Salary: $51,200 Catholic Charities of Baltimore Esperanza Center is currently seeking a Case Manager (Bilingual English/Spanish) who provides intensive case management including, but not limited to, as-needed home visitation services to participants / clients. The Case Manager assesses household needs, coordinates resources based on goals, and provides advocacy, information, and referral services. The work schedule is Monday - Friday 8:30am-4:30pm business hours 37.5 hours per week- This is an onsite position only.
The Esperanza Center provides a welcoming atmosphere, social services, health services, legal support and English as a Second Language (ESL) to immigrants in the Baltimore metropolitan area. The center's staff offers assistance with employment, English proficiency, legal services, and access to primary and urgent health care and referral services. The health services are offered in partnerships with area hospitals and medical schools.
This position is supported by grant funding, and the responsibilities associated with the role are governed by the terms and conditions of the grant. Employment may depend on the continuation or termination of the funding.
JOB DUTIES & RESPONSIBILITIES:
* Develops Individualized Service Plans (ISPs) in collaboration with participants, with a clear focus on achieving self-sufficiency within the established timeframe, and tracks progress to ensure timely goal completion.
* Maintains consistent contact and conducts required visits to evaluate progress, address challenges, and ensure participant engagement and compliance with the ISP.
* Conducts thorough self-sufficiency assessments at intake and service closure using the Arizona Self-Sufficiency Matrix, and accurately documents and communicates results to inform service planning.
* Advocates effectively for individuals, families, and children, securing needed services within the community and following up to confirm support is received.
* Collaborates with staff and volunteers to actively engage clients in programs that support their path to self-sufficiency, and takes initiative to ensure service alignment with individual goals.
* Maintains accurate and up-to-date case files, enters data into the Service Point database in a timely manner, and ensures data integrity to support reporting and analysis.
* Collects, inputs, and reports data in accordance with program requirements, and keeps supervisors informed of trends, issues, and participant outcomes as appropriate.
* Maintains a working knowledge of program services offered and appropriately connects clients to internal resources, reinforcing comprehensive support toward self-sufficiency.
* Perform other duties as assigned.
EDUCATION & EXPERIENCE REQUIREMENTS:
* Bachelor's degree in Human Services, Social Work, or related field.
* Two years related experience.
* Excellent knowledge of case management and crisis intervention services
* An equivalent combination of education and experience may be considered.

What you will Do:
Provide individual, group, post discharge follow up behavioral health therapy that includes comprehensive biopsychosocial assessments, individualized treatment plans using SMART goals, timely clinical documentation (e.g., SOAP notes), and tracking progress of each goal to completion.
Directly or in collaboration with other staff members, provide crisis intervention and de-escalation for members presenting in a crisis situation (via telephone or in person), including suicidality, disruptive and/or threatening behaviors, and disposition as clinically appropriate.
Serve as the clinical behavioral health SME within the interdisciplinary care team (ICT) by providing recommendations, referrals, and follow up for behavioral health aspects of member care.
Provide and advocate for clinical care using evidence-based approaches including trauma informed care, harm-reduction, motivational interviewing, cognitive-behavioral therapies, etc.; may include providing short-term solutions-based therapy or emotional management (e.g., anger management)

Requirements:
Must be clinically licensed in the state of practice with 3+ years of experience in behavioral health (e.g., social work, mental health, or addiction).
Acceptable licenses include: LCSW-C, LPC, LPCC, LCPC, LMHC
At least 2 years of experience in settings like community mental health centers, FQHCs, or inpatient psychiatric facilities working with clients who have complex behavioral health needs.
Current CPR certification required.
Comfortable providing both group and one-on-one therapy.

JOB SUMMARY
Reporting to the Rehabilitation Manager the Rehabilitation Coordinator is responsible for monitoring client progress, scheduling medical appointments, and providing clinical support services. RESPONSIBILITIES

Serves as a liaison and consultant to residential providers, mental health professionals, medical staff, community agency workers, family, care providers and others involved in program planning, conflict resolution and on-going clinical support, as directed.
Evaluates client performances and makes recommendations for referrals.
Participates in service delivery to promote client progress. Coordinates services with other community agencies and participates as required in joint planning, assessment, and service delivery.
Monitors overall client progress and prepares reports as directed. Communicates progress summary to appropriate staff and with client consent, other persons involved in client's treatment program.
Provides crisis intervention services as necessary.
Works with advisory groups and human service planning agencies to develop and improve service and integration opportunities for persons with chronic mental illness.
Establishes and monitors client records as required by regulating agencies.
Participates in the fee determination and billing processes and in the collection of fees.
Conducts and attends in-house, program, staff, and training meetings as required.

Summary: This position provides Homeless Prevention and Rapid Re-Housing services through the Supportive Services for Veteran Families Grant. Services are to be provided to low-income Veteran families that are at risk of becoming homeless or who currently are homeless. The Veterans First program follows a Housing First model and philosophy. The Case Manager will always utilize best practices and participant-centeredness. The Case Manager will assist Veteran Families in quickly maintaining or obtaining permanent housing, with a goal of achieving housing stability within 90 days. The Case Manager will work to empower low-income Veteran Families in DC, Arlington, Alexandria, Loudoun, Prince William, Fairfax, Montgomery, Frederick, Prince George's, Washington, Anne Arundel, Howard, Allegany, Carroll, and Garrett counties to become stably housed.
Essential Position Requirements and Competencies:
Provide prevention and rapid re-housing services to at least 50 low-income Veteran Families annually
Maintain services to a caseload of a minimum of 15 Veteran families at any time
Develop, implement and revise 90-day housing stability plan with Veteran Families
Case Management as it pertains to housing stability plan
Deliver face-to-face contacts to participants, when safe and appropriate
Foster wellness and independence
Engage participants and assess individual needs, preferences and strengths
Assist participants in locating and maintaining permanent housing
Make appropriate referrals to identified needed services
Act as a liaison with landlords and other community stakeholders
Collaborate with agencies providing services to participants, including community, government, and VA
Participate in multidisciplinary meetings and regular supervision
Maintain paper and electronic files
Maintain participants' electronic records in the Homeless Management Information System
Demonstrate empathy and flexibility in responding to participants' needs and requests.
Participate in multidisciplinary team meetings, supervision, and actively seek out professional development opportunities
Communicate professionally and clearly in all interactions-both written and verbal-to support seamless service delivery and collaboration
Demonstrate integrity, cultural responsiveness, and mission alignment in a fast-paced, equity-driven environment
Maintains compliance with all assigned mandatory training and background clearances

At least 1 year experience in case management, customer service, community resources provider or similar setting preferred :
Valid driver's license, insurance and ability to operate motor vehicle
Must have or be able to obtain a First Aid/CPR/AED certification that includes a hands-on skills testing module. Friendship Place will provide First Aid/CPR/AED training
Valid driver's license and current auto insurance
Excellent time management, detail-oriented, organized, capable of maintaining confidentiality, and able to work independently with minimal supervision
Understanding and/or experience working with Veterans
Experience with HMIS data entry preferred

Physical & Environmental Requirements: The role requires a balance of office-based tasks and fieldwork, including travel throughout the service area to meet with households, as well as participating in meetings and engagements with key stakeholders.
Employees must be able to sit for extended periods with repetitive motions and positions. The employee must be able to operate a motorized vehicle. Some lifting may be required (up to 20-25 lbs.) Other time will be spent in the office completing administrative paperwork and meeting with the supervisor.
Background Checks: The selected candidate will be required to pass a criminal history background check, including but not limited to, CFSA, NSO, previous employment and education verification, personal and professional references, and clearance from the Department of Motor Vehicles.

Specific Requirements: Positions requiring travel must have reliable access to personal transportation to fulfill job duties. The vehicle must meet all legal requirements, including valid registration and current auto insurance. Additionally, employees are required to notify the employer and provide updated documentation within a legally appropriate timeframe if there are any changes to their driver's license status or insurance coverage.

Compensation: The hiring pay range is between $58,117.00 - $63,506.00 annually. The starting annual salary is based on a step system. A candidate's starting pay determination is made based on meeting the basic position requirements, and current completed education, credentials, and work experience.
