Schedule: Monday-Friday 8:00 am - 4:30 pm with other shifts assigned Pay Range: $22.00 - $24.50 / hourly Addiction Treatment Discharge Counselor/Therapist Position Responsibilities: * Coordinates with the various members of the multidisciplinary treatment team in relation to discharge planning. * Provides appropriate referral for follow-up and facilitates referrals for needed services. * Organizes and maintains appointments and phone interviews for ongoing services. * Performs assessments which include the identification of cultural differences in planning for patient care. * Provides person-centered treatment to a diverse patient caseload that could include individual and group counseling. * Incorporates knowledge of evidence based and promising practices into all facets of interaction with patients. * Demonstrates competency in the provision of service. * Formulates diagnostic impressions. * Determines the appropriate level of care. * Document client notes clearly, concisely and according to policy. Addiction Treatment Discharge Counselor/Therapist Position Requirements: * Associate's degree in a related field required. * 3 years of addiction counseling experience. * Years of experience can be substituted with a Bachelor's or Master's degree in a related field. * OASAS QHP or CASAC designation required. Daily inspections of all equipment operations, adjusting or repairing where necessary. * Performs inspection of all heating equipment boiler, fire and water sides, pumps, heating units during the summer month. * Help grounds keeper with snow removal and mowing of grass as needed. * Sheet rock, taping, and painting of walls. Tile work and grouting of walls and floors, wallpapering. * Assists in snaking out sewer lines, upkeep of all plumbing, toilets, sinks, water fountains, etc. * Assist in upkeep of all electrical work and air conditioners. * Wheelchair maintenance as needed. * 3 years' experience in maintenance field Education * High School Diploma or equivalent

* Provide health related tasks under the supervision of a Registered Professional Nurse in a way that is consistent with overall participant safety and accepted standards of participant care. * Assist the participant with the use of medical equipment and self-administered medication as prescribed. * Assess, record and report participants condition as needed including pulse, temperature, BP, signs of discomfort, etc. * Assist with nutritional and fluid intake, documenting and notifying nurses on dietary preferences. - Establishing positive rapport with the participants and ensuring confidentiality. * Provides supervision, companionship and care to individuals with physical and/or cognitive impairment and assist in socialization and recreational activities. * Knowledge of medical instruments and administration of care, common safety hazards and organizational policies regarding patient care. * Ability to react calmly and effectively in emergency situations and communicate clearly. * May perform ambulation and transfer activities under direction. * Approved NY State HHA Training Program or equivalent * First Aid Certification required Education: - High School Diploma/GED

Responsibilities: Under the supervision of the Food Service Director/Supervisor, the food service aide works in conjunction with the cooks in preparing resident meal trays and special dietary requirements, along with serving food to residents, visitors, or staff. * Prepares menu items and delivers trays to residents in floor dining room areas and to those who do not come to the dining room, ensuring their proper diets are administered appropriately. * Responsible for maintaining cleanliness, setting up and clearing dining room tables and serving carts. * Observes residents and reports complaints, problems, or anything out of the ordinary to supervisor. * Assists in receiving and storing incoming food and supplies. Responsibilities: * Plans, organizes, and conducts a comprehensive program of individual and group nutrition services including provision of nutrition needs of the participant, clinical guidance, nutrition education and counseling to meet normal and therapeutic needs. * Evaluates nutritional status of individual participants utilizing nutritional guidelines, professional knowledge, and interviewing skills to provide quality care. * Develops and implements nutritional plans, monitors participant's progress and recommends/revises changes in nutritional plan for effective participant care. * Documents changes in the participant's condition and details care provided by completing all required documentation and ensuring compliance to standards and policies. * Coordinates with other members of the interdisciplinary team to ensure that the comprehensive plan of care is followed and revised as needed. * Provides nutrition counseling to participants and/or caregivers utilizing educational materials, modified diets, and teaching aids to ensure participants understanding of the plan of care and its implications. * Oversees the work of Geriatric Caregivers in the kitchen by working with Food Services Manager to recommend solutions regarding specific dietary needs.
