with individual residents using principles of Psychiatric Rehabilitation. Assists in the delivery of restorative services as designated on the service plan. Meet consistently with each resident to provide one-to-one support. Provides transportation for residents to other needed services with the assistance of other staff. Organizes therapeutic activities which include socialization (discussion groups) and recreational opportunities (sports, games, arts and crafts, music). Assist with resident relocation including moving of personal items and cleaning of previous rental site. Resource Management: Obtains a thorough knowledge of appropriate community resources, serves as an advocate for residents in securing needed services from other agencies, refers residents to appropriate social, rehabilitative, medical, mental health and vocational services. Case Documentation: Evaluates each resident's growth potential by utilizing the needs assessment tool and the consumer's self-assessment, Gains input from the team in order to develop service plan goals, documents case records and progress notes, obtains ongoing required medical records, assist with completing chart audits as assigned.     Summary/Objective: CHJC's Family Permanency Program (FPP) is funded through the New York State Office of Children and Family Services and serves families in Jefferson, Lewis, Oswego and St. Lawrence Counties. The target population is exclusively families who have finalized the adoption of their child(ren), families with legal guardianship of their child(ren), with or without a KinGap agreement, and families with formal or informal custody agreements. The program model is trauma-informed, child-centered, and family-focused, and culturally responsive. The model takes a strength-based approach to family engagement and is non-judgmental and affirming. Program services are evidence-based, easily accessible, as well as flexible and adaptable to the family's needs. The program model is designed to: · Promote permanency for post-adoptive, post-guardianship, and kinship families. · Provide assistance to families so children may be cared for in their own homes with their adoptive parent(s), legal guardian(s), or kinship caregiver(s); and · Strengthen post-adoptive, post-guardianship, and kinship families to avoid foster care or other out-of-home placements. The FES works as a member of the treatment team to deliver high intensity case management to assigned youth and families. Essential Functions: 1. Assists in gathering essential documentation for program enrollment 2. Collaborates with clinical staff to completes required assessments and work with the family to create treatment plans/goals 3.     This position is associated with Fort Drum, NY. Candidates must be local or willing to relocate to become local and licensed at the independent level as a Social Worker (LICSW, LCSW-R or LICSW-CP), Mental Health Counselor (LPC or LMHC), or Therapist (LMFT) or equivalent independent licensure. Provides the full breadth of MFLC consultation and counseling services to the command structure and military service members and their families as an embedded counselor assigned to a dedicated military unit. Responsible for providing MFLC services including non-medical, short-term, walk-around counseling, training/health and wellness presentations, and consultation to the assigned brigade/regiments command regarding behavioral health issues with an understanding of the military unit, cultural and mission. Works closely with the Regional Supervisor/Regional Director, installation, and military branch Points of Contact (POC) to assure that the program is provided within scope and meets the needs of the installation. Provides non-medical, short-term, solution focused, walk-around counseling directly to eligible service and family members at assigned primary and secondary locations of a dedicated military unit (brigade, wing, or regiment) as an embedded counselor. Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated. Delivers training and health and wellness presentations and activities as requested/ directed. Enters counselor activity data daily through smart phone or web application to assure that reporting is accurate from assigned installation, while maintaining client / service member confidentiality. Cultivates and maintains direct relationships with commanders and personnel within the assigned military unit, utilizing relationship management skills and recognizing the unique structure and mission of the assigned military unit. Responsible for development of an effective and professional working relationship with the installation POC (Point of Contact), command, and the command POC. Consistently visible within the brigade/regiment which allows MFLC services to be accessible. Establish and maintains working relationships with community resources and military populations. Partners with POC to provide MFLC services in a manner that addresses the needs of the brigade/regiment and maintains confidentiality. Provides MFLC response to critical incidents, surge, and special requests as directed by the POC and approved by the Office of Secretary of Defense (OSD) program manager. Counselor will flex schedule to respond to urgent requests as needed or at the request of the Regional Supervisor/Regional Director. Manages duty to warn and restricted reporting situations according to Department of Defense (DoD) protocol and staffs the cases with Regional Supervisor/Regional Director. Communicates with Regional Supervisors and participates in regular individual and group supervision, sharing information regarding trends and issues on the installations to which the counselor is assigned. Participates in regular in-services/training, Quality Improvement committees or other contract activities as assigned/appropriate. Participates in initiatives, studies, and pilot programs as directed by the customer and/or Magellan. This includes participation in pilot program certification and training processes, completion of activity documentation, integration of pilot activities in non-medical counseling work, and the utilization of applicable technology to complete required activities. Works in an office setting with a controlled temperature environment. Occasional exposure to inclement weather conditions may occur depending upon assignments. Requirements Required Education and Experience: Bachelor's Degree in a Human Services field, with at least two years relevant experience in community-based mental health or case management is required. As this program is seeking applicants possessing a Master's Degree in a clinical field (i.e., Mental Health Counseling or Social Work), those possessing a Master's Degree and/or those as licensed clinicians will receive preference. Skills/Abilities/Knowledge: Strong organizational abilities and office skills are essential for this position. This person must also have excellent people skills and be a team player. Ability to work with diverse populations. Must demonstrate effective and excellent oral and written communication skills and the ability to work cooperatively with others. Must be detail-oriented and possess excellent time-management skills. Requires self-direction, tact, diplomacy, and the ability to be clear, courteous, and professional. Able to react to change productively and handle other duties as assigned. Additional Eligibility Qualifications: Must maintain privacy of confidential information. Successful completion of routine pre-employment requirements as mandated by the New York State Department of Health, Office of Mental Health and Office of Children and Family Services along with ongoing clearances and exclusion checks. These requirements are to include employment health requirements, reference checks, criminal background checks/fingerprinting, State Central Registry clearance and your eligibility to work in the United States. A valid NYS driver's license required. Corporate Compliance, HIPAA Privacy Security, and Workplace Harassment and Discrimination Training and Nonviolent Crisis Intervention, First Aid and CPR will be required following date of hire.
