Summary:
The Physical Therapy Assistant (PTA) works under the supervision of a Physical Therapist to help implement treatment plans that improve patients' mobility, reduce pain, and enhance functional independence. Essential Work Functions:
Set up therapy equipment and treatment areas, ensuring all devices and supplies are calibrated, clean, and ready for patient sessions
Guide patients through prescribed exercises and activities to improve mobility, strength, and balance, while providing hands-on assistance with movements and transfers as needed
Observe patient performance during therapy sessions, note changes in functional abilities, and report progress or concerns to the supervising Physical Therapist
Accurately record patient treatment details, progress, and any adjustments made during therapy sessions in the medical record
Instruct patients and their caregivers on home exercise programs, proper use of assistive devices, and techniques for safe mobility and injury prevention
Work in conjunction with physical therapists, occupational therapists, and other healthcare professionals to coordinate a comprehensive approach to patient care
Maintain a safe treatment environment by adhering to infection control protocols, safety guidelines, and regulatory standards while addressing patients' comfort needs during therapy sessions
Engage in quality assurance initiatives and professional development activities to remain current with best practices in physical therapy
Collaborate in discharge planning to ensure smooth transitions and continued patient progress in community or home-based settings
Performs other duties as assigned within scope of practice
Required Essential Skills:
Critical thinking, service excellence, and good interpersonal communication skills
The ability to read, write, and communicate in the English language
Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills
Physical Abilities - Must be able to remain in a stationary position, move about, move equipment (50-100lbs), push, pull, and bend
Interpersonal Skills - Must be able to work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and professionalism
Technical/Motor Skills - Must have the ability to grasp, perform fine manipulation, push/pull, and move about when assisting with procedures and/or using department equipment
Mental Requirements - Must be able to cope with frequent contact with the general public and customers while meeting deadlines under pressure. Must be able to work under close supervision occasionally, as well as working without assistance from other personnel. Must be able to contend with irregular activity schedules occasionally and continuous concentration to detail

SUMMARY
This position is responsible for all functions of case management including advocacy, coordination assessment, linking, and monitoring of services for an assigned caseload of individuals with a primary diagnosis of developmental disability or mental illness. Settings include the CMH office, home, and/or the community. This position will perform the following duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
Gathers and Reviews information such as medical, psychological, and social history contributing to the individual's situation. Evaluates this information to determine the individual's needs and capabilities towards the least restrictive environment and/or services.
Initiates and coordinates professional assessments according to the presenting problem and relevant standards and is responsible for sharing and presenting assessment/treatment data as authorized and required for implementation and coordination with other agencies, courts, and caretakers.
Completes the person-centered plan of service and periodic review of progress and assures that the preferences of the individual served are reflected in the individual plan of treatment. Makes direct/indirect contacts in accordance with the person-centered plan with the assigned individuals, develops short- and long-term goals related to the needs presented and is responsible for assuring the implementation, coordination, and monitoring of the person-centered plan of service in residential, employment, school, and other settings.
Serves as the primary advocate for assigned individuals, and as the primary liaison with their families, guardians, caretaker and/or their agencies involved in their service network.
Provides consultation, referral and in-services to staff, caretakers, families, and other agencies as needed to meet the specialized needs of individuals served.
Participates in and assists with placement planning and completes necessary forms and paperwork for placement. For individuals requiring specialized, contracted, and other placement services, the care manager completes and coordinates a residential treatment plan and visits the home as required to monitor progress in the placement setting.
Provide community outreach to individuals who have a history of hospital recidivism in order to be proactive in preventing crisis situations
The care manager participates in after-hours on-call participation, crisis intervention during the workday, and other duties as assigned.
Participates in evidence-based practices as assigned (DBT/IDDT/FPE/IPS etc.).
Provides care management services to veterans and their families, as assigned.
Staff will complete appropriate OBRA Assessments and/or provide OBRA coordination as directed by supervisor.
Provide assistance with carrying out Grant requirements as needed.
Provide linking and coordinating to ensure Integrated Health Care needs of individuals are addressed.

Upon completion of Recruit Training, BMs attend a four-week A school in Great Lakes, IL, where they learn basic general safety, watch standing, deck seamanship, underway replenishment, and preventive maintenance. Selected BMs may receive additional amphibious
training depending on future assignments. Promotion opportunities are above average compared to other Navy ratings.
Education Opportunities
BMs can earn college credits through the American Council on Education for courses taken in this rating. Additionally, the United States Naval Community College (USNCC) offers degree
programs that support professional development and educational advancement.

GENERAL RESPONSIBILITIES:
Implement a quality health care program that is safe, promotes good and healthy practices, and meets state laws.
SPECIFIC RESPONSIBILITIES:
Inventory and stock all necessary first aid supplies.
Enter all injuries, first aid treatments, and medication in the bound camp medical log.
Report all serious injuries or diseases to the director and to the state (as required by law)
Distribute all medication according to the prescription
Handle all emergencies according to the emergency medical procedures posted in the nurses' station.
Keep all drugs and supplies locked up
Make purchases as budgeted and directed
Follow procedures to care for bodily fluid clean up
Various other duties as assigned by Sponsored Ministries Manager
Minister to campers and staff physical, emotional and spiritual needs through relationships
