This individual provides ongoing community psychiatric support treatment services for a select group of clients who have a serious and/or persistent mental illness co-occurring with substance abuse disorder. This Case Manager will provide training, assistance, and support to help this client receive wrap-around rehabilitation services in the community. Our ACT Teams work together, meeting frequently, to facilitate an integrated, multidisciplinary team approach to service regarding progress, general problems, and concerns pertaining to clients served and their community adjustment. What You'll Do: Collaborate with the client and the Service Coordinator to develop treatment plans that shall include services and assistance necessary to achieve recovery goals, including an explanation of the availability, intensity, and duration of each service. Implement programming according to the developed treatment plan, participating in the ongoing assessment and therapeutic engagement of ACT team clients. Make observations and document progress in accordance with established agency procedures. Assist clients in developing daily living skills, including teaching and assistance with medically necessary services such as laundry, cooking, cleaning, communication and social skills, recreation, grocery shopping, personal hygiene, and self-care skills. Monitor medications in accordance with agency procedures to ensure that proper dosages are taken as prescribed. Assist clients in the proper budgeting and use of their funds, acting as an advocate for clients to secure needed services and financial entitlements.
