Principal Responsibility
Fr. Alfred Center (FAC) provides a residential recovery program to homeless men who are ready to create sober and stable lives for themselves. The Fr. Alfred Center's year-long, abstinence-based program empowers men with no income or resources with the tools to overcome addiction and the support to establish productive and healthy lives. The holistic treatment offered through St. Anthony's multiple services is rare in the field of recovery, and allows for immediate assessment and thoughtful resolution of clients' medical, legal, vocational and educational needs, most or all of which have been affected profoundly by the cycles of poverty and addiction. Counselors conduct assessments, one-on-one counseling, educational and group counseling sessions to a caseload of up to 12 residents and provide general oversight and support services to all residents. Essential Duties and Responsibilities
Resident Engagement
Conducts in-depth, intake interviews and assessments of incoming residents assigned to caseload to determine their appropriateness for the program.
Provides individual counseling and case management for a regular caseload of approximately 10 to 12 residents.
Develops and updates clients' casework plans.
Assists residents in the development of their personal recovery plans.
Assesses dynamics that are harmful for residents' recovery, intervenes and counsels.
Monitors residents' compliance with program requirements, implements consequences as
Documents client benchmarks and other required information into Efforts to Outcomes (ETO), daily as required by the program.
Participates in the weekly case conference.
Conducts group sessions for residents (process groups, anger management group, etc.).
Assist with the caseloads of counselors who are absent (vacation, sick or other leave).
Collaborates with other St. Anthony programs providing services to the residents (the Dining Room, the Tenderloin Tech Lab, the Medical Clinic, Workforce Development and the Social Work Center) and with outside community agencies.
Oversees all resident activities and transportation at Fr. Alfred Center, monitors compliance with program and residence rules.

Minimum Qualifications
Valid Alcohol and Other Drug (AOD) counselor certification issued by California Consortium of Addiction Programs and Professionals (CCAPP) and the California Department of Health Care Services (DHCS). Two years current experience in chemical dependency field.
High School diploma/GED and three years' experience in social services, particularly in drug and alcohol residential programs.
Two years current experience in client caseload management and group counseling
Demonstrated knowledge of the disease and treatment of chemical dependency including 12-step work and social model programs.
Excellent interpersonal, listening, oral, and written communication skills are required.
Bilingual Spanish/English strongly preferred.

Summary of Duties:
The Resident Services Case Manager will be part of LTSC's Resident Services Department, providing comprehensive case management support to residents of Umeya, a 175-unit deeply affordable and permanent supportive housing community located at the intersection of Little Tokyo and Skid Row. This position focuses on serving residents in Umeya's general affordable units through direct outreach, individualized case management, job-readiness coaching, childcare referrals, and connection to community resources.
We are seeking a compassionate individual with experience supporting individuals and families through a trauma-informed lens.
On a day-to-day basis, the Case Manager will conduct regular outreach to residents, build trusting relationships, and collaborate with community partners to foster a supportive environment and strengthen access to services. The Case Manager will provide case management support including intake and assessment, goal setting, progress monitoring and reassessment, life-skills development, supportive counseling, benefits assistance, and referrals to appropriate resources.
The primary goal of this roles is to help residents achieve long-term stability by addressing immediate needs while supporting progress toward greater self-sufficiency.
Additionally, the Case Manager will work closely with the Umeya Resident Services Coordinator, the Director of Resident Services, as well as the other members of the Resident Services team, Community Organizing, Social Services, and Small Business Counseling teams to strengthen relationships with local stakeholders, including small businesses, institutions, and residents in Little Tokyo, to create meaningful community-building and engagement opportunities for Umeya residents.

POSITION SUMMARY: The TAY Case Manager provides intensive, weekly case management for transition age youth (TAY) ages 18-24 who experienced homelessness and are referred from San Francisco's Coordinated Entry System to First Place for Youth's TAY Rapid Re-Housing (RRH) Program. Through comprehensive case management, the TAY Case Manager helps youth to identify and attain goals in the areas of self-sufficiency, independent living, health and wellness, relationships, employment and education, and personal advocacy, with a keen focus on housing procurement, retention, and long-term stability. The Case Manager also provides community-based services throughout the community and at the youth's home to teach and model independent living skills development. This position works within a team of Housing Coordinators and a Resource Specialist ensuring housing procurement and connection to community resources. This position is required to work onsite and, in the community, primarily in San Francisco, Alameda and Contra Costa Counties, Monday - Friday.
Essential Duties and Responsibilities:
Caseload Management
Responsible for providing case management and skills development to participants
Conduct assessment, planning, and implementation of services for assigned participants to identify and accomplish short and long-term goals.
Provide direct participant support weekly including a minimum of bi-weekly face-to-face meetings with participants in response to participant needs.
Create and deliver impactful service plans for participants that focus on achieving outcomes in the areas of wellness, budgeting, long-term housing, education, employment, parenting, relationships and overall health.
Track participants' progress and develop creative ways for participants to practice and strengthen their independent living skills towards self-sufficiency.
Prepare clients to maintain housing after program exit or help them to identify alternative housing options if necessary.
Support participants to develop and practice social emotional learning skills that promote conflict resolution and decision making.
Work collaboratively with Resource Specialist to support participants assess career goals, work towards educational milestones, and obtain and retain employment towards living sustainable wage.
Coach participants on handling work-related issues, promote self-advocacy and increase success in job retention.
Identify, coordinate and form partnerships with critical community connections and resources that can help link participants with community connections, building networks, wellness and career opportunities.

Qualification and Requirements:

POSITION SUMMARY: The TAY Case Manager provides intensive, weekly case management for transition age youth (TAY) ages 18-24 who experienced homelessness and are referred from San Francisco's Coordinated Entry System to First Place for Youth's TAY Rapid Re-Housing (RRH) Program. Through comprehensive case management, the TAY Case Manager helps youth to identify and attain goals in the areas of self-sufficiency, independent living, health and wellness, relationships, employment and education, and personal advocacy, with a keen focus on housing procurement, retention, and long-term stability. The Case Manager also provides community-based services throughout the community and at the youth's home to teach and model independent living skills development. This position works within a team of Housing Coordinators and a Resource Specialist ensuring housing procurement and connection to community resources. This position is required to work onsite and, in the community, primarily in San Francisco, Alameda and Contra Costa Counties, Monday - Friday.
Essential Duties and Responsibilities:
Caseload Management
Responsible for providing case management and skills development to participants
Conduct assessment, planning, and implementation of services for assigned participants to identify and accomplish short and long-term goals.
Provide direct participant support weekly including a minimum of bi-weekly face-to-face meetings with participants in response to participant needs.
Create and deliver impactful service plans for participants that focus on achieving outcomes in the areas of wellness, budgeting, long-term housing, education, employment, parenting, relationships and overall health.
Track participants' progress and develop creative ways for participants to practice and strengthen their independent living skills towards self-sufficiency.
Prepare clients to maintain housing after program exit or help them to identify alternative housing options if necessary.
Support participants to develop and practice social emotional learning skills that promote conflict resolution and decision making.
Work collaboratively with Resource Specialist to support participants assess career goals, work towards educational milestones, and obtain and retain employment towards living sustainable wage.
Coach participants on handling work-related issues, promote self-advocacy and increase success in job retention.
Identify, coordinate and form partnerships with critical community connections and resources that can help link participants with community connections, building networks, wellness and career opportunities.
Collaborate effectively with both internal and external teams to coordinate and provide optimum services leveraging participant strengths and responding to participant needs and stated goals.
Help participants move into housing including but not limited to secure furnishings, shop for home necessities, and locate resources in their new community
Ensure that apartments meet cleanliness standards by conducting monthly apartment inspections
Support property management to address serious housing concerns related to TAY
Coordinate community events and workshops to assist TAY with independent living skills
Documentation/Administrative Duties
Attend weekly program meetings, monthly agency staff meetings, and individual weekly supervision with Program Manager
Ensure proper and timely documentation of services including written case notes
Maintain files for caseload and update paperwork in a timely manner
Work collaboratively with colleagues across the organization and within respective department

Qualification and Requirements:
Bachelor's degree in social work, psychology, counselling or equivalent required with Minimum four years of experience working in a child welfare social services setting, with transition age youth, or individuals who have experienced homelessness with complex presentations including chronic trauma OR Master's degree in social work, psychology, counselling or equivalent with minimum two years of experience working in a child welfare social services, with transition age youth, or individuals who have experienced homelessness with complex presentations including chronic trauma. Experience does not substitute for a professional degree and certification.
Must be comfortable working in community settings.

General Qualifications
Criminal History/Background: The County may access criminal history information on candidates who have accepted a conditional offer of appointment for this class consistent with the provisions of Board of Supervisors Resolution No. 82-602, Personnel Policies and Procedures B-5, and applicable federal and state laws. The County shall not consider for employment any candidate who has been convicted of a felony or misdemeanor that relates to or impacts the candidate's ability to perform the job duties of this class unless it is determined that mitigating circumstances exist. For purposes of accessing criminal history information, the candidate will be fingerprinted. A subsequent arrest notification may be obtained.
The County may also conduct a background check on the candidate prior to appointment to a position within this class. The background check may include personal and professional reference checks, credit history checks where authorized by Labor Code section 1024.5, Social Security Number verification, professional license/registration verification, military service information and driving history. LICENSING, CERTIFICATION AND REGISTRATION REQUIREMENTS
* Applicants are required to possess, or have the ability to obtain, a valid California Driver's license, Class C.
* Incumbents are required to possess a current First Aid and Personal Safety certificate or obtain one within 60 days of appointment.
* Incumbents are required to possess a current CPR certificate or obtain one within 60 days of appointment.
* Incumbents have limited Peace Officer powers as delineated in the California Penal Code and therefore are required to complete training under Section 832 of the California Penal Code or equivalent within twelve (12) months of appointment.
* Peace Officers must meet minimum standards concerning citizenship, age, character, education and physical/mental condition as set forth in Section 1031 of the California Government Code.

PHYSICAL REQUIREMENTS

Job Description This is the entry-level class in the Protective Services Worker series.
Adult Protective Services (APS) is a state-mandated program administered by the City and County of San Francisco (CCSF), HSA, and DAS.
Under general supervision by the APS program, the Protective Services Worker (PSW) receives and responds to reports of suspected “elder” and “dependent adult” abuse including neglect, self-neglect, and financial abuse.
The PSW performs professional investigations and case management services to victims of abuse.
This includes activities related to evaluating the level of harm or risk of harm, to the person and the estate of an “elder” or “dependent adult;” assessing biopsychosocial functioning, including the medical condition, cognitive/psychological status, and adequacy of care and support system; orchestrating ongoing services to maximize the use of community resources, and to help protect victims from further harm once the case is closed.

Conducts unannounced visits to see elders and dependent adults in their living environment.

Monitors the Service Plan and follows up through field visits and telephone contacts with clients, relatives, and other concerned individuals until the service goals have been achieved.
Assists with multidisciplinary team meetings with healthcare providers, hospitals, and the Public Health/Mental Health Department, and other City agencies involved to further their understanding of significant social and emotional factors underlying the client's health, social, or environmental problems.
Designs interventions in collaboration with medical professionals, social service providers, law enforcement, or other relevant systems by contributing with information to help determine the level of care and services pertinent to the client's welfare.
Provides education to individuals and agencies on the role and authority of APS.
Conducts public presentations about casework in a confidential, multi-disciplinary setting; collaborates and cooperates with the community and the CCSF partner agencies.
Uses a personal computer to prepare, review, and maintain records, correspondence, and reports; manages case documentation in an APS database; maintains timely email correspondence.
Qualifications 1.
Education: Possession of a Master's Degree in Social Work from a school accredited by the Council on Social Work Education (CSWE); OR Master's Degree from a two (2) year counseling program from an accredited college or university.
(Qualifying Master's Degrees in counseling include Marriage and Family Therapy, Clinical Counseling, Mental Health Counseling, Addiction Counseling and Counseling Psychology that included a supervised field placement as part of the course curriculum); AND 2.
Experience: Satisfactory completion of two semesters of graduate field placement which involved the provision of direct services to elders and/or dependent adults and/or adults with physical, cognitive or mental health disabilities in one of the following or closely related areas: Adult Protective Services, intensive case management such as MSSP/Linkages, medical social work in a hospital or clinic, mental health program/agency, home health agency, or alcohol and drug counseling and treatment program; OR Six (6) months post-baccalaureate professional work experience which involved the provision of direct services to elders, dependent adults, or adults in one of the areas previously described.

Job Description
Under general supervision, positions in class 2940 Protective Services Worker within the Office of the Public Conservator in the Department of Aging and Adult Services provide a broad range of clinical/advocacy services to temporary/permanent conservatees and collateral contact with a multidisciplinary team. In general, an average caseload is approximately 65 clients and can be more or less depending on case intensity and program capacity.

Therefore travel is required in order to meet the needs of the conservatee and our service mandate by State and County regulations. The average number of miles driven per month is 350.
The 2940 is responsible for managing an independent caseload and is assigned mental health (LPS) conservatorship investigations as well. This involves interaction with Mental Health Court, including Public Defender and District Attorney, Department of Public Health, hospitals, jails and community agencies. In addition to assessments and making recommendations for levels of care, the conservator is also responsible for advocating these recommendations, which may at time be at conflict with the interests of other stakeholders.
Basic computer skills are required to perform charting on electronic systems (Avatar and Panoramic), emailing (the bulk of inter and intra agency communication is done through e-mail).
ESSENTIAL DUTIES AND FUNCTIONS

Qualifications
1. Education: Possession of a master's degree in Social Work from a school accredited by the Council on Social Work Education (CSWE);
OR
A Master's Degree from a two-year (2) counseling program from an accredited college or university. (Qualifying Master's Degrees in counseling include Marriage and Family Therapy, Clinical Counseling, Mental Health Counseling, Addiction Counseling and Counseling Psychology that included a supervised field placement as part of the course curriculum).

2. Experience: Satisfactory completion of two semesters of graduate field placement which involved the provision of direct services to elders and/or dependent adults and/or adults with physical, cognitive or mental health disabilities in one of the following or closely related areas: Adult Protective Services, intensive case management such as MSSP/Linkages, medical social work in a hospital or clinic, mental health program/agency, home health agency or alcohol and drug counseling and treatment program

This office manages and provides oversight to all female programs, in addition to seven designated male and female institutions, fire camps and community programs.
The California Department of Corrections Rehabilitation (CDCR) Headquarters has implemented a new automated system to streamline the hiring process. The Human Resources Technology System (HRTS) is an intuitive, user-friendly system that provides the applicant access to real time updates throughout the hiring process.
